INSTRUCTION FOR COMPLETING THE CERTIFICATE OF CONFORMITY<br />
1. Part description Name of the part as it appears on the drawing<br />
2. Part number The first 10 chars of the part ID.<br />
3. Part ID The LHC part identifier (see LHC-PM-QA-206.00).<br />
4. Serial number: The last 8 chars of the part ID.<br />
5 Other identification Optional identification of the part, .e.g. the manufacturer serial number.<br />
6. Recipient contractor: Organisation/firm to which the part(s) are supplied.<br />
7. Contract/order No: <strong>CERN</strong>'s contract or order No.<br />
8. Responsible person at <strong>CERN</strong>: The Project Engineer or a member of his team in charge of the contract/order.<br />
9. Reference specification: A specification or any other document, e.g. a standard, defining the characteristics<br />
of the part.<br />
10. Reference drawings: Drawing number and revision index of related drawings.<br />
11. Part manufactured by: Name of the firm that made/supplied the part.<br />
12. Acceptance test reference: Document number of test procedure.<br />
13. Acceptance test results Results of the tests on reception of parts at the contractor/firm site.<br />
14. Comments: Any other additional information/instruction regarding the part.<br />
