The PRMC President’s Leadership Award honors<br />
individuals who embrace the philosophy that<br />
leadership is a privilege and a position of shared trust.<br />
With that philosophy also comes accountability and<br />
ownership.<br />
Carol Deal, Executive Director, Surgical Services<br />
Deal, who has been with PRMC since 1995, was<br />
recognized for exceeding her goals, being a fair,<br />
equitable and driven leader who accepts<br />
responsibilities and brings proactive solutions to her<br />
issues. Deal was also recognized for her leadership of<br />
the ongoing operating room expansion project.<br />
Roger Follebout, Jr., Director, Community Relations<br />
Follebout, a member of the PRMC family since 1998,<br />
was recognized for his leadership in dramatically<br />
improving community and media relations during his<br />
tenure as Director, which began in August of 2009.<br />

Late Preterm Infant Initiative<br />
The team was recognized for reducing the percentage of<br />
Neonatal ICU admissions and Pediatric readmissions for late<br />
preterm infants (between 34 & 36.6 weeks). After process<br />
changes using evidenced-based education and clinical<br />
guidelines, Neonatal ICU admissions were reduced from 33%<br />
to 22% and a readmission rate to Pediatrics was reduced<br />
from 11% to 0% within 10 days of birth.<br />
Ventilator Associated Pneumonia Reduction<br />
The team was recognized for reducing Ventilator<br />
Associated Pneumonia, one of the most common infections<br />
acquired in hospital Intensive Care Units (ICU). Using or<br />
reintroducing nearly 20 evidence-based care measures, the<br />
ICU team has achieved zero Ventilator Associated<br />
Pneumonias for <strong>Medical</strong>/Surgical patients in the ICU for 13<br />
consecutive months and counting.<br />

Clinical Documentation Improvement-Acute Pulmonary<br />
Edema/Respiratory Failure<br />
The team was recognized for a change in the way Acute<br />
Pulmonary Edema/Respiratory Failure patients were<br />
documented. The inaccurate information has impacted<br />
PRMC’s performance rating for potentially preventable<br />
hospital-acquired complications. Once a new<br />
documentation template was created to more accurately<br />
reflect post-operative complications, the incidence of Acute<br />
Pulmonary Edema/Respiratory Failure dropped from 97%<br />
above expected to 12% below expected.

So here we are, around a month into a<br />
new year, and hopefully we’re still<br />
committed to most of our New Year’s<br />
resolutions. It’s a challenge, isn’t it Getting<br />
to and maintaining a state of “always” in our<br />
lives can be the toughest thing we have to<br />
do—personally and professionally. But as I<br />
like to remind our team, WHEN IT COMES<br />
TO PATIENT CARE there is no substitute for<br />
always.<br />
At <strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong> <strong>Center</strong>,<br />
our goal is to continue to weave that into<br />
our culture—“A Culture of Always.” As a<br />
healthcare team, we must ensure that we<br />
are doing everything we can to always get it<br />
right for every patient, every person, every<br />
time. We’re not perfect, but much of this<br />
journey is about learning how to improve so<br />
that our partnership is built on the most<br />
solid of foundations—trust. We trust that<br />

We clearly understand that it’s an earned<br />
trust. We have to prove to you every day that<br />
we stand by our commitment and<br />
partnership. We take great pride in doing<br />

For instance, in a recent Premier, Inc.<br />
Medicare/Medicaid study, PRMC received<br />
Top Improvement awards in heart attack,<br />
heart failure and surgical care. The study<br />
found that the lives of 149 heart attack<br />
patients here were saved over the past six<br />
years due to the quality and reliability of our<br />
care. Improvements in the way we diagnose<br />
and rapidly treat severe sepsis has reduced<br />
our mortality rate in that condition alone<br />

by 39%—dozens of lives saved. And relative<br />
to heart conditions, within 90 minutes—the<br />
accepted gold standard—anyone<br />
presenting in our Emergency Department<br />
requiring a heart catheterization is<br />
receiving a cath 94% of the time within that<br />
window; and we’ve reduced the median<br />
time to just 62 minutes. For over a year<br />
now, there have been ZERO Ventilator<br />
Associated Pneumonia cases in our ICU<br />
and ZERO readmissions to our Pediatric<br />
Unit within 10 days of birth for late-term<br />
babies born prematurely, resulting in a 22%<br />
reduction in Neonatal ICU admissions.<br />

Gavin Stephens is our 16-year-old son who had just<br />
returned from a High Adventure Boy Scout trip on July 8,<br />
2011 and began complaining of discomfort when he took<br />
deep breaths. After taking him to his pediatrician and seeing<br />
that the initial x-rays were clear, it was thought to have been a<br />
possible pulled or inflamed chest muscle due to a recent<br />
injury. Because Gavin, a high school sophomore at James M.<br />
Bennett High School in Salisbury, is very active in a variety of<br />
sports, anything more serious was not a concern (Gavin plays<br />
three sports—football, wrestling & lacrosse and participated<br />
in an adult triathlon in May 2011).<br />
Over a two-week period, Gavin increasingly complained of<br />
chest pain, mostly at night, and on July 24th it became so<br />
severe we took him to the PRMC Emergency Department at<br />
3:00 a.m. Once evaluated by the PRMC Emergency<br />
Department team (vitals and an echocardiogram were<br />
done), we were told Gavin had a large amount of fluid<br />
around his heart—a pericardial effusion, creating<br />
tremendous pressure on his heart. Cardiologist Steven<br />
Hearne, MD was called in to evaluate and confirmed Gavin’s<br />
condition. The fluid buildup was so severe that Dr. Hearne<br />
performed a pericardiocentesis in the Emergency<br />
Department to drain approximately 800cc’s of fluid from<br />
around the heart.<br />
Children’s National <strong>Medical</strong> <strong>Center</strong> in Washington, DC<br />
was notified and Gavin was immediately transported there,<br />
where he spent 6 days (3 days in ICU) for intensive testing<br />
and consults with medical specialists. A diagnosis was not<br />
found, and the cause was attributed to a virus. Gavin was<br />

Overall Cardiology Services in <strong>2012</strong>.<br />
released with prescribed medications to keep the<br />
inflammation and fluid down, and further follow-up was<br />
scheduled. Gavin is fortunate that there was no permanent<br />
damage to his heart, or any other organs, thanks to the rapid<br />
response of Dr. Hearne and so many others.<br />
Gavin has experienced a few minor setbacks since July<br />
24th (a fever and some reoccurrence of fluid), and we have<br />
found ourselves in the PRMC Emergency Department twice<br />
since. I was so pleased to see the ease at which PRMC staff<br />
was able to connect with Children’s National staff to<br />
communicate quickly to provide follow-up medical care.<br />
Gavin is recovering and continues to work with his<br />
pediatrician and Children’s National specialists to determine<br />
the possible cause. I am so grateful for the quick response of<br />
the PRMC Emergency Department staff and Dr. Hearne for<br />
stabilizing Gavin during this very serious situation. The<br />
recent affiliation of PRMC with Children’s National has been<br />
a tremendous asset for Gavin and our family, and the<br />
teamwork among the various physicians has been<br />
outstanding.<br />
As a parent, you never imagine a trip in the middle of the<br />
night to the Emergency Department can result in the need<br />
for such serious medical care. We never know when<br />
emergencies may happen, but we’re so thankful the talented<br />
people and processes were in place to respond when we<br />
needed it. Thank you PRMC and your Emergency<br />
Department team!<br />
“They Never Gave Up On Me”—Bruce Somers<br />
and an Exceptional PRMC Experience<br />
Some of the members of the PRMC ICU and Emergency/Trauma teams<br />
who cared for Bruce Somers join him and fiancé Tanna Johnson, center<br />
front, outside the PRMC Emergency/Trauma <strong>Center</strong>.<br />
Bruce Somers of Marion Station is a pretty fortunate guy.<br />
He was once described by Emergency Department<br />
physician Clark Willis, MD, as “the patient closest to death<br />
that I’ve ever treated who still made it through.”<br />
It was June 20, 2009; Bruce Somers’ worst day, but as fate<br />
would have it, maybe his luckiest too, because he landed in<br />
the arms of a talented PRMC trauma team that simply<br />
refused to let him go. “People and other doctors will ask us<br />
when we went to Shock Trauma and we tell them never,” said<br />
Tanna Johnson, Bruce’s fiancé. “Everyone is amazed because

<strong>Medical</strong> <strong>Center</strong> one of America’s 100 Best<br />
Hospitals for Critical Care in <strong>2012</strong>. PRMC was also<br />
awarded the HealthGrades Critical Care<br />
Excellence Award for the 4th consecutive year.<br />
of the severity of Bruce’s injuries, but we know there’s no way<br />
he would have received any better care on the other side of<br />
the bridge or anywhere else.”<br />
It was in Bishopville, Maryland a little over two years ago<br />
now when the driver of a large box truck ran a red light. In<br />
the middle of that intersection, as fate would have it, was<br />
Bruce Somers, who was driving a smaller truck delivering a<br />
load of crab pots. The crash was ugly…a t-bone impact.<br />
Bruce arrived at PRMC by helicopter where, based on the<br />
critical nature of his condition as relayed by the EMS crew,<br />
he was designated as a Red Code trauma patient—the<br />
category reserved for the most critically injured trauma<br />
patients. Given this designation, a trauma surgeon, three<br />
Emergency Department physicians, Emergency Department<br />
nursing and tech staff, lab, and respiratory personnel were all<br />
awaiting his arrival. When wheeled into the trauma bay,<br />
Bruce had multiple obvious and visible injuries and a nearly<br />
amputated left arm.<br />
While the team was stitching, stapling, and compressing,<br />
x-rays were obtained and Bruce was taken to the Emergency<br />
Department’s CT scanner for further evaluation of his<br />
internal injuries. While this evaluation was in progress, Bruce<br />
was getting unit after unit of blood and human plasma<br />
through a rapid infusion machine in an attempt to correct his<br />
prior and continued blood loss.<br />
Radiology tests confirmed what doctors feared. Bruce had<br />
a horrific array of trauma injuries in addition to those already<br />
known, including bleeding around and inside the brain in<br />
multiple areas, extensive facial fractures, fractures of his<br />
neck, upper and lower back with multiple fractures in each<br />
site. He had also suffered a lung contusion, bleeding inside<br />
his chest, 10 fractured ribs, bleeding in his abdomen<br />
including bleeding from his right kidney and liver, multiple<br />
pelvic fractures and a complex right hip socket fracture.<br />
Such an extensive and complex degree of injuries in one<br />
patient is sobering to even to a seasoned trauma team like<br />
the one at PRMC. With game faces on, they worked feverishly<br />
to save Bruce’s life, but privately there was the realization<br />
that he was facing a very difficult challenge; one that very few<br />
people in his condition survive. “It was touch and go for a<br />
long time,” said Tanna. “We’re just so glad that everyone was<br />
there and knew what to do.”<br />
Following this evaluation, a decision was made to take<br />
Bruce for immediate surgery in a further attempt to stop his<br />
internal bleeding. Amazingly, he was still alive, and now<br />
considered stable enough for transport to the operating<br />
room.<br />
In the OR, Bruce had Trauma Damage Control Surgery,<br />
where the goal is to stop and repair major bleeding only. This<br />
style of trauma surgery has an emphasis on limiting the time<br />
a patient spends on the table and under general anesthesia,<br />
with minor injuries to be corrected at a later time. Bruce was<br />
then taken to the ICU for further stabilization.<br />
Bruce continued to stabilize in the ICU and returned to the<br />
operating room multiple times in the following days and<br />
weeks for various corrective surgeries.<br />
Bruce’s care team grew to include specialists from<br />
Neurosurgery, Orthopaedic Surgery, Ophthalmology, Plastic<br />
Surgery and Rehab Medicine in addition to Anesthesia and<br />
the Trauma Surgeon who initiated his operating room<br />
journey.<br />
Bruce would spend 43 days in PRMC’s ICU before being<br />
transferred to Bryn Mawr Rehab in Pennsylvania, where he<br />
would spend an additional 10 weeks, ultimately awakening<br />
from his coma.<br />
Bruce then transitioned to home with family support for<br />
extensive home care and therapy despite his inability to walk<br />
at that time. After a tremendous amount of effort and hard<br />
work, he was able to walk unassisted only six months after<br />
his accident. “There’s no doubt in our minds whatsoever that<br />
the people at PRMC gave Bruce a second chance at life,”<br />
added Tanna.<br />
Today Bruce walks, laughs and converses with surprising<br />
ease. He has recovered much of his former function,<br />
although he’s limited to lifting no more than 20 pounds, and<br />
is currently working on getting his driver’s license back. “I’ve<br />
got an ‘05 Road King, a 730-pound Harley that I’m hoping to<br />
get converted to a trike so I can ride again,” said Bruce. “I<br />
can’t do everything I could before the accident, but I feel<br />
good and I can’t thank everybody enough.”<br />
Getting “Back” to Life<br />
Ralph Greene takes the first ride on his Harley Davidson motorcycle since<br />
endoscopic spine surgery at PRMC. Joining him outside the <strong>Peninsula</strong><br />
Orthopaedic Associates, P.A. office in Salisbury is orthopaedic spine<br />
surgeon Scott McGovern, MD.<br />
Passion and dedication. 68-year-old Ralph Greene of<br />
Nanticoke, Maryland will tell you he has both. His golf<br />
game—at a 30 handicap—is equal parts dedication and<br />
frustration. His passion, however, is his Harley, which he’s<br />
ridden for 53 years and in all 50 states.<br />

