Department: Community Outreach Office: Catholic Center<br />
Status:<br />
Summary:<br />
Full Time (Exempt)<br />
Assist Department Direc<strong>to</strong>r in administrative program management<br />
including casework supervision for Catholic Center staff. Handle caseload<br />
<strong>to</strong> provide direct care management services through advocacy, information<br />
and referral, crisis intervention services and case management. .<br />
Duties and Responsibilities:<br />
1. Provides day-<strong>to</strong>-day general casework supervision <strong>to</strong> department staff. Acts<br />
as resource <strong>to</strong> staff members for guidance in handling difficult or complex<br />
cases.<br />
2. Assists program direc<strong>to</strong>r supervisor with administrative tasks related <strong>to</strong><br />
program management.<br />
3. Provides crisis intervention <strong>to</strong> individuals and families facing emergencies<br />
related <strong>to</strong> basic needs such as food, shelter, medical needs, and any other<br />
concrete need as presented by the client.<br />
4. Conducts thorough needs assessment and develops a service plan <strong>to</strong> address<br />
identified concrete needs, and follows up with referrals <strong>to</strong> other CCCS<br />
programs or outside resources as deemed appropriate.<br />
5. Maintains current and accurate documentation <strong>of</strong> services provided <strong>to</strong> clients<br />
via the Outcomes Database. Assists/trains staff members in completing<br />
accurate case records and moni<strong>to</strong>rs use <strong>of</strong> the Outcomes system for quality<br />
assurance purposes.<br />
6. Processes emergency assistance requests by obtaining supporting program<br />
and fiscal documentation.<br />
7. Attends Agency’s in-service training and unit meetings as well as any other<br />
agency-related activities.<br />
8. Develops contacts in the community for referral purposes.
9. Develops and maintains good communication with other programs and<br />
departments within the organization <strong>to</strong> facilitate inter-agency referrals.<br />
10. Other duties as assigned.<br />

Education and/or experience required:<br />

1. MSW or MA in Human Services with minimum three years experience in<br />
the social services field. Bilingual, Spanish-English preferred.<br />

2. Excellent interpersonal skills, knowledge <strong>of</strong> social service delivery systems,<br />
knowledge <strong>of</strong> resources available in NYC, good organizational, timemanagement<br />
and communication skills.
