NURSE LED CLINICS<br />
The Mental Health in Learning Disability Team is setting up clinics that are run by nurses. These are called<br />
<strong>Nurse</strong> <strong>Led</strong> <strong>Clinic</strong>s. This is so you can be seen more quickly. The nurses will work together with the doctors in<br />
the team and with you and your carer/s to meet your care needs.<br />

Make a care plan for you<br />
with help from your carers<br />
about your mental health.<br />
Give you information on<br />
how to stay healthy and<br />
get advice about your<br />
emotional wellbeing.<br />

medication and your<br />
physical health.<br />
The nurse can give<br />
medication and<br />
prescriptions.<br />
