2011-2012 ENROLLMENT INFORMATION
Enrollment Form
The enrollment form has been filled in with information we have on record at Hudsonville Christian School. This includes child‟s
name, birth date, and grade, along with parents‟ employment information and emergency contact information. Please check this
information carefully and make any necessary changes.
Actual Cost of Tuition
You may have noticed on the enrollment form for this year that we have two columns for tuition: the “Actual Tuition Cost”
and “Required Tuition Cost”. We are asking that parents pay the “Required Tuition Cost”; however, if you feel so blessed,
we ask that you prayerfully consider paying the “Actual Tuition Cost” of your child‟s tuition at Hudsonville Christian.
Two Ways to Enroll
1. Enrollment by Mail
The Board encourages enrollment by mail for those families who meet the following criteria:
A. The balance of your 2010-2011 tuition paid prior to or with your 2011-2012 enrollment fee. (Note: if you are paying your
2010-2011 balance along with your enrollment fee, separate checks are appreciated.)
B. Completed enrollment forms and enrollment fee (which is part of tuition, not an additional fee) must be made prior to
or with your mail-in enrollment by Friday, May 27, 2011;
C. Specify your payment plan for 2011-2012.
D. Please note that all TRIP payments and any other credits to your account will have been posted through April 2011.
2. Enrollment in Person
You must enroll in person if the current year‟s tuition will not be paid in full by May 27, 2011. You may enroll in person if
you choose not to enroll by mail.
