Important Notice: Re-Register for Your Verizon Wireless Discount
Verizon Wireless is doing an audit of all Associate lines. Associates utilizing the Verizon
Wireless discount must re-register for their discount or it will be taken off their account.
Overview
The Verizon Wireless Employee Liable End User (ELEU) program offers customers the benefit
of an employer-negotiated monthly discount for their wireless service. Beginning April 2013,
Verizon will begin to periodically ask their customers to validate their employment so that they
ensure they are receiving the correct discount.
If You Have a St. Mary’s Email Address:
Visit www.verizonwireless.com/renewdiscount and follow the instructions.
If You DO NOT Have a St. Mary’s Email Address:
Validate by Paystub (Note: You will still need to go to:
www.verizonwireless.com/renewdiscount)
If customer
is providing copy of
paystub ...
ONLINE
First name, last name, company name and paystub date must be legible on the paystub
copy.
Date on paystub must be within 60 days of the date it is received for validation.
Black out any income or other sensitive information on the paystub, such as Social
Security number, employee number, actual pay information or bank account number.
Customers choosing to Validate by Paystub must provide a copy of a recent paystub
either online or by mail:

Complete the Customer Information and Employer Information sections of the

Discount Eligibility form and click Continue.
Click Browse for a digital copy of a recent paystub and click Upload.
The paystub copy must be in one of the following formats:

.jpeg,
.jpg,
.png, or
.gif.
A submission confirmation page will display for the customer with the confirmation

number for status tracking purposes.
Note: Customer receives a status notification via email within five days of receipt for online
submissions.

Step Action
1 Go to the bottom of the form and select Send a copy of your paystub via mail.
Complete the Contact Information and Employer Information sections of the Discount

Eligibility form and click Continue.
3 Click Print Completed Form.
Mail the printed form, along with a copy of a paystub that meets the guidelines stated
above these instructions to:
