Agent For Service Of Legal Process
The agent for service of process is the Plan Administrator. Service of process may also be made on any
individual Trustee.
Collective Bargaining Agreement
The Plan is maintained pursuant to a collective bargaining agreement. The collective bargaining
agreement is available for examination by plan participants and beneficiaries as required by law and
you may obtain a copy of it for a reasonable charge by written request to the Plan Administrator.
Plan Fiscal Year
The fiscal records of the Plan Administrator are maintained on the basis of a Plan Year beginning each
January 1 and ending each December 31.
Funding and Contributions
Your health plan is a self-funded employee welfare benefit plan sponsored by your employer. The cost
of your coverage, which includes the plan benefits and administrative expenses, is borne by your
employer. Employees may contribute to the cost through payroll deduction. Your employer has
entered into an administrative services contract with Anthem to carry out certain functions with
respect to claims operation.
