RESPITE CARE WEEKEND DATE REQUESTS —<br />

Requests for Respite Care Weekends occurs 3 weeks prior<br />

to the respite weekend. See page 2 for the specific dates and<br />

process.<br />

Registration/Date Request Process:<br />

1. Review the dates provided in this document and select your<br />

top three camping dates. Every effort will be made to place in<br />

the first date of your request, but this is not always possible.<br />

Please provide your second and third options.<br />

2. There are two options to make a date request at <strong>Camp</strong><br />

<strong>Courageous</strong>:<br />

ONLINE at www.campcourageous.org and follow the<br />

link provided for “date requests.” Link will not open until the<br />

specified time. Read and follow all instructions. Provide all<br />

contact information, camper information, and top three camping<br />

dates.<br />

PHONE the camp at 319-465-5916 and enter the proper<br />

extension for the request that you are making (see above).<br />

Phones messages will not be accepted until specified time.<br />

Listen carefully to instructions provided. Provide all contact<br />

information, camper information, and the top three camping<br />

dates.<br />

3. Once you have submitted your online request or recorded<br />

your voice mail message then you are done. Please refrain<br />

from making duplicate submissions to the online system or<br />

additional phone calls since they will only delay the request<br />

process. You will be notified in the next two to three weeks<br />

about placement and paperwork details.<br />

Answers to Frequently Asked Questions<br />

Should I make requests online or by phone It is<br />

recommended that you make your requests using the online system<br />

at www.campcourageous.org. Regardless of which you use, be<br />

prepared to make your request as promptly as you can on the date<br />

and time listed. Please only make one request using either system<br />

(not both).<br />

After I make my date request, how long before I am<br />

notified if I got the date I wanted It may take two to three<br />

weeks to get campers scheduled. Each camper requires special<br />

consideration by the camp staff to make sure that their stay will be<br />

fun and safe. Please be patient during this time since hundreds of<br />

requests are submitted.<br />

Is there a waiting list Yes. If all three of your dates are already<br />

filled and an alternate date will not work then you can be put on a<br />

waiting list.<br />

When do I fill out the camper’s application, physical,<br />

and medical forms Paperwork will be made available once<br />

a camper has been placed. After being notified that a date has<br />

been assigned, the application and physical forms will be mailed<br />

to you or you can fill them out online. If you have Internet access,<br />

the camp prefers that you use the online forms system to reduce<br />

paper, printing, mailing, and other costs and time associated with<br />

processing applications.<br />

CAMP COURAGEOUS TRAVEL PROGRAM<br />

<strong>Camp</strong> <strong>Courageous</strong>’ Travel Program provides<br />

campers 18 and older the opportunity to experience<br />

the joys and thrills of visiting unique destinations in<br />

a safely monitored atmosphere. Experienced staff,<br />

who have aided in planning each detail of the trip,<br />

accompany travel campers on every trip.<br />

Every <strong>Camp</strong> <strong>Courageous</strong> trip is designed to<br />

cater to each camper group and aid our campers<br />

in functioning successfully in a variety of public<br />

settings. A typical trip is created for campers that<br />

thrive in a 3:1, or greater, more independent yet safely<br />

monitored setting. <strong>Camp</strong> <strong>Courageous</strong> is especially<br />

