STUDENT VISA CHECKLIST
CONSULATE OF SPAIN IN HOUSTON
YOU MUST APPLY FOR THIS VISA, IN PERSON,
BETWEEN MAY 20 AND JUNE 14, 2013
Use this checklist if:
Your UNIVERSITY (as shown on your Student ID) or PERMANENT RESIDENCE (as
shown on your Drivers License) is located in AR, AL, LA, MS, NM, OK, TN and TX.
You MUST apply for your Student Visa IN PERSON to the Houston Consulate by May 20 and June 14,
2013. (Students from AR, AL, LA, MS, NM, OK, TN can use a visa processing service to help with the
issuance of their visa – call phone number 620-461-7564).

Important Notes:
• If you are a Full Year student, or plan on staying for another semester (180+ days) please see
additional instructions (10-11) at the end of the checklist.
• If you plan on traveling outside the USA over the summer, please contact Lisa Anthony
(lanthony@syr.edu) immediately for additional instructions.
You must submit all documents in the check list below:
1. Original Passport AND Photocopy of Passport ID page: valid until at least June 2014 (fall
students) and December 2014 (yearlong students). Passports must have at least one blank page to
affix the visa. Passports must be signed!
In addition to a national passport, Non-US citizens should provide I-20 /IAP-66 or Alien
Registration Card in and a permit to return to the United States of America in the form of: a
resident card, or a student/work visa that is stamped on the passport and is valid for a period of
three months beyond the expected length of stay in the Schengen area, or the period of validity of
the visa, whichever is longer. (Photocopies required as well).
*Before submitting your passport, make an extra copy for your own records! If your passport
expires before June 2014 (fall students) and December 2014 (yearlong students), you will need
to renew it using an expedited service. Details of this can be found here:

2. Identification Card and photocopy: This ID must show that your permanent residence OR school
address falls within the Houston Consulate jurisdiction. Forms of ID which are accepted are: US
Driver’s License, US State ID Card, or Student ID.

3. 2 National Application Forms (Duly Completed): Application Forms must be completed in blue or
black ink, and in print (not cursive). Application forms must be signed (please find application form
instructions below).
4. 2 color passport sized photos: Passport photos can be taken at any Kinko’s, Walgreens,
CVS, etc. Photos must be RECENT with white background and professionally taken.
Photos must be GLUED to each application form in the top right corner. DO NOT
STAPLE PHOTOS TO APPLICATION FORMS!
5. Letter of acceptance as a full time student from Spain's University/School or US program: You
MUST contact Lisa Anthony for these letters, at least two weeks prior to your visa
appointment so they may be sent to you.
6. Proof of financial means during your stay. SU Abroad will provide the following:
• Letter from the University or School in Spain or in the USA assuming full financial
responsibility during your stay.
7. Proof of Health Insurance: Provide Original Letter guaranteeing full International Health
insurance at a minimum of €30,000 Euros, including the necessary Repatriation Coverage
for the total period of stay in Spain. Please contact Lisa Anthony at least two weeks prior
to your visa appointment at lanthony@syr.edu, if you need like SU Abroad to provide
you with a letter and proof of health insurance.
8. US Postal Service Money Order payable to the “Consulate General of Spain” for $160.00
(US citizens), $78.60 (most non-US citizens). For nationals of Russia, Macedonia,
(ARYM), Montenegro, Serbia, Albania and Moldavia the fee is $45.85. Do not submit
cash, personal checks, or bank checks because the Consulate will not accept them. Money
Orders can be purchased at any US Post Office.

Academic Year Students (180+ days) ONLY:
9. Medical Certificate (original and photocopy): Doctor’s recent statement (no older than 3
months) in doctor’s or medical center letterhead, stating: “The applicant has been examined
and found in good physical and mental health to travel to study abroad and is free of
contagious diseases susceptible of quarantine and drug addiction or any other illnesses
which could lead to Public Health repercussions according to the International Sanitary
Regulations of 2005”.
*The original must be translated into Spanish and submitted along with a photocopy,
contact a legal translator.
10. Certification of Absence of Police Records (original and photocopy): FBI or State
Police Criminal Background. Must be legalized with the “Apostille of the Hague
Convention” from the U.S. A police record must be obtained (criminal background check)
from all non EU countries where you have lived for the past five years for more than 6
months, legalized by the Spanish Consulate or Embassy in the respective countries. If the
Certificate is issued outside the USA it must be duly legalized by the corresponding
Consulate of Spain. LOCAL POLICE BACKGROUND CHECK WILL NOT BE
ACCEPTED.

I declare that to the best of my knowledge and belief all particulars supplied by me are correct and complete. I
am aware that any false statements shall lead to my application being refused or to the annulment of
a visa already granted.
I am aware that once the visa request has been submitted, a copy of this form, stamped to
indicate the date and place of reception, shall be returned to the applicant and that arrangements
may be made with the applicant as to the channel for requesting corrections or the supply of documents or certificates
required, as well as for serving notice of summons and for notifying him/her of the decision adopted.
Notices and requests shall be made by telephone or fax to the contact number provided by the
interested party or his legal representative. If these go unanswered, they shall be served
in writing to the domicile set out in the application, which must be located within the consular
district.
Summons and requests served must be complied with within a period not exceeding ten days, unless
personal attendance is required, in which case the deadline is fifteen days.
Once all the possibilities of serving a notice have been exhausted, it shall be carried out by
means of an announcement posted on the relevant notice board of the Consular Post for
ten days.
If no reply is received to a request or summons within the time period, the applicant shall be deemed to have
withdrawn his/her request and he/she shall be notified of the decision establishing this to be the case.
Decisions on a visa application shall be notified within a maximum period of one month, which cannot be extended,
counting from the day following the date the application was submitted at the competent Consular Post for processing,
except in the case of residence visas without work permit in which case the maximum period shall be three months. In
this latter case, the application for the relevant residence authorisation which must be issued by the corresponding
Government Department Office or Provincial Government Office shall put a hold on this time period until this
authorisation is notified to the Consular Post.

Once the visa is granted it must be collected within a month, unless it is a visa for a family reunion or a
visa for study purposes; in these cases, the period for collection is two months. If a visa is not
collected within the aforesaid periods, the applicant shall be deemed to have renounced the visa
granted and the application and accompanying documentation will be archived.
