CITIZENSHIP BY DESCENT – APPLICATION DOCUMENT CHECKLIST
This document lists the information and supporting documents required to lodge your application. It also lists
additional documents that you may need to provide depending on your individual circumstances. You may also
need to provide additional documentation on request from the Department during the processing of your
application.
Complete applications are likely to be processed more quickly. Incomplete applications may be delayed or, in
some cases, refused. The Department is not required to contact you if your application is incomplete. It is
therefore in your best interest to submit all the required documents with your application.
Please make sure you photocopy all the required official documents and English translations and present the
photocopies and the original documents and English translations of documents to the counter officer so that we
can certify the photocopies and return the original documents back to you immediately. Please note that this
office does not photocopy or translate documents.
Note: We strongly advise that you do not purchase airline tickets or make travel bookings before you receive
notification that your application is decided.
After completing this checklist, please attach it to the front of your application.

Application Charge. $130USD (cash only) is payable for each application. Current Application
Charges available at: http://www.immi.gov.au/allforms/990i/citizenship.htm.
You must make an appointment to lodge an application. Appointments can be made by phone,
fax or email using the appointment contact details provided at the bottom of this letter.
PERSONAL DOCUMENTS OF APPLICANT

Two passport sized photographs of the applicant (the photograph must be signed and endorsed on
the back of the photo as per the instructions in Form 118 – Part D – Proof of Identity declaration for
details.
Full birth certificates of the applicant, showing details of the applicant’s parents.
You will need to provide penal clearance certificates from overseas countries you lived in for the
last 10 years since the age of 18 years, if:
the total time spent overseas added up to 12 months or more; and
the time spent in any one country was more than 3 months; or
requested to do so by the department.
Australia should be considered an overseas country.
If you live in Phnom Penh, you must provide a police certificate from the Ministry of Justice. If you
live outside of Phnom Penh, you must provide a policy certificate from the Chief of Police of the
local police authority.

DOCUMENTS OF APPLICANTS’ PARENTS

Marriage Certificate, if applicant’s parents are married. Divorce decree if applicant’s parents are
divorced.
Brief statement outlining the history of, and inception of the parents’ relationship. If the parents are
no longer together the statement should also provide information relating to the cessation of the
relationship.
Australian parent/s must provide:
Copy of whole passport including bio data page and movement stamps up until the date of
application from the period including 12 months prior to the birth of the child.
A document showing the Australian parent/s address (eg. Drivers licence, bill etc).
Full birth certificate of Australian parent/s (if born in Australia).
Certificate of Australian citizenship (if granted citizenship).
If one of the parents is NOT an Australian citizen they must provide copy of whole passport
including bio data page and movement stamps up until the date of application from the period
including 12 months prior to the date of birth of the child. If the parent does not have a passport
because they have not travelled, copy of an identification document with photograph is required.
If the Australian citizen parent is an Australian citizen by descent, evidence of that parent’s 2 years
residence in Australia (such as passport showing entry to Australia, school or employment records,
any documents issued by an Australian Government agency).
Family Book and Residence Book (for Cambodian citizens).
OTHER DOCUMENTS
Check Box
Evidence of any name changes that have occurred with the child or parent/s, if applicable.
Selection of photographs of applicant with both parents from birth to present (provide originals and
photocopies of the originals).
Photocopy of passport bio-page or Khmer ID card, and evidence of employment/qualifications of
person providing proof of identity.
Please note that DNA testing may be offered where parentage cannot be conclusively
established from the documentation provided.