Just not much the past year.<br />
“The pain was so severe it literally brought me to tears. I<br />
never left the house, canceled vacations, couldn’t drive,<br />
couldn’t ride the bike…I didn’t want to take a shower it hurt<br />
so bad,” said Ralph of the severe back, groin and leg pain that<br />
from October 2010 to May 2011 left him absolutely unable to<br />
do anything. “It was awful, I couldn’t stand it.”<br />
It was most likely a fall a decade ago from a ladder and an<br />
awkward landing that caused the bulging disc that was now<br />
controlling and crippling Ralph’s life. When strong pain<br />
medications and injections failed, he turned to orthopaedic<br />
spine surgeon Dr. Scott McGovern, to discuss surgical<br />
options. “Surgery should always be the last option for<br />
treating spinal disorders,” added Dr. McGovern. “Our goal is<br />
to provide patients with the safest, best outcomes possible,<br />
and for Ralph surgery was not only the last option, it was the<br />
absolute best option.”<br />
Dr. McGovern assured Ralph he was confident he could<br />
correct the problem and that he was a strong candidate for<br />
endoscopic spine surgery at <strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong><br />
<strong>Center</strong>.<br />
The surgery took about an hour. Ralph was awake the<br />
entire time under just a local anesthetic. Instead of an open,<br />
larger incision, Dr. McGovern inserted a specialized high<br />
resolution camera and customized surgical instruments<br />
through just one very small incision to repair Ralph’s disc<br />
problem. The camera allowed him to see inside the spine,<br />
and the instruments to fix the problem with much less stress<br />
on surrounding tissue and bone versus an open procedure.<br />
“Although certain patients will benefit more from a<br />
traditional surgery, minimally invasive and endoscopic<br />
surgery can replace most traditional open surgeries without<br />
being minimally effective,” said Dr. McGovern. “We’re<br />
fortunate at PRMC to be a forerunner in this technology.” For<br />
patients, that translates into less blood loss from a single,<br />
small incision, faster recovery, less pain and a safer surgery.<br />
Just six hours after arriving PRMC, Ralph was on his way<br />
back home to Nanticoke, and pain free. “Dr. McGovern and<br />
this surgery have given me my life back, no question about<br />
it,” added Ralph. “I’m flabbergasted. I can’t believe this has<br />
been so successful. Even today I expect to wake up and find<br />
out this isn’t real.” Next mission, getting that handicap into<br />
the 20’s.<br />

