PURPOSE<br />
<strong>New</strong> <strong>York</strong> <strong>State</strong> <strong>PTA</strong> proudly offers another way to honor your most dedicated volunteers – the Golden Oak Award plaque<br />
<strong>and</strong> the Diamond Award table-top sculpture. Each award is beautifully crafted to make a distinguished presentation to an<br />
honoree. Both feature personalized engraving with the name of the award recipient, <strong>and</strong> bear the signature of our state<br />
president.<br />
Proceeds from the Golden Oak <strong>and</strong> Diamond awards will be dedicated to leadership development throughout the state,<br />
while honoring the dedication <strong>and</strong> excellence of past <strong>and</strong> present members of your school community.<br />
CRITERIA<br />
These awards are requested by a <strong>PTA</strong> unit, council or region to honor person(s) of their choice; an order is placed to<br />
purchase a specific award for the honorees.<br />
ORDER INFORMATION<br />
An order form <strong>and</strong> accompanying payment for both of these awards is available on the <strong>New</strong> <strong>York</strong> <strong>State</strong> <strong>PTA</strong> website (www.<br />
nyspta.org). Please print the order form, fill it out <strong>and</strong> mail the form with the appropriate payment to the <strong>New</strong> <strong>York</strong> <strong>State</strong><br />
<strong>PTA</strong> office. Please remember to have two signatures on the <strong>PTA</strong> check <strong>and</strong> allow six weeks for delivery.<br />
DEADLINE<br />
There is no deadline for these awards other than planning for the six weeks delivery time of the completed award.<br />
