4. Install new skimmers and encase in concrete with #4 bent U bars around back of skimmers. 5. Dowel and epoxy U bar into existing pool shell. 6. Patch pool around skimmer throat. 7. Use a waterproof agent to ensure a watertight seal at patching. All trash and debris from pool renovations must be removed and disposed of by Contractor daily. The City of Forest Park is a franchised area for sanitation and debris removal. Materials and debris can be disposed of by means of a dump truck or pickup truck. Should a roll-off container be needed due to the amount discarded a roll-off container would need to be set up through the City of Forest Park Public Works Department (404-608-2310). Outside roll-off companies are not permitted in the city. INFORMATION FOR BIDDERS Each BID must be accompanied by a BID Bond payable to the OWNER for five percent of the total amount of the BID. As soon as the BID prices have been compared, the OWNER will return the BONDS of all except the three lowest responsible BIDDERS. When the Agreement is executed the bonds of the two remaining unsuccessful BIDDERS will be returned. The BID BOND of the successful BIDDER will be retained until the payment BOND and performance BOND have been executed and approved, after which it will be returned. A certified check may be used in lieu of a BID BOND. A PERFORMANCE BOND and a payment BOND each in the amount of 100 percent of the CONTRACT PRICE, with a corporate surety approved by the OWNER, will be required for the faithful performance of the Contract. Attorney-in-fact who sign BID BONDS or payment BONDS and performance BONDS must file with each BOND a certified and effective dated copy of their power of attorney. Insurance Requirements: A. Statutory Workers Compensations Insurance Employers Liability Bodily Injury Accident - $100,000 Each Accident Bodily Injury by Disease - $500,000 Policy Limit Bodily Injury by Disease - $100,000 Each Employee B. Comprehensive General Liability Insurance $500,000 Limit of Liability per Occurrence for bodily injury and property damage. C. Auto Liability Insurance $500,000 Limit of liability per occurrence for bodily injury and property damage. Comprehensive form covering all owned, non-owned and hired vehicles D. Umbrella Liability Insurance- $1,000,00 limit of liability
E-Verify- All Vendors must submit an affidavit to certify that they participate in the Federal Employment Eligibility Verification. Participation in the E-Verify program must commence before the proposal deadline, and E-Verify affidavit is included in the package. The bidder are required to submit one (1) original and three (3) copies of the Bid. Bids must be received by the opening date and time. Any bids not received by that time will be returned unopened. All bids must be submitted in a sealed envelope or package clearly marked with: Date of Opening, Time of Opening, and Description of Bid. Bidders shall make an oath in writing that he/she has not engaged in any collusive or anticompetitive practices in the responding to the bid. Declaration of non-collusion is a statement that the bid submitted is a bona fide bid prepared independent of other bidders, without collusion or fraud (bid rigging, price fixing, rotation of award, etc.)
