Overview<br />

<strong>Center</strong> <strong>for</strong> <strong>Talent</strong> <strong>Development</strong>’s (CTD) <strong>math</strong> curriculum covers topics found in traditional school <strong>math</strong> programs from<br />

Algebra through AP Calculus. As courses at your school may vary in title and content, you will need to provide CTD with<br />

this completed <strong>questionnaire</strong> and a syllabus or course content list from your school (<strong>for</strong> the course equivalent to the one you<br />

plan to take with CTD) be<strong>for</strong>e you arrive on site <strong>for</strong> the program. For example, if you plan to take Algebra II & Trigonometry<br />

at CTD, provide us with the Algebra II syllabus/course content in<strong>for</strong>mation <strong>for</strong> the course offered at your school. The CTD<br />

Program Coordinator will work with you to address questions about course content.<br />

The following <strong>questionnaire</strong> is required <strong>for</strong> all students in <strong>math</strong> courses and designed to help us plan <strong>for</strong> your participation.<br />

Please complete the <strong>questionnaire</strong> in its entirety, as the in<strong>for</strong>mation you provide will allow CTD instructors to adjust your<br />

course material and place you appropriately, giving you the best possible chance to complete the course successfully and <strong>for</strong><br />

credit and/or placement. You need to complete this <strong>questionnaire</strong> even if you are not planning to request credit or placement<br />

at your academic year school.<br />

You must mail your completed <strong>questionnaire</strong> to CTD along with syllabus or curriculum content in<strong>for</strong>mation from your<br />

school.<br />

Credit & Placement <strong>for</strong> CTD Courses<br />

Given the <strong>Center</strong> <strong>for</strong> <strong>Talent</strong> <strong>Development</strong>’s accreditation through the North Central Association Commission on<br />

Accreditation and School Improvement (NCA CASI), students may earn credit <strong>for</strong> courses successfully completed. A grade<br />

of C or better is considered successful completion of a course, and courses carry either one or two semesters of high school<br />

credit. (See course descriptions <strong>for</strong> specifics.) Even if you are not looking to earn credit <strong>for</strong> a course, you may want to<br />

complete the course in order to receive appropriate placement in the next course in the <strong>math</strong> sequence (i.e., if you take<br />

Algebra II over the summer being placed in Pre-Calculus in the fall).<br />

CTD cannot guarantee the acceptance of credit or placement at your academic-year school. The acceptance of credit(s) or<br />

placement at a student’s academic year school depends on that school’s institutional policy about the recognition of credit<br />

from outside institutions. Students who wish to receive credit or placement from their home school <strong>for</strong> a course should<br />

discuss this option with their academic counselor or school administrator be<strong>for</strong>e applying to the CTD Summer Program.<br />

<strong>Center</strong> <strong>for</strong> <strong>Talent</strong> <strong>Development</strong> sends an official transcript to the students’ school unless applicants request not to have it sent<br />

on the Summer Program application <strong>for</strong>m. (Apogee Algebra I students must request that a transcript be sent to the school as<br />

they are not sent automatically.) If you need to update or change your request, please contact the CTD Summer Program<br />

office. Additional transcripts may be requested <strong>for</strong> a fee.<br />

Our experience demonstrates that students who make arrangements <strong>for</strong> credit or placement with their schools be<strong>for</strong>e they<br />

attend the CTD Summer Program have better success in securing appropriate credit or placement. The documentation you<br />

provide in this <strong>questionnaire</strong>, along with the documentation you will have at the completion of the course, will be helpful in<br />

working with the school on credit and placement issues. We cannot overemphasize how important it is to meet with<br />

appropriate contacts at the school you will be attending in the fall prior to your participation in the summer course.

Completing the Questionnaire<br />

<strong>questionnaire</strong>. The person(s) with whom you meet should be the one(s) responsible <strong>for</strong> helping you secure appropriate<br />

2. Share the following in<strong>for</strong>mation with your school representative(s): The CTD <strong>math</strong> sequence includes courses in<br />

Pre-Algebra, Algebra I, Geometry, Algebra II with Trigonometry, Pre-Calculus, AP Calculus AB, and other advanced<br />

topics. The course curriculum is linear, rather than spiral, and is grounded in mastery learning, utilizing diagnostic<br />

testing and prescriptive instruction. In addition to the course text, instructors use other practice problems and test<br />

students on course objectives. Objectives may be added or modified based on the needs of the student and requirements<br />

of his/her academic-year school. Small classes, allowing <strong>for</strong> individualized instruction, and flexible pacing enable<br />

students to master new material at a pace commensurate with their abilities. To ensure that the CTD program of study<br />

meets the requirements of your school, please review the appropriate course description and the syllabus provided by<br />

CTD. In the <strong>questionnaire</strong> please note any topics/objectives covered in your school’s version of the course that are not<br />

currently listed as covered by CTD. CTD staff will do their best to address any discrepancies. CTD requires that students<br />

pass a comprehensive final exam. Nationally normed, standardized tests are also available if your school requires a<br />

standardized test score in addition to the final exam score. If you require a standardized test score in addition to the final<br />

exam, please indicate this on the <strong>questionnaire</strong>. CTD will send final evaluation materials to the student’s family. These<br />

materials include a syllabus, transcript, and narrative evaluation of the student’s per<strong>for</strong>mance. The student’s family is<br />

expected to provide copies of these documents to the school. If requested by the family, a copy of the transcript will be<br />

sent directly to the school.<br />
