Overview<br />
Periodic assessment of community health needs is a critical component to the mission<br />
and vision of <strong>Terrebonne</strong> General Medical Center (TGMC). Development of strategic initiatives<br />
to address findings of these assessments is essential to improving the health and wellness of the<br />
community. TGMC has performed several community health needs assessments in the last<br />
decade to assess, take action and monitor improvement. The last assessment was performed in<br />
2006. The findings from the 2012 survey have been analyzed and prioritized by TGMC<br />
Executive Team, TGMC Quality Committee of the Board of Commissioners, and the Medical<br />
Staff Performance Improvement Committee. Additional data from several sources was also<br />
vetted in comparison to best determine an action plan to address the overall health needs of our<br />
community.<br />
TGMC will use this document as a planning tool to prioritize and develop strategic<br />
initiatives to address the findings of this survey. TGMC has partnered with many community<br />
health and social organizations within the parish to provide healthcare services to all people<br />
within the service district regardless of social status or ability to pay. This tool will be used to<br />
determine gaps in care and to focus on the priority items noted in this report.<br />
