family concern<br />

<strong>Diabetes</strong> is a serious disease, but<br />

learning more about it can help you<br />

and your family understand and better<br />

manage <strong>Type</strong> 2 diabetes.<br />

Community Education for<br />

<strong>Type</strong> 2 <strong>Diabetes</strong><br />

FREE Community education sessions are<br />

provided around the <strong>Nelson</strong> Tasman<br />

region by <strong>Nelson</strong> <strong>Bays</strong> <strong>Primary</strong> <strong>Health</strong> and<br />

supported by <strong>Diabetes</strong> <strong>Nelson</strong>.<br />

Who can attend<br />

The sessions are for people (and their<br />

families) who are at risk of developing<br />

diabetes, or those who have <strong>Type</strong> 2<br />

diabetes. We also welcome those who<br />

are caring for someone with diabetes.<br />

When you learn more, you can help<br />

reduce the risk of developing<br />

complications of diabetes.<br />

The sessions are held over 1-day (10am—<br />

3pm) or can be split into 2 x evening (or day)<br />

sessions. See timetable on the back.<br />

With knowledge and understanding,<br />

you can take action to keep yourself<br />

well with diabetes (or help keep others<br />

around you well)<br />

The sessions promote awareness, wellness<br />

and confidence to help you self-manage<br />

diabetes. Self management is what you do<br />

yourself to manage your diabetes.<br />

The education sessions are delivered in small<br />

groups (15-20 people) using everyday<br />

language and visual aids to enhance your<br />

learning.<br />
