<strong>Parent</strong> <strong>In<strong>for</strong>mation</strong> <strong>Sheet</strong><br />

What is the purpose of the project?<br />

The overall aim of the project is to evaluate the Us Girls Alive clubs which work with young<br />

women aged 16-25 years old to improve their health and well-being.<br />

Who is doing this evaluation and why?<br />

The BHF National Centre <strong>for</strong> Physical Activity and Health at Loughborough University has been<br />

asked by <strong>StreetGames</strong> (the organisation managing the Us Girls Alive project) to assess the<br />

impact of this programme on young women. The findings from this project will help to improve<br />

the future delivery of this type of health and well-being programme.<br />

What will my daughter be required to do?<br />

Your daughter is invited to take part in two surveys. The first survey will be completed once<br />

your daughter starts taking part in the Us Girls Alive club, and the second survey will be<br />

completed in 12 months’ time. The surveys may be completed online or using a paper version,<br />

and it will ask your daughter some simple questions about her health, well-being and physical<br />

activity levels.<br />

What personal in<strong>for</strong>mation will be required from my daughter?<br />

Any personal in<strong>for</strong>mation your daughter provides on the survey will be separated from her<br />

answers. This means that we will not know who she is based on the answers she gives.<br />

Will my daughter taking part in this study be kept confidential?<br />

All in<strong>for</strong>mation that your daughter provides will be kept strictly confidential and will be seen<br />

only by the evaluation team at Loughborough University. Your daughter’s name or identity will<br />

not be used when the project is written or talked about in the future. All in<strong>for</strong>mation will be<br />

stored in a secure <strong>for</strong>mat at Loughborough University to which only authorised personnel will<br />

have access.<br />

Once the study has started, can my daughter change their mind?<br />

Yes! If at any time you or your daughter wish to withdraw from the study please just let the<br />

investigator know. You or your daughter can withdraw at any time, <strong>for</strong> any reason and you/she<br />

will not be asked to explain the reasons <strong>for</strong> withdrawing.<br />
