OMHA Tournament Package 2012-2013 How to Apply The Ontario Minor Hockey Association (OMHA) requires an Application for a Tournament Sanction Permit and payment submitted to the OMHA for approval to list on the OMHA website. This form can be found at www.omha.net under the Administration section. OMHA CENTRES Prior to submitting the Tournament Sanction Permit, please ensure the following: • Centre President and Secretary have signed the form. • Attach appropriate Tournament Sanction Permit fee with application. • Retain a copy for your Centre's records and submit the original to the OMHA office. W.O.A.A. CENTRES The original Application for a Tournament Sanction Permit and payment must be forwarded to the W.O.A.A. for approval before a tournament can be sanctioned by the OMHA. Once approval has been granted by the W.O.A.A., the application will then be forwarded to the OMHA office. Page 4 of 32
OMHA Tournament Package 2012-2013 Responsibilities of Host Centre The Centre Contact will receive the Tournament Information Package for all sanctioned tournaments. Included with this Package is the Permit and the Reconciliation and Cancellation form. These are to be distributed to the appropriate Contact Person. RESPONSIBILITIES • Verify the Sanction Permit (yellow copy) is correct. • Tear off bottom section of Permit and forward to your Regional Executive Member. • At least fifteen (15) days prior to the start of the tournament a list of the competing teams, a copy of the Tournament Regulations, a copy of the invitation being sent out, and a Tournament schedule must be submitted to the Regional Executive Member. • Display the Permit in a visible location. • Complete the Alliance MD form and return with Reconciliation (if applicable). • Fax the Suspension form to your Regional Executive Member IMMEDIATELY following the final game of the tournament. • Complete and return the Tournament Reconciliation and Cancellation form (pink copy) to the OMHA office within 30 days of the end date of your tournament, including UNUSED game sheets to obtain a refund. • Forward all USED game sheets to your Regional Executive Member at the end of the tournament. • Do not allow any player who receives a match penalty to play any further games in the tournament and IMMEDIATELY inform your Regional Executive Member. • Failure to comply with Tournament Regulation 19.1o) may result in withdrawal of the Tournament Sanction Permit.
