<strong>OMEPRAZOLE</strong><br />

Approved by Neonatal Clinical Committee<br />

DESCRIPTION Proton-pump inhibitor. Inhibits gastric acid secretion by inhibition of<br />

hydrogen-potassium ATPase, the enzyme responsible for the final step in<br />

the secretion of HCL by the gastric parietal cell.<br />

USE Treatment of reflux oesophagitis or duodenal ulcer refractory to<br />

conventional treatment.<br />

PHARMACOLOGY Onset of action is within one hour of administration and the duration of<br />

action is about 72 hours.<br />

DOSE 1-3mg/kg/day daily or in 2 divided doses. Neonates may need 3mg/kg than<br />

1mg/kg in view of variable absorption. Even higher doses may be needed at<br />

the discretion of gastroenterologist and paediatrician.<br />

PRESENTATION 20mg/capsule<br />

2mg/ml suspension<br />

RHW Pharmacy prepares omeprazole suspension by dispersing 100mg<br />

omeprazole (5 capsules) in 50ml of 8.4%sodium bicarbonate solution.<br />

1ml of Omeprazole suspension contains 2mg Omeprazole<br />

1mmol Sodium<br />

1mmol Bicarbonate<br />

STORAGE Stable for 30 days. Refrigerate. Protect from light. Shake the bottle well<br />

before administration.<br />

ADVERSE EFFECTS Limited data. Hypergastrinemia and mild transaminase elevations with<br />

prolonged usage.<br />

MONITORING<br />

1. Consider checking gastric pH (aim >5.0) if there is no symptomatic<br />

improvement.<br />

2. Measure LFTs if duration of therapy is >8 weeks.<br />

DISCHARGE ADVICE RHW Pharmacy supplies a 50 ml bottle of suspension as well as an<br />

instruction sheet to be given to the community pharmacist, if subsequent<br />

bottles should be required.<br />

PREPARATION OF <strong>OMEPRAZOLE</strong> SUSPENSION (2mg/ml)<br />

METHOD<br />

Take 5 capsules of Omeprazole (Probitor 20mg)<br />

Instruction Sheet for Community Pharmacist<br />

Approved by Neonatal Clinical Committee<br />

Carefully open the capsules and empty pellets into a 100ml bottle, containing 50 ml of sodium<br />

bicarbonate 8.4% solution. (Use Injection Sodium Bicarbonate 8.4% 100ml<br />

(Pharmacia Upjohn) or any other brand available).<br />

Shake vigorously from time to time to disperse the pellets. This could take over 60 minutes. (As there<br />

is a certain amount of froth, dispense the suspension in the 100ml bottle)<br />

1. Omeprazole 2mg/1ml<br />

2. Directions<br />

3. Shake the bottle<br />

4. Refrigerate<br />

5. Discard after 30 days<br />
