TOURIST VISA (SUBCLASS 676) – APPLICATION DOCUMENT CHECKLIST<br />

This document lists the information <strong>and</strong> supporting documents required to lodge your application. It also lists<br />

additional documents that you may need to provide depending on your individual circumstances. You may also<br />

need to provide additional documentation on request from the Department during the processing of your<br />

visa.<br />

Note: We strongly advise that you do not purchase airline tickets or make travel bookings before you receive<br />

notification that your visa is decided.<br />

Complete applications are likely to be processed more quickly. Incomplete applications may be delayed or, in<br />

some cases, refused. The Department is not required to contact you if your application is incomplete. It is<br />

therefore in your best interest to submit all the required documents with your application.<br />

Please make sure you photocopy all the required official documents <strong>and</strong> English translations <strong>and</strong> present the<br />

photocopies <strong>and</strong> the original documents <strong>and</strong> English translations of documents to the counter officer so that we<br />

can certify the photocopies <strong>and</strong> return the original documents back to you immediately. Please note that this<br />

office does not photocopy or translate documents.<br />

After completing this checklist, please attach it to the front of your application.<br />

FORMS, FEE AND APPOINTMENT<br />

Completed <strong>and</strong> signed Form 48R: Application to visit Australia for tourism or other recreational<br />

activities. Available at: http://www.immi.gov.au/allforms/pdf/48r.pdf. If you are travelling with<br />

children who are included in your passport, their details should be included in the same application<br />

form. Children not included in your passport, <strong>and</strong> any other family members or friends<br />

accompanying you, need to complete a separate form.<br />

Details of Relatives Form – Use this form to provide details of all your family members as well as<br />

those of your spouse (if applicable). You can obtain a copy of this form on the website at<br />

http://www.cambodia.embassy.gov.au/penh/Visas_<strong>and</strong>_Migration.html.<br />

You must make an appointment to lodge an application. Appointments can be made by phone,<br />

fax or email using the appointment contact details provided at the bottom of this letter.<br />

PERSONAL DOCUMENTS<br />

Current passport <strong>and</strong> any recent passports of all applicants included in the application (originals).<br />

4 recent passport sized photographs of all persons included in the application. The photographs<br />

should be attached to the front page of your application <strong>and</strong> the names <strong>and</strong> dates of birth of each<br />

applicant should be written on the back of each photograph.<br />

Family Book <strong>and</strong> Residence Book (for Cambodian citizens).<br />

Full birth certificates of all applicants included in the application.<br />

FINANCIAL DOCUMENTS<br />

Evidence to demonstrate that you <strong>and</strong> all persons included in the application have sufficient funds<br />

or access to sufficient funds for their visit to Australia. The emphasis is on demonstrating a<br />

savings history (over a period of approximately 2-3 months). This may include documents such as:<br />

• Evidence of your savings such as bank deposit books.<br />

• Evidence of bank statements or payslips over a period of time.<br />

• Credit card statements.<br />

• Evidence of assets.<br />

• Evidence of your employment or business activities.<br />

It should be evident from the evidence provided where the funds have come from. You do not<br />

need to open a new bank account for the purposes of demonstrating funds. Little weight<br />

will be given on this form of evidence as the emphasis is on a savings history.<br />

If a friend or relative in Australia is paying for your visit, a letter of invitation from them <strong>and</strong> evidence<br />

that they have the necessary funds.<br />

EMPLOYMENT AND STUDY DOCUMENTS<br />

If you are employed, a letter from your employer showing: your position <strong>and</strong> salary; length of your<br />

employment; approval for any leave; name <strong>and</strong> contact details of the person providing the letter.<br />

If you are a business owner, your business patent <strong>and</strong> evidence of business activities.<br />

If you are a student, evidence of your enrolment at school<br />

CHILDREN UNDER 18 YEARS<br />

Check Box<br />

If a child under 18 years is coming to Australia without one or both parents or legal guardians, a<br />

consent form signed by the non-accompanying parent or legal guardian.<br />

Form 1229: Consent form to grant an Australian visa to a child under the age of 18 years.<br />

Available at: http://www.immi.gov.au/allforms/pdf/1229.pdf. Evidence must be provided to confirm<br />

family relationship. A birth certificate of the child, showing names of both parents should be<br />

provided.<br />

Bio data pages of the passport (or if unavailable, Cambodian ID card) of any parent or guardian not<br />

accompanying the child should be provided.<br />

Evidence of child’s enrolment in school (if applicable)<br />

Copy of parents’ marriage certificate (if applicable)<br />

Contact details including full name, date of birth, address <strong>and</strong> phone numbers should be provided<br />

of the non-accompanying parent or guardian.<br />

If a child under 18 years is staying in Australia with someone other than a parent, legal guardian or<br />

relative, a declaration signed by the person responsible for that child in Australia.<br />

HEALTH REQUIREMENTS – you will be advised at your appointment when you lodge your<br />

application if you need to undergo medical examinations<br />

You may be asked to undergo medical <strong>and</strong>/or x-ray examinations. This may be the case if you are<br />

staying in Australia for more than 3 months (for Cambodian citizens), you are likely to enter a<br />

classroom for more than 3 months, you are likely to enter a health care or hospital environment,<br />

you are pregnant, you are likely to work at a childcare centre or you are over 70 years of age. For<br />

If you are 75 years or older, evidence of private travel insurance covering the proposed period of<br />

stay in Australia.<br />

OTHER DOCUMENTS<br />

If you are visiting family or friends in Australia – your inviter’s name <strong>and</strong> contact details <strong>and</strong> a copy<br />

of their passport or Australian visa. If visiting family provide evidence of relationship.<br />

Any other information you wish to provide that shows that you have an incentive to return to your<br />

home country.<br />

PROCESSING TIME. Applications are generally decided within 10 business days of receipt. However this<br />

depends on whether the applications are complete <strong>and</strong> applications are decided on a case by case basis. Some<br />

applications may take longer to decide. Please also note that if a medical examination <strong>and</strong>/or chest x-ray is<br />

required then a decision may not be made until the results are finalised. It is in your best interest to lodge your<br />

application well in advance of your proposed travel date.<br />
