Sign the bottom of the slip yourself and get your parent/guardian to also sign.
See fees overleaf. CHEQUES or CORRECT CASH only please – No change will
be given. Cheques made payable to St Martin’s School.
Return this form to Mrs C Perks in the Exams Office
with your payment by Friday 16 th March 2012.
Entries will only be made for students who have discussed their resit
entry with their subject teacher and made the appropriate payment.

Cash-In Y/N
To ‘Cash-In’ means that all of your unit results for that subject are pulled together to give you an
overall grade. We need to know if you want your resit mark to be ‘Cashed-In’ to recalculate a new
overall AS grade or whether you just want it to go towards your overall A-Level grade.
If you are definitely not, or even possibly not, continuing the subject to
A-Level, answer ‘Yes’
If you are resitting the unit to get better marks towards your A-Level,
answer ‘No’
