Child Care Subsidy program is now serving ALL Native American families who qualify within the C.C.D.F. guidelines with the Child Care Certificate program. Education Department The following are current programs administered by the Absentee Shawnee Tribe Education Department: u u u u u Academic (K-12) Program: assists with academic related expenses, and a portion of funds can be used for school-related athletic program expenses. Zahn Program: established from a trust fund, program can help with some graduation expenses, and awards two (2) incentives: one for GED recipients and one for High School graduates. Job Training Adult Education Program: funds for vocational training at accredited vocational institutions. Higher Education: Education Incentive Award Program: funds for undergraduate degrees, one associates or bach elors degree, at any accredited college or university. Graduate Scholarship Program: funds for one master’s degree or one doctoral degree, and can fund for one graduate admissions test. Attention: If you are interested in taking any courses for the Fall 2008 semester contact the Education Department as soon as possible to request the appropriate guidelines and application. Please remember all programs are first come, first served and based upon the availability of funds. AST/EOC Technology Center contract courses now available: The Absentee Shawnee Tribe has an agreement with Eastern Oklahoma County Technology Center to offer courses for tribal members and employees. More information is available on the tribe’s website at www.astribe.com, click on the link Adult Career Development. No online courses are covered in the contract. All courses are campus-based courses only. The contract covers tuition and books, additional supplies required for the courses are the responsibility of the tribal member. This contract covers up to $6,000.00 worth of tuition and books for tribal members, once this amount is depleted the agreement ends. For tribal members interested in taking a course, the application is available for download or a copy can be picked up at the Education Department. Tribal members need to make an appointment to meet with the Education Director, Tresha Spoon, to complete the application process. Please bring your completed application and a copy of your tribal enrollment card. TRIBAL HOUSING IMPROVEMENTS PROGRAM TRIBAL HOUSING IMPROVEMENTS APPLICATION The Tribal Housing Improvement Program’s goal is providing sanitary, safe, and decent housing for its members by utilizing the following criteria to select the most “in need” tribal members with homes that are in disrepair but are still suitable for rehabilitation. All applicants must meet the following criteria, which is approved by the Absentee Shawnee Tribe. Submit completed application to the above address. DOCUMENTS NEEDED BEFORE APPLICATION WILL BE COMPLETED: 1. Copy of Absentee Shawnee Tribe enrollment card (CDIB) 2. Social Security Card 3. Provide copy of one of the following (occupant must be living in the home for at least one (1) year prior to the date of requesting assistance): • Owner (Copy of a filed stamped Warranty Deed) • Buying (Recorded Warranty Deed) • Lease (Official BIA Recorded AST Lease) • Other (Explain) 4. Income Verification (attach verification such as Tax Returns and most recent 2 pay stubs) 5. Award letter for income such as Social Security, SSI, Disability, Unemployment Benefits, Workman’s Comp, etc. THIP list all applicants by a priority list: a. Handicapped/Disabled and Elderly b. Income c. Family Size/Overcrowded Living Conditions d. Condition of Home *All applications must be updated once a year* HOMES UNDER THE MANAGEMENT OF ANY HOUSING AUTHORITY AND HAVE NOT BEEN PAID OFF DO NOT QUALIFY FOR ASSISTANCE • All questions in this application must be answered. If a question is not applicable, write “N/A”. • This application is subject to the Privacy Act of 1974, P.L. 93-579. • Read the certification carefully before you sign and date the application. All signatures must be in ink. Sanitation Facilities Assistance Program This program assists with establishing a new water well and/or septic system or with rehab of an existing water well and/or septic system. OEH&E provides assistance to Absentee Shawnee Tribal members who reside within one of the following six counties: Pottawatomie, Cleveland, Oklahoma, Payne, Lincoln, or Logan. We assist Absentee Shawnee Tribal members who have an existing home, are building a new home, or moving a mobile home to their land. You must update your application every year. Eligibility Requirements: 1. Copy of Absentee Shawnee Tribe (CDIB card) 2. Provide documentation proving homeownership in the Tribal Members name a. Owner (Copy of a filed stamped Warranty Deed) b. Buying (Recorded Warranty Deed) c. Lease (Notarized Lease Agreement; and must be Indian owned) d. Other (Explain) 3. Copy of Social Security card Homes under the management of any housing authority and have not been paid off do not qualify for assistance You can obtain and return applications with documentation to: Absentee Shawnee Tribe Complex Office of Environmental Health & Engineering, Building 13 If you have any questions, you can reach us by phone at (405) 275-4030 Applications are also available online at: www.astribe.com Pest Control Program The Environmental Pest Control Program is designed to assist Absentee Shawnee Tribal elders (55 years and older) in controlling the infestation of termites and insects such as roaches, ticks, fleas, and spiders, etc. Each household can receive one spraying every six months and one termite treatment, if needed. You must update your application every year if you need your home sprayed. Eligibility Requirements: 1. Copy of Absentee Shawnee Tribe (CDIB card) 2. Proof of residency in Tribal member’s name (utility bill, driver’s license, etc.) 3. Must be at least 55 years and older 4.
