Please send the following to G3 Visas & Passports:
1. Your original valid and signed passport . The passport must have at least two blank pages marked “Visas” and must
be valid for a least 6 months past the end of your trip.
2. Non-US citizens must contact G3 for additional requirements.
3. One visa application form, completed and signed; must be typed or filled in on the PDF form (available at
www.g3visas.com/GLA.html.) Handwritten applications are not accepted. (Travelers residing in Connecticut, Maine,
Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont require a
different application; please contact G3 or visit our website to obtain this.)
4. One passport style photograph (2”x2”), taken within the last 6 months. The photo must be on photo paper and have a
plain white background. Attach photos with a paper clip; do not use tape or glue.
5. A copy of your state-issued driver’s license, or copies of both parents’ driver’s licenses.
6. Proof of departure: i.e. copy of your confirmed/purchased flight itinerary.
7. If you are under age 18, both parents must sign the visa application along with the traveler, and the following
additional items are required:
a. Original or certified copy of your birth certificate showing both parents’ names.
b. Photocopies of both parents’ passports (photo/signature page) or driver’s licenses.
c. Notarized Letter of Consent (attached) signed by both parents.
8. This cover sheet with all fields completed; one cover sheet is required per traveler.

fees are subject to change without notice. Visas will be
valid for a single entry with a 30 day stay. The Consulate
will not issue visas more than 90 days before departure.
