<strong>Officer</strong> <strong>Nomination</strong> <strong>Form</strong><br />

<strong>DaVinci</strong> <strong>PTO</strong>’s mission is to enrich the learning opportunities <strong>and</strong> environment at <strong>DaVinci</strong> <strong>Academy</strong>. We will encourage<br />

involvement <strong>and</strong> strive to maintain communication <strong>and</strong> cooperation <strong>of</strong> all parents, teachers, <strong>and</strong> staff to ensure that<br />

students achieve their fullest potential <strong>and</strong> receive the best education possible. We will fundraise for the school to help<br />

provide outst<strong>and</strong>ing educational services <strong>and</strong> technology for our teachers <strong>and</strong> students.<br />

The <strong>DaVinci</strong> <strong>Academy</strong> <strong>PTO</strong> is seeking nominations for the <strong>PTO</strong> Executive <strong>Officer</strong> Positions for<br />

the 2013-2015 school years. The responsibilities <strong>of</strong> the <strong>of</strong>ficers are detailed on the back <strong>of</strong> this<br />

form. If you have the time <strong>and</strong> interest to serve as an <strong>Officer</strong>, please complete this form <strong>and</strong><br />

place it in an envelope marked <strong>DaVinci</strong> <strong>PTO</strong> Nominating Committee, <strong>and</strong> return to school no<br />

later than Tuesday, 8/20/13 by 3:30 pm. The election will take place on Tuesday, 8/27/13.<br />

Installation <strong>of</strong> elected <strong>of</strong>ficers will be after a three month training process.<br />

<strong>Officer</strong> <strong>Nomination</strong> <strong>Form</strong><br />

Fundraising Coordinator:<br />

• Chair the <strong>PTO</strong> Fundraising Committee, which shall investigate details <strong>of</strong> any new fundraiser for possible proposal for the <strong>PTO</strong>. All new<br />

fundraisers must be approved by the <strong>PTO</strong> Board <strong>and</strong> then must be approved by the School Development Committee.<br />

• Meet with chairpersons <strong>of</strong> specific <strong>PTO</strong> fundraisers <strong>and</strong> oversee <strong>and</strong> coordinate details connected to each fundraiser with all fundraisers<br />

being documented in a detailed manner on a spreadsheet <strong>and</strong> presented to the <strong>PTO</strong> Executive Board monthly <strong>and</strong>/or when requested by the<br />

President.<br />

• Work with the Executive Board to summarize <strong>and</strong> report the results <strong>of</strong> each fundraiser including working with the Treasure <strong>and</strong> President to<br />

report the financials <strong>of</strong> each individual Fundraising Event.<br />

Secretary/Record Keeping:<br />

• Shall keep an accurate record <strong>of</strong> all general DVAAS <strong>PTO</strong> meetings.<br />

• Shall present to the membership the minutes <strong>of</strong> the previous DVAAS <strong>PTO</strong> meeting.<br />

• Shall create <strong>and</strong> submit the Purely <strong>PTO</strong> Newsletter Bi-Weekly.<br />

• Shall update <strong>and</strong> maintain the DVAAS <strong>PTO</strong> website.<br />

• If possible, shall be a part <strong>of</strong> the Communications Committee (attend when the President cannot).<br />

Treasurer/Finance:<br />

• Shall have custody <strong>of</strong> all financial records <strong>and</strong> DVAAS <strong>PTO</strong> funds. Shall maintain current records <strong>and</strong> shall submit monthly financial reports to<br />

the board <strong>and</strong> membership.<br />

• Shall give a report at each DVAAS <strong>PTO</strong> meeting <strong>and</strong> make available a copy <strong>of</strong> the report to the Executive <strong>Officer</strong>s.<br />

• Shall secure by a majority vote <strong>of</strong> the membership approval <strong>of</strong> all non-budgeted expenditures over $750.00.<br />

• Shall prepare a budget with the President, Vice President <strong>and</strong> Director prior to the first fall meeting.<br />

• Ensure all tax paperwork is complete <strong>and</strong> taken care <strong>of</strong>.<br />

• Shall be one <strong>of</strong> the <strong>Officer</strong>s authorized to sign checks <strong>of</strong> the DVAAS <strong>PTO</strong> <strong>and</strong> do any banking deposits.<br />

• Shall be in charge <strong>of</strong> all fundraising activity funds along with the President <strong>and</strong> Fundraising Coordinator.<br />