Home is Where the Heart “Care” is<br />
John Hauswald on the beach in Ocean City, Maryland<br />
“You need bypass surgery. You have an abdominal<br />
aortic aneurysm. Both require immediate surgical<br />
attention.”<br />
That’s exactly what John Hauswald of Ocean City heard<br />
from his cardiologist while wintering in Florida a few years<br />
ago.<br />
John, 83 at the time, faced some tough choices. So he<br />
turned to someone he trusted—his daughter, Carol Wright of<br />
Salisbury. “Carol said my wife Elsa and I should fly home,<br />
have the surgery here and recuperate near her,” added John.<br />
“She told us <strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong> <strong>Center</strong> was well<br />
respected throughout the country as a premier, exceptional<br />
heart hospital. That’s all we needed to hear.”<br />
Cardiothoracic surgeon Nicholas Ogburn, MD performed<br />
quadruple bypass surgery, and a month later repaired John’s<br />
abdominal aortic aneurysm using minimally invasive<br />
techniques. “It couldn’t have been better from start to finish;<br />
it was excellent,” shared John. “Everyone, and I mean<br />
everyone, was wonderful. It might sound strange to say they<br />
made my heart surgery and recovery seem easy, but they<br />
did.”<br />
John says he feels great today, still plays golf and enjoys—<br />
now more than ever—life with Elsa including Summers in<br />
Ocean City, winters in Florida and a year-round healthy<br />
outlook thanks to the Guerrieri Heart & Vascular Institute<br />
team at <strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong> <strong>Center</strong>.<br />

New Hope for Delmarva Stroke Patients<br />
Time is of the Essence in Stroke Care—every<br />
passing minute after stroke symptoms appear makes<br />
them harder to overcome.<br />
More than 70% of people who display symptoms will<br />
suffer a stroke within 48 to 72 hours if they are not treated<br />
rapidly and properly with medication that works within a<br />
three-hour window of opportunity from the first sign of<br />
those symptoms.<br />
But a new procedure performed in the Interventional<br />
Radiology Department at <strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong><br />
<strong>Center</strong> doubles that traditional three-hour window for<br />
treatment. Mechanical thrombectomy is a state-of-the-art,<br />
minimally invasive procedure that allows doctors to<br />
remove a blood clot up to six hours after the start of stroke<br />
symptoms. In mechanical clot removal, specially trained<br />
Interventional Radiologists guide a tiny catheter from the<br />
femoral artery (in the leg) to the site of the blood clot, grab<br />
it and remove it from the body. Blood flow resumes<br />
immediately, minimizing the effects of the stroke.<br />
<strong>Peninsula</strong> <strong>Regional</strong> is the only hospital on Delmarva<br />
currently performing the procedure. <strong>Peninsula</strong> <strong>Regional</strong> is<br />
fortunate to have three Interventional Radiologists who<br />
are privileged to perform this procedure. They are Michael<br />
Marks, MD, William Reid, MD and Andrew Vennos, MD.<br />

The Stroke Team at <strong>Peninsula</strong> <strong>Regional</strong> treats on<br />
average nearly 40 patients with stroke symptoms every<br />
month, volumes that rival centers like Johns Hopkins in<br />
Baltimore and others in much larger, urban areas.<br />
The Primary Stroke <strong>Center</strong> at <strong>Peninsula</strong> <strong>Regional</strong><br />
<strong>Medical</strong> <strong>Center</strong> is one of 17 hospitals in Maryland and the<br />
only hospital on the Eastern Shore to achieve and<br />
maintain the Joint Commission Gold Seal of Approval for<br />
certification as a Primary Stroke <strong>Center</strong> since 2006. The<br />
service received a “perfect score” from evaluators who<br />
indicated no requirements for improvement.<br />

