JOB DESCRIPTION<br />
POSITION TITLE: CHOICE Home Care Worker<br />
DIVISION: Adult Programming<br />
PROGRAM: Reuther Older Adult & Wellness <strong>Services</strong><br />
DIRECTLY RESPONSIBLE TO: Program Manager<br />
POSITION OBJECTIVE:<br />
Home Care Workers perform in-home assistance with activities of daily living (ADL)<br />
and routine household tasks <strong>to</strong> maintain an adequate living environment for older<br />
individuals with functional limitations.<br />
QUALIFICATIONS:<br />
High school diploma or GED.<br />
Current State CNA certification<br />
MUST be able <strong>to</strong> work in within Wayne, Oakland or Macomb <strong>to</strong> service clients<br />
MUST have ability <strong>to</strong> safely lift, and physically manipulate patients<br />
Experience with Hoyer Lift (manual and electric) preferred<br />
Experience working with the elderly is preferred<br />
MUST be available <strong>to</strong> work a variety of hours<br />
Valid driver’s license, good driving record, and au<strong>to</strong> insurance; reliable personal<br />
transportation (must be a vehicle which can accommodate equipment and/or client)<br />
Previous experience performing homemaker & personal care tasks such as but not<br />
limited <strong>to</strong>: bathing, grooming, cooking and wound care.<br />
Successful candidate must submit <strong>to</strong> post offer, pre-employment physical<br />
examination/medical his<strong>to</strong>ry check, drug screening and background check and TB<br />
screening. A health exam, drug screening; driving record check and police clearance<br />
are required.<br />
Be comfortable working in a variety of living environments.<br />
Strong work ethic and commitment <strong>to</strong> quality services for older adults.<br />
Knowledge of good housekeeping techniques.<br />
Skills in operating equipment such as a vacuum, washing machine/dryer, dishwasher<br />
and mop.<br />

Ability <strong>to</strong> read, understand and follow oral/written instructions. Ability <strong>to</strong> establish and<br />
maintain effective working relationships with clients, employees, and the public. Ability<br />
<strong>to</strong> communicate clearly.<br />
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO THE<br />
FOLLOWING:<br />
Light housekeeping tasks that include, but are not limited <strong>to</strong>: vacuuming, dusting,<br />
washing floors, cleaning bathrooms, washing dishes, and making beds.<br />
Perform grocery shopping and prepare meal, if requested by client.<br />
Handle laundry at a Laundromat or the home.<br />
If assigned a client approved for personal care services, provide assistance with<br />
activities of daily living (ADL) for an individual client including assistance with<br />
bathing, dressing, grooming, <strong>to</strong>ileting, transferring, eating and ambulation.<br />
Daily documentation of client services and submission of monthly reports in a<br />
timely manner.<br />
Participate in staff meetings and other meetings as required.<br />
Enhance professional growth and development through in-service training and<br />
educational programs.<br />
Adhere <strong>to</strong> <strong>Matrix</strong> <strong>Human</strong> <strong>Services</strong>/RSS policies, procedures, objectives.<br />
Promote the values of RSS by demonstrating behaviors supportive of a commitment<br />
<strong>to</strong> quality, client and agency confidentiality, community welfare, respect for clients<br />
and co-workers, teamwork, open and effective communication, innovation and<br />
education, and efficient and effective resource use.<br />
Performs other duties as assigned.<br />
