CITIZENSHIP BY DESCENT – APPLICATION DOCUMENT <strong>CHECKLIST</strong><br />
This document lists the information and supporting documents required to lodge your application. It also lists<br />
additional documents that you may need to provide depending on your individual circumstances. You may also<br />
need to provide additional documentation on request from the Department during the processing of your<br />
application.<br />
Complete applications are likely to be processed more quickly. Incomplete applications may be delayed or, in<br />
some cases, refused. The Department is not required to contact you if your application is incomplete. It is<br />
therefore in your best interest to submit all the required documents with your application.<br />
Please make sure you photocopy all the required official documents and English translations and present the<br />
photocopies and the original documents and English translations of documents to the counter officer so that we<br />
can certify the photocopies and return the original documents back to you immediately. Please note that this<br />
office does not photocopy or translate documents.<br />
Note: We strongly advise that you do not purchase airline tickets or make travel bookings before you receive<br />
notification that your application is decided.<br />
After completing this checklist, please attach it to the front of your application.<br />

Application Charge. $130USD (cash only) is payable for each application. Current Application<br />

You must make an appointment to lodge an application. Appointments can be made by phone,<br />
fax or email using the appointment contact details provided at the bottom of this letter.<br />
PERSONAL DOCUMENTS OF APPLICANT<br />

Two passport sized photographs of the applicant (the photograph must be signed and endorsed on<br />
the back of the photo as per the instructions in Form 118 – Part D – Proof of Identity declaration for<br />
details.<br />
Full birth certificates of the applicant, showing details of the applicant’s parents.<br />
You will need to provide penal clearance certificates from overseas countries you lived in for the<br />
last 10 years since the age of 18 years, if:<br />
the total time spent overseas added up to 12 months or more; and<br />
the time spent in any one country was more than 3 months; or<br />
requested to do so by the department.<br />
Australia should be considered an overseas country.<br />
If you live in Phnom Penh, you must provide a police certificate from the Ministry of Justice. If you<br />
live outside of Phnom Penh, you must provide a policy certificate from the Chief of Police of the<br />
local police authority.<br />

DOCUMENTS OF APPLICANTS’ PARENTS<br />

Marriage Certificate, if applicant’s parents are married. Divorce decree if applicant’s parents are<br />
divorced.<br />
Brief statement outlining the history of, and inception of the parents’ relationship. If the parents are<br />
no longer together the statement should also provide information relating to the cessation of the<br />
relationship.<br />
<strong>Australian</strong> parent/s must provide:<br />

Copy of whole passport including bio data page and movement stamps up until the date of<br />
application from the period including 12 months prior to the birth of the child.<br />
A document showing the <strong>Australian</strong> parent/s address (eg. Drivers licence, bill etc).<br />
Full birth certificate of <strong>Australian</strong> parent/s (if born in Australia).<br />
Certificate of <strong>Australian</strong> citizenship (if granted citizenship).<br />
If one of the parents is NOT an <strong>Australian</strong> citizen they must provide copy of whole passport<br />
including bio data page and movement stamps up until the date of application from the period<br />
including 12 months prior to the date of birth of the child. If the parent does not have a passport<br />
because they have not travelled, copy of an identification document with photograph is required.<br />
If the <strong>Australian</strong> citizen parent is an <strong>Australian</strong> citizen by descent, evidence of that parent’s 2 years<br />
residence in Australia (such as passport showing entry to Australia, school or employment records,<br />
any documents issued by an <strong>Australian</strong> Government agency).<br />
Family Book and Residence Book (for Cambodian citizens).<br />
OTHER DOCUMENTS<br />

Evidence of any name changes that have occurred with the child or parent/s, if applicable.<br />
Selection of photographs of applicant with both parents from birth to present (provide originals and<br />
photocopies of the originals).<br />
Photocopy of passport bio-page or Khmer ID card, and evidence of employment/qualifications of<br />
person providing proof of identity.<br />
Please note that DNA testing may be offered where parentage cannot be conclusively<br />
established from the documentation provided.<br />
