<strong>New</strong> <strong>Submission</strong> <strong>Requirements</strong> <strong>for</strong><br />

<strong>Commercial</strong> <strong>Lines</strong> <strong>Applications</strong><br />

ATB #0xx-12 – May xx, 2012<br />

Effective (insert date), all applications, whether emailed or postmarked, must be<br />

sent with all required documents in a single, complete submission.<br />

In order the ease the submission process, all commercial applications and required<br />

documents can be submitted via email to:<br />

commercial.submissions@citizensfla.com.<br />

Note: This email address is <strong>for</strong> new-business submissions only and does not replace<br />

the commercial email addresses used <strong>for</strong> endorsement requests and<br />

correspondence.<br />

Faxed submissions no longer will be accepted to secure an effective date <strong>for</strong> windonly<br />

policies. The policy effective date cannot be earlier than the day the complete<br />

submission is received via mail or email.<br />

Rejection of <strong>New</strong>-Business <strong>Submission</strong>s<br />

All incomplete new-business submissions, whether received via mail or email, will be<br />

rejected and returned to the agency. A processing fee will be assessed <strong>for</strong> each<br />

incomplete submission.<br />

• Rejected new business submissions will either be returned by email or<br />

mailed back to the agency with the non-cashed check (if applicable) via<br />

overnight mail.<br />

• A rejection/resubmission processing fee of $25 will be billed to the agency<br />

<strong>for</strong> each rejected new business submission.<br />

To obtain coverage <strong>for</strong> a risk that has been returned to the agency as incomplete,<br />

the agent must submit a new and complete application with a new effective date.<br />

<strong>Commercial</strong> <strong>New</strong> Business <strong>Submission</strong> Checklist and <strong>Submission</strong> Instructions<br />

All new-business submission checklists have been updated to make the submission<br />

process easier <strong>for</strong> agents. Each checklist identifies items that must be received <strong>for</strong><br />

each policy type. <strong>Submission</strong> instructions also are available and provide additional<br />

details on each required document. The revised checklists can be found on Citizens<br />

website under:

& CNR-W) and Residential Multiperil (CR-M)<br />

Proof of Eligibility – <strong>New</strong> Business <strong>Submission</strong> Requirement<br />

One of the eligibility documents listed below now must be submitted with all<br />

applications:<br />

• A copy of at least one declination or cancellation/non-renewal letter from an<br />

authorized insurer <strong>for</strong> the named insured at the same property location<br />

• A copy of at least one new or renewal offer showing the authorized insurer’s<br />

premium offer is more than 15 percent higher than the premium <strong>for</strong><br />

comparable coverage at Citizens<br />

• A renewal notice, quote or rejection notice excluding the perils of windstorm<br />

and hail (CNR-W only)<br />

Required Documents<br />

The following documents are required <strong>for</strong> all submissions:<br />

• Signed and completed application<br />

• Signed and completed supplemental application<br />

• Photographs<br />

• Proof of eligibility<br />

• Proof of prior coverage<br />

• Signed Inspection Contact In<strong>for</strong>mation Form<br />

• Proof of valuation (except CNR-M)<br />

• Signed Acknowledgement of Potential Surcharge and Assessment Liability<br />

Form<br />

There are additional required documents that vary by situation and commercial<br />

product. Refer to the new-business submission checklists <strong>for</strong> a complete list of<br />

required documentation.<br />

Credits<br />

If the submission includes all required documents except documentation required to<br />

apply credits, the policy will be issued. However, credits without proper<br />

documentation will not be applied. The applicant will receive a bill <strong>for</strong> any additional<br />

premium owed.<br />

The required documentation to apply credits includes:<br />

• Uni<strong>for</strong>m Mitigation Verification Inspection Form [OIR-B1-1802 (Rev.01/12)]<br />

• Building Type II and III Mitigation Verification Affidavit<br />

• Windstorm Protective Device – Proof of Compliance


Agents and CSRs may submit underwriting questions via the Contact Us option on our website. You<br />

should receive a response within five business days.<br />
