Hours – Monday – Thursday: 7 a.m. – 9 p.m., Friday: 7 a.m. – 5 p.m. and Saturday: 8:30 a.m. – 2 p.m. (effective April 1,
2011) Amenities – Two basketball gymnasiums, meeting rooms, art classrooms, dance classrooms, fitness classrooms,
fitness membership area, indoor walking track and a banquet facility with a catering kitchen.

The fitness room membership includes access
to treadmills, stationary bikes, elliptical machines
and total body workout weight machines.
ACTIVITY REGISTRATION INFORMATION
To register for an activity, you will need a completed registration form (page 5) and full payment. Out-of-county residents
will be charged an additional 20% for all activities and building rentals. Please register at least two days prior to the
start date.
Payment accepted: cash, personal check, money order or credit/debit card (MasterCard and Visa)
Registration is first come, first served. Please note some activities may have assigned registration dates. Minimum
and maximum numbers of participants have been established for each activity. If enrollment is insufficient, activity will
be cancelled and money refunded. If an activity becomes full, we will place your name on a waiting list and you will
be contacted if a space becomes available. We do not prorate class fees. Registration is not accepted by instructors.

Online To use the online registration, visit www.forsythco.com; you must be in our computer system.
If you have registered with us before, your HH ID # is located on your receipt. If you have not
registered with us before, you will need to complete a registration form and fax, mail or drop it by
one of our offices.

FACILITY RESERVATION INFORMATION
Many of the park buildings, pavilions and other areas may be rented for meetings, family gatherings, parties and other
events. Please see a list of parks and facilities on page 4 of this guide. Facility reservations are available first come, first
served. To inquire about facility availability and rental fees, please call 770-781-2215 or visit www.forsythco.com. To
reserve a facility, you will need to complete a rental agreement and pay the rental fee and deposit fee.
REFUND AND TRANSFER POLICY
Any activity cancelled by FCPRD will be refunded in full.
Class Refund Policy - Participants may receive a full refund or transfer if the customer request is received 2 or more
business days before the program start date. Participants may receive a 50% refund of the program fee or full credit
toward another class if the customer request is received before the second meeting of the program. No refunds or
transfers after the second meeting of the program.
Camp Refund Policy - Refunds or transfers requested 1 week prior to the camp start date will receive a full refund or
transfer of fees. Refunds or transfers requested less than 1 week but by noon on the Friday before the camp start date
will receive 50% of registration fees refunded or transferred. No refunds or transfers after noon on the Friday before
camp starts.
Youth Baseball/Softball Fall League - No refunds will be issued except under special circumstances as approved by the
department.
Adult Athletic Leagues - No refunds will be issued for teams except under special circumstances as approved by the
department.
