TOURIST VISA (SUBCLASS 676) – APPLICATION DOCUMENT CHECKLIST
This document lists the information and supporting documents required to lodge your application. It also lists
additional documents that you may need to provide depending on your individual circumstances. You may also
need to provide additional documentation on request from the Department during the processing of your
visa.
Note: We strongly advise that you do not purchase airline tickets or make travel bookings before you receive
notification that your visa is decided.
Complete applications are likely to be processed more quickly. Incomplete applications may be delayed or, in
some cases, refused. The Department is not required to contact you if your application is incomplete. It is
therefore in your best interest to submit all the required documents with your application.
Please make sure you photocopy all the required official documents and English translations and present the
photocopies and the original documents and English translations of documents to the counter officer so that we
can certify the photocopies and return the original documents back to you immediately. Please note that this
office does not photocopy or translate documents.
After completing this checklist, please attach it to the front of your application.
FORMS, FEE AND APPOINTMENT

Completed and signed Form 48R: Application to visit Australia for tourism or other recreational
activities. Available at: http://www.immi.gov.au/allforms/pdf/48r.pdf. If you are travelling with
children who are included in your passport, their details should be included in the same application
form. Children not included in your passport, and any other family members or friends
accompanying you, need to complete a separate form.
Details of Relatives Form – Use this form to provide details of all your family members as well as
those of your spouse (if applicable). You can obtain a copy of this form on the website at
http://www.cambodia.embassy.gov.au/penh/Visas_and_Migration.html.

You must make an appointment to lodge an application. Appointments can be made by phone,
fax or email using the appointment contact details provided at the bottom of this letter.
PERSONAL DOCUMENTS

Current passport and any recent passports of all applicants included in the application (originals).
4 recent passport sized photographs of all persons included in the application. The photographs
should be attached to the front page of your application and the names and dates of birth of each
applicant should be written on the back of each photograph.

Full birth certificates of all applicants included in the application.
FINANCIAL DOCUMENTS

Evidence to demonstrate that you and all persons included in the application have sufficient funds
or access to sufficient funds for their visit to Australia. The emphasis is on demonstrating a
savings history (over a period of approximately 2-3 months). This may include documents such as:
• Evidence of your savings such as bank deposit books.
• Evidence of bank statements or payslips over a period of time.
• Credit card statements.
• Evidence of assets.
• Evidence of your employment or business activities.
It should be evident from the evidence provided where the funds have come from. You do not
need to open a new bank account for the purposes of demonstrating funds. Little weight
will be given on this form of evidence as the emphasis is on a savings history.
If a friend or relative in Australia is paying for your visit, a letter of invitation from them and evidence
that they have the necessary funds.
EMPLOYMENT AND STUDY DOCUMENTS

If you are employed, a letter from your employer showing: your position and salary; length of your
employment; approval for any leave; name and contact details of the person providing the letter.
If you are a business owner, your business patent and evidence of business activities.
If you are a student, evidence of your enrolment at school
CHILDREN UNDER 18 YEARS
Check Box
If a child under 18 years is coming to Australia without one or both parents or legal guardians, a
consent form signed by the non-accompanying parent or legal guardian.
Form 1229: Consent form to grant an Australian visa to a child under the age of 18 years.

family relationship. A birth certificate of the child, showing names of both parents should be
provided.
Bio data pages of the passport (or if unavailable, Cambodian ID card) of any parent or guardian not
accompanying the child should be provided.
Evidence of child’s enrolment in school (if applicable)
Copy of parents’ marriage certificate (if applicable)
Contact details including full name, date of birth, address and phone numbers should be provided
of the non-accompanying parent or guardian.
If a child under 18 years is staying in Australia with someone other than a parent, legal guardian or
relative, a declaration signed by the person responsible for that child in Australia.

HEALTH REQUIREMENTS – you will be advised at your appointment when you lodge your
application if you need to undergo medical examinations

You may be asked to undergo medical and/or x-ray examinations. This may be the case if you are
staying in Australia for more than 3 months (for Cambodian citizens), you are likely to enter a
classroom for more than 3 months, you are likely to enter a health care or hospital environment,
you are pregnant, you are likely to work at a childcare centre or you are over 70 years of age. For

If you are 75 years or older, evidence of private travel insurance covering the proposed period of
stay in Australia.
OTHER DOCUMENTS
Check Box
If you are visiting family or friends in Australia – your inviter’s name and contact details and a copy
of their passport or Australian visa. If visiting family provide evidence of relationship.
Any other information you wish to provide that shows that you have an incentive to return to your
home country.
PROCESSING TIME. Applications are generally decided within 10 business days of receipt. However this
depends on whether the applications are complete and applications are decided on a case by case basis. Some
applications may take longer to decide. Please also note that if a medical examination and/or chest x-ray is
required then a decision may not be made until the results are finalised. It is in your best interest to lodge your
application well in advance of your proposed travel date.