It’s a great honor to be elected Chairman of a critically<br />
important institution such as <strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong><br />
<strong>Center</strong>. It’s particularly a privilege to follow an<br />
outstanding community leader such as Dr. Ray Hoy in that<br />
capacity, and to serve with a group of thoughtful<br />
community representatives on the Board of Trustees. As<br />
with so many of the achievements that I’ll cite here, my<br />
message will speak to what others have done—not what I<br />
do.<br />
I can say without question that throughout my period of<br />
service on this Board that I have been impressed and<br />
touched by the dedication and commitment that I’ve seen<br />
among the executives at PRMC and among the medical<br />
professionals with whom I’ve served.<br />
It’s a challenging time in this industry with the issues of<br />
rate reductions, healthcare reform, new quality measures<br />
and the impact of a weak economy. At the same time, we<br />
must restate our focus on patient safety and quality care.<br />
While we take great pride on the quality of that care, as<br />
Board members we cannot and do not do the work that<br />
leads to the quality for which the hospital has been<br />
consistently recognized. Those awards and recognitions<br />
are the result of the outstanding work of the care team.<br />
The Board merely puts appropriate emphasis on quality<br />
care and the appropriate allocation of resources and then<br />
supports the staff and medical team as they implement<br />
continuous quality improvements. The results speak for<br />
themselves:<br />
• PRMC has been recognized with a Distinguished<br />
Hospital Award for Clinical Excellence by<br />
HealthGrades in 2009, 2010, 2011 and <strong>2012</strong> placing<br />
the <strong>Medical</strong> <strong>Center</strong> among the top 5% in the nation.<br />

programs number one in the state of Maryland.<br />

There’s so much more—PRMC’s leadership in<br />
technology that has been recognized as one of the<br />
nation’s best, the adoption of the eICU for telemonitoring<br />
critically ill patients, significant growth in the <strong>Peninsula</strong><br />
<strong>Regional</strong> <strong>Medical</strong> Group, the operating room expansion<br />
which follows the major expansion represented by the<br />
Layfield Tower and new collaborative efforts with<br />
Children’s National <strong>Medical</strong> <strong>Center</strong> in Washington D.C.,<br />
and the Johns Hopkins Clinical Research Network.<br />
PRMC is an outstanding community asset with an<br />
outstanding medical and administrative team leading the<br />
way. Our region is fortunate to have such a team always at<br />
their best during those times we need them most. We<br />
look forward to continued success and improvement in<br />

HQID Finds 149 Heart Attack Patient Lives Saved<br />
One hundred forty-nine people survived heart attacks<br />
thanks to <strong>Peninsula</strong> <strong>Regional</strong>’s advancements in the treatment<br />
of acute myocardial infarction (AMI), according to results of<br />
the Hospital Quality Incentive Demonstration (HQID) project,<br />
a value-based purchasing initiative by the <strong>Center</strong>s for<br />
Medicare & Medicaid Services and Premier healthcare alliance<br />
that rewards hospitals for delivering high quality care in six<br />
clinical areas.<br />
Based on the results from the HQID project, <strong>Peninsula</strong><br />
<strong>Regional</strong> received four awards for Top Improvement and<br />
Attainment in the clinical areas of AMI/Heart Attack, Heart<br />
Failure and Surgical Care. Because of its advancements in the<br />
treatment of AMI and improvements in the quality of care, the<br />
HQID project reported that the lives of 149 heart attack<br />
patients at PRMC were saved over the course of the six-year<br />
study.<br />

and nearly 150 lives saved, are a testament to our ongoing<br />

PRMC Named 2011 Most Wired Hospital<br />
Nation’s Most Wired Hospitals Make Progress in Adoption of<br />
Health Information Technology<br />
<strong>Peninsula</strong> <strong>Regional</strong> <strong>Medical</strong> <strong>Center</strong> (PRMC) was recently<br />
recognized as one of the nation’s Most Wired hospitals by the<br />
American Hospital Association’s Hospitals & Health Networks<br />
magazine.<br />
<strong>Peninsula</strong> <strong>Regional</strong> was one of just 205 American hospitals<br />
to win the award and was the only recipient on the Delmarva<br />
<strong>Peninsula</strong>. Just three Maryland hospitals were presented with<br />
this distinguished honor.<br />
The nation’s Most Wired hospitals are making progress<br />
towards greater health information technology (IT) adoption,<br />
according to Hospitals & Health Networks’ 2011 Most Wired<br />
Survey. As a field, hospitals are focused on expanding and<br />
adopting certain kinds of IT, such as computerized physician<br />
order entry (CPOE), to promote improved patient care and<br />
data collection.<br />
