YOU MUST APPLY FOR THIS VISA, IN PERSON,<br />

BETWEEN MAY 20 AND JUNE 14, 2013<br />

Use this <strong>checklist</strong> if:<br />

Your UNIVERSITY (as shown on <strong>you</strong>r Student ID) or PERMANENT RESIDENCE (as<br />

shown on <strong>you</strong>r Drivers License) is located <strong>in</strong> AR, AL, LA, MS, NM, OK, TN and TX.<br />

You MUST apply for <strong>you</strong>r Student Visa IN PERSON to the Houston Consulate by May 20 and June 14,<br />

2013. (Students from AR, AL, LA, MS, NM, OK, TN can use a <strong>visa</strong> process<strong>in</strong>g service to help with the<br />

issuance <strong>of</strong> their <strong>visa</strong> – call phone number 620-461-7564).<br />

Important Notes:<br />

• If <strong>you</strong> are a Full Year <strong>student</strong>, or plan on stay<strong>in</strong>g for another semester (180+ days) please see<br />

additional <strong>in</strong>structions (10-11) at the end <strong>of</strong> the <strong>checklist</strong>.<br />

• If <strong>you</strong> plan on travel<strong>in</strong>g outside the USA over the summer, please contact Lisa Anthony<br />

(lanthony@syr.edu) immediately for additional <strong>in</strong>structions.<br />

You must submit all documents <strong>in</strong> the check list below:<br />

1. Orig<strong>in</strong>al Passport AND Photocopy <strong>of</strong> Passport ID page: valid until at least June 2014 (fall<br />

<strong>student</strong>s) and December 2014 (yearlong <strong>student</strong>s). Passports must have at least one blank page to<br />

affix the <strong>visa</strong>. Passports must be signed!<br />

In addition to a national passport, Non-US citizens should provide I-20 /IAP-66 or Alien<br />

Registration Card <strong>in</strong> and a permit to return to the United States <strong>of</strong> America <strong>in</strong> the form <strong>of</strong>: a<br />

resident card, or a <strong>student</strong>/work <strong>visa</strong> that is stamped on the passport and is valid for a period <strong>of</strong><br />

three months beyond the expected length <strong>of</strong> stay <strong>in</strong> the Schengen area, or the period <strong>of</strong> validity <strong>of</strong><br />

the <strong>visa</strong>, whichever is longer. (Photocopies required as well).<br />

*Before submitt<strong>in</strong>g <strong>you</strong>r passport, make an extra copy for <strong>you</strong>r own records! If <strong>you</strong>r passport<br />

expires before June 2014 (fall <strong>student</strong>s) and December 2014 (yearlong <strong>student</strong>s), <strong>you</strong> will need<br />

to renew it us<strong>in</strong>g an expedited service. Details <strong>of</strong> this can be found here:<br />

2. Identification Card and photocopy: This ID must show that <strong>you</strong>r permanent residence OR school<br />

address falls with<strong>in</strong> the Houston Consulate jurisdiction. Forms <strong>of</strong> ID which are accepted are: US<br />

Driver’s License, US State ID Card, or Student ID.


3. 2 National Application Forms (Duly Completed): Application Forms must be completed <strong>in</strong> blue or<br />

black <strong>in</strong>k, and <strong>in</strong> pr<strong>in</strong>t (not cursive). Application forms must be signed (please f<strong>in</strong>d application form<br />

<strong>in</strong>structions below).<br />

4. 2 color passport sized photos: Passport photos can be taken at any K<strong>in</strong>ko’s, Walgreens,<br />

CVS, etc. Photos must be RECENT with white background and pr<strong>of</strong>essionally taken.<br />

Photos must be GLUED to each application form <strong>in</strong> the top right corner. DO NOT<br />

STAPLE PHOTOS TO APPLICATION FORMS!<br />

5. Letter <strong>of</strong> acceptance as a full time <strong>student</strong> from Spa<strong>in</strong>'s University/School or US program: You<br />

MUST contact Lisa Anthony for these letters, at least two weeks prior to <strong>you</strong>r <strong>visa</strong><br />

appo<strong>in</strong>tment so they may be sent to <strong>you</strong>.<br />

6. Pro<strong>of</strong> <strong>of</strong> f<strong>in</strong>ancial means dur<strong>in</strong>g <strong>you</strong>r stay. <strong>SU</strong> <strong>Abroad</strong> will provide the follow<strong>in</strong>g:<br />

• Letter from the University or School <strong>in</strong> Spa<strong>in</strong> or <strong>in</strong> the USA assum<strong>in</strong>g full f<strong>in</strong>ancial<br />

responsibility dur<strong>in</strong>g <strong>you</strong>r stay.<br />

7. Pro<strong>of</strong> <strong>of</strong> Health Insurance: Provide Orig<strong>in</strong>al Letter guarantee<strong>in</strong>g full International Health<br />

<strong>in</strong>surance at a m<strong>in</strong>imum <strong>of</strong> €30,000 Euros, <strong>in</strong>clud<strong>in</strong>g the necessary Repatriation Coverage<br />

for the total period <strong>of</strong> stay <strong>in</strong> Spa<strong>in</strong>. Please contact Lisa Anthony at least two weeks prior<br />

to <strong>you</strong>r <strong>visa</strong> appo<strong>in</strong>tment at lanthony@syr.edu, if <strong>you</strong> need like <strong>SU</strong> <strong>Abroad</strong> to provide<br />

