Dear Parent/Carer
Re: Us Girls Alive evaluation
Your daughter is currently taking part in the Us Girls Alive programme which aims to improve
the health and well-being of young women. I am writing to invite your daughter to take part in
the evaluation of this project which is being conducted by an evaluation team from the BHF
National Centre for Physical Activity and Health at Loughborough University.
They would like to ask your daughter to complete two surveys (one now and one in 12 months’
time) so that we can better understand what the impact of the Us Girls Alive programme.
All information provided by your daughter will be kept strictly confidential, and your daughter
will not be identified or linked to any of the information provided. The completed surveys will
be kept securely in a locked office and only the evaluation team at Loughborough University
will be allowed to have access to the information.
With this letter you will find:
1. An information sheet about the project
2. A opt out form
Please read the information sheet carefully and discuss it with your daughter. You are
welcome to email the evaluation team if you would like to ask any questions. You can find
their contact details at the end of the participant information sheet.
If you do not wish your daughter to take part in the survey, please complete the opt-out form
and return it to the evaluation team using the freepost address below (this can be written
directly on to an envelope and no stamp is needed) then post it back to us. We will then
ensure your daughter does not participate in the survey or your daughter’s data is destroyed.
Your decision for your daughter to take part in the evaluation will not affect their eligibility to
participate in other Us Girls Alive activities.

Parent Information Sheet
What is the purpose of the project?
The overall aim of the project is to evaluate the Us Girls Alive clubs which work with young
women aged 16-25 years old to improve their health and well-being.
Who is doing this evaluation and why?
The BHF National Centre for Physical Activity and Health at Loughborough University has been
asked by StreetGames (the organisation managing the Us Girls Alive project) to assess the
impact of this programme on young women. The findings from this project will help to improve
the future delivery of this type of health and well-being programme.
What will my daughter be required to do?
Your daughter is invited to take part in two surveys. The first survey will be completed once
your daughter starts taking part in the Us Girls Alive club, and the second survey will be
completed in 12 months’ time. The surveys may be completed online or using a paper version,
and it will ask your daughter some simple questions about her health, well-being and physical
activity levels.
What personal information will be required from my daughter?
Any personal information your daughter provides on the survey will be separated from her
answers. This means that we will not know who she is based on the answers she gives.
Will my daughter taking part in this study be kept confidential?
All information that your daughter provides will be kept strictly confidential and will be seen
only by the evaluation team at Loughborough University. Your daughter’s name or identity will
not be used when the project is written or talked about in the future. All information will be
stored in a secure format at Loughborough University to which only authorised personnel will
have access.
Once the study has started, can my daughter change their mind?
Yes! If at any time you or your daughter wish to withdraw from the study please just let the
investigator know. You or your daughter can withdraw at any time, for any reason and you/she
will not be asked to explain the reasons for withdrawing.
