and 12/31/09.<br />
2) Fill out the information above.<br />

Any returns of Kodak product on the Kodak Zx1 forfeit rebate application. ShopNBC packing slip or<br />
internet invoice and UPC barcode must represent submitter’s actual consumer purchase. Offer is nontransferable.<br />
Copies of UPC barcode not accepted. Limit three (3) rebates per household, address, group,<br />
or organization. Maximum claim per household, address, group or organization is $150. Offer void where<br />
prohibited by law and is limited to residents of the 50 United States and the District of Columbia. We<br />
cannot deliver to PO boxes. Offer excludes resellers of Kodak products and their families. Offer valid only<br />
if obtained from Kodak or a licensed publisher of the rebate. Rebate valid only if redeemed by its original<br />