<strong>you</strong> with a letter and pro<strong>of</strong> <strong>of</strong> health <strong>in</strong>surance.<br />

8. US Postal Service Money Order payable to the “Consulate General <strong>of</strong> Spa<strong>in</strong>” for $160.00<br />

(US citizens), $78.60 (most non-US citizens). For nationals <strong>of</strong> Russia, Macedonia,<br />

(ARYM), Montenegro, Serbia, Albania and Moldavia the fee is $45.85. Do not submit<br />

cash, personal checks, or bank checks because the Consulate will not accept them. Money<br />

Orders can be purchased at any US Post Office.<br />

Academic Year Students (180+ days) ONLY:<br />

9. Medical Certificate (orig<strong>in</strong>al and photocopy): Doctor’s recent statement (no older than 3<br />

months) <strong>in</strong> doctor’s or medical center letterhead, stat<strong>in</strong>g: “The applicant has been exam<strong>in</strong>ed<br />

and found <strong>in</strong> good physical and mental health to travel to study abroad and is free <strong>of</strong><br />

contagious diseases susceptible <strong>of</strong> quarant<strong>in</strong>e and drug addiction or any other illnesses<br />

which could lead to Public Health repercussions accord<strong>in</strong>g to the International Sanitary<br />

Regulations <strong>of</strong> 2005”.<br />

*The orig<strong>in</strong>al must be translated <strong>in</strong>to Spanish and submitted along with a photocopy,<br />

contact a legal translator.<br />

10. Certification <strong>of</strong> Absence <strong>of</strong> Police Records (orig<strong>in</strong>al and photocopy): FBI or State<br />

Police Crim<strong>in</strong>al Background. Must be legalized with the “Apostille <strong>of</strong> the Hague<br />

Convention” from the U.S. A police record must be obta<strong>in</strong>ed (crim<strong>in</strong>al background check)<br />

from all non EU countries where <strong>you</strong> have lived for the past five years for more than 6<br />

months, legalized by the Spanish Consulate or Embassy <strong>in</strong> the respective countries. If the<br />

Certificate is issued outside the USA it must be duly legalized by the correspond<strong>in</strong>g<br />

Consulate <strong>of</strong> Spa<strong>in</strong>. LOCAL POLICE BACKGROUND CHECK WILL NOT BE<br />

ACCEPTED.<br />

I am aware that once the <strong>visa</strong> request has been submitted, a copy <strong>of</strong> this form, stamped to<br />

<strong>in</strong>dicate the date and place <strong>of</strong> reception, shall be returned to the applicant and that arrangements<br />

may be made with the applicant as to the channel for request<strong>in</strong>g corrections or the supply <strong>of</strong> documents or certificates<br />

required, as well as for serv<strong>in</strong>g notice <strong>of</strong> summons and for notify<strong>in</strong>g him/her <strong>of</strong> the decision adopted.<br />

Notices and requests shall be made by telephone or fax to the contact number provided by the<br />

<strong>in</strong>terested party or his legal representative. If these go unanswered, they shall be served<br />

<strong>in</strong> writ<strong>in</strong>g to the domicile set out <strong>in</strong> the application, which must be located with<strong>in</strong> the consular<br />

district.<br />

Summons and requests served must be complied with with<strong>in</strong> a period not exceed<strong>in</strong>g ten days, unless<br />

personal attendance is required, <strong>in</strong> which case the deadl<strong>in</strong>e is fifteen days.<br />

Once all the possibilities <strong>of</strong> serv<strong>in</strong>g a notice have been exhausted, it shall be carried out by<br />

means <strong>of</strong> an announcement posted on the relevant notice board <strong>of</strong> the Consular Post for<br />

ten days.<br />

If no reply is received to a request or summons with<strong>in</strong> the time period, the applicant shall be deemed to have<br />

withdrawn his/her request and he/she shall be notified <strong>of</strong> the decision establish<strong>in</strong>g this to be the case.<br />

Decisions on a <strong>visa</strong> application shall be notified with<strong>in</strong> a maximum period <strong>of</strong> one month, which cannot be extended,<br />

count<strong>in</strong>g from the day follow<strong>in</strong>g the date the application was submitted at the competent Consular Post for process<strong>in</strong>g,<br />

except <strong>in</strong> the case <strong>of</strong> residence <strong>visa</strong>s without work permit <strong>in</strong> which case the maximum period shall be three months. In<br />

this latter case, the application for the relevant residence authorisation which must be issued by the correspond<strong>in</strong>g<br />

Government Department Office or Prov<strong>in</strong>cial Government Office shall put a hold on this time period until this<br />

authorisation is notified to the Consular Post.<br />

Once the <strong>visa</strong> is granted it must be collected with<strong>in</strong> a month, unless it is a <strong>visa</strong> for a family reunion or a<br />

<strong>visa</strong> for study purposes; <strong>in</strong> these cases, the period for collection is two months. If a <strong>visa</strong> is not<br />

collected with<strong>in</strong> the aforesaid periods, the applicant shall be deemed to have renounced the <strong>visa</strong><br />

granted and the application and accompany<strong>in</strong>g documentation will be archived.<br />

I am aware that the <strong>visa</strong> fee is not refundable if the <strong>visa</strong> is refused<br />