proud to offer trips for campers who benefit from<br />

more enhanced supervision and care, which are<br />

indicated as Wheeling (1:1) and Assisting (2:1) trips.<br />

All participants must provide camp a non-<br />

refundable deposit on, or before, February 26th<br />

(unless otherwise specified). $300 deposit for<br />

week-long trips and $100 deposit for weekend trips.<br />

Balances for non-flight trips is due at or before 45<br />

days prior to travel. For trips requiring flight the<br />

balance is due at or before 90 days prior to travel.<br />

<strong>Camp</strong>ers are welcome to pay the total amount for<br />

their trip(s) at registration. We also welcome setting<br />

up an individual camper’s travel account with <strong>Camp</strong><br />

<strong>Courageous</strong> for campers to plan ahead for future<br />

travel.<br />

NOTE: <strong>Camp</strong>ers attending week-long trips in May, June, and<br />

Dec. and the CO Adventure will travel by plane. Spaces are<br />

available for campers who use a wheelchair on the “Wheeling”<br />

trips and week-long December trip. <strong>Camp</strong>ers traveling to<br />

Toronto, ON must have a passport to cross borders.<br />

REQUIREMENTS FOR INDIVIDUALS<br />

WHO PARTICIPATE IN TRIPS:<br />

• Be on-the-go A LOT. (The Assisting trips are<br />

created with a more relaxed pace)<br />

• Use stairs, elevators, and/or escalators.<br />

• Attend sporting and cultural events in large<br />

stadiums or venues (schedule permitting).<br />

• Cross city streets.<br />

• Get in and out of a 15-passenger motor coach.<br />

(With the exception of the “Wheeling” and weeklong<br />

December trips, there is no lift on our vehicle.<br />

Please register accordingly.)<br />

• Ride in a motor coach for 3 or more hours<br />

• Stay with the whole group. (Assisting trips plan for<br />

the most 1:1 time comparatively.)<br />

• Share a quad room with two other campers and<br />

one staff person.<br />

• Be independent in personal care (exceptions being<br />

our “Assisting” and “Wheeling” trips).<br />

• Have appropriate social skills.<br />

Nursing and Medication Requirements and Information<br />

H Prepackaged Medication REQUIRED for All Week-Long and Travel <strong>Camp</strong>ers In Advance H<br />

<strong>Camp</strong>er medications must be prepared in a multi-dose or medication cassette (preferably prepared by a pharmacist). Pictures of<br />

each type are shown below. Prepackaged medications have a set of compartments or “bubbles” for each day of the week. The<br />

medications are packaged together according to the time they are to be given.<br />

“Bubble pack” medications prepared by a<br />

pharmacist.<br />

Medication cassettes can be purchased at<br />

many pharmacies and drug stores.<br />

• Request the pharmacist to pack enough doses for the camper’s duration at camp<br />

plus one extra dose.<br />

• Send only the designated amount plus one extra dose. The extra dose, if not<br />

needed, will be returned to you at the end of the week.<br />

• Use the multi-dose system when packaging medications. In a multi-dose system,<br />

each med time for each day has one container/compartment for all meds to be<br />

administered at that time.<br />

• Label the medication bubble pack or cassette with the camper’s name.<br />

• Provide a current list of medications. Please include: name of medication, dosage<br />

(mg. and # of tabs), and times for administration.<br />

• Package and label separately any medication that requires a blood pressure, pulse<br />

check, or other nursing intervention prior to med administration. “Odd-Time”<br />

medications should also be identified separately.<br />

• Use caution before sending non-FDA approved herbal or homeopathic remedies to<br />

camp. Contact the Nursing Department for specific packaging instructions.<br />

The <strong>Camp</strong> <strong>Courageous</strong> Nursing Staff is not liable<br />

for any errors in “pre-packaged” medications sent to camp.<br />

Nursing Check-In Process<br />

Requirements:<br />

• Verify that all necessary paperwork has been<br />

submitted and signed.<br />

• Update and verify contact information.<br />

• Review dietary concerns and bowel<br />

assessments.<br />

• Update camper’s recent medical history.<br />

• Update current medications taken by the<br />

camper to be entered in the camp’s computer<br />

system.<br />

NOTE: Sending or faxing changes in medications<br />

to the camp two weeks prior to camper’s arrival will<br />

greatly enhance the check-in process.<br />

NURSING CHANGES FOR 2010:<br />

• Medications need to be sent to camp two<br />

weeks prior to the scheduled date for weeklong<br />

and travel campers. You may ask your<br />

pharmacist to mail them.<br />

• Send an updated list of medications to camp<br />

with the prepackaged medications.<br />

• Bring liquid medications and topical<br />

treatments in their original labeled bottles<br />

on the day of check in.<br />

• Reminder: ALL campers must have a<br />

parent or staff check in with nursing<br />

department upon arrival.<br />

How long will the check-in process take The nursing<br />

staff will thoroughly review health history and medications for all<br />

campers. Wait times may range from a few minutes to up to an hour<br />

to ensure proper camper care.<br />

My camper won’t be taking meds while at camp. Do I still<br />

need to report to nursing for check-in Yes. ALL campers<br />

must check-in through the nursing department.<br />
