<strong>Casa</strong> <strong>Mutua</strong> <strong>Supportive</strong> <strong>Housing</strong> <strong>Program</strong><br />

Lenox Hill Neighborhood House, widely recognized as one of New York’s premier human services<br />

providers, is a 118-year-old settlement house that provides an extensive array of effective and integrated<br />

evidence-based programs--social, educational, legal, housing, health, mental health and fitness--which<br />

significantly improve the lives of 20,000 people in need each year, ages 3 to 103, on the East Side of<br />

Manhattan. Our constituents include older adults, homeless and formerly homeless adults, children and<br />

families, recent immigrants, disabled persons and thousands more. Our programs—staffed by social workers,<br />

educators, public interest lawyers, case managers, advocates and many others—include a nationallyrecognized<br />

Early Childhood Center, RealArts After School <strong>Program</strong> and Summer Camp, Women’s Mental<br />

Health Shelter, two Senior Centers, transportation program for frail seniors, caregivers programs, two social<br />

adult day programs, case management program for frail seniors, Legal Advocacy Department, Visual and<br />

<strong>Casa</strong> <strong>Mutua</strong> is our supportive housing program on East 102 nd Street in Manhattan for 54 formerly homeless<br />

adults with histories of mental illness and substance use. The <strong>Clinical</strong> <strong>Case</strong> <strong>Manager</strong> collaborates with the<br />

social service team to provide comprehensive supportive services to a caseload of tenants that ensure safety<br />

and support.<br />

Reports to:<br />

Supervising Social Worker, <strong>Casa</strong> <strong>Mutua</strong>, Lenox Hill Neighborhood House<br />

Specific Responsibilities:<br />

The specific responsibilities of the <strong>Clinical</strong> <strong>Case</strong> <strong>Manager</strong> include but are not limited to the following:<br />

• Provide direct service to clients including assessment, supportive counseling, case management,<br />

advocacy for entitlements and services, group work and referral to resources including psychiatric<br />

and medical care, substance use services, etc.<br />

• Participate in a teamwork approach to service provision and work cooperatively with staff at all levels<br />

at <strong>Casa</strong> <strong>Mutua</strong>, Lenox Hill Neighborhood House and the East Side Homeless Network to ensure<br />

quality service provision.<br />

• Complete comprehensive assessments to inform, develop and implement service plans.<br />

• Complete related paperwork (psychosocials, progress notes, surveys, consent forms) well and in a<br />

timely manner.<br />

• Participate in case conferences and staff meetings.<br />

• Escort tenants to appointments as needed.<br />

• Assist tenants in problem-solving and conflict resolution between neighbors or outside<br />

family/friends.


• Attend and facilitate group activities including monthly parties, meals and groups to model<br />

appropriate social behavior and build relationships with tenants.<br />

• Perform other related duties incidental to the work as assigned.<br />

Qualifications<br />

Applicants must possess a Bachelor’s Degree; minimum of one year of relevant experience in the field of<br />

substance use and/or mental illness. The successful applicant must possess excellent interpersonal and<br />

relationship building skills. A social justice perspective, experience and commitment to working with mentally<br />

ill and homeless populations, and desire to end homelessness are all required. Ability to communicate<br />

effectively in Spanish is required.<br />
