The Citrus Valley Health Foundation is proud to sponsor the 38th Annual Golf Invitational and wishes to 

thank all of our past and present supporters for helping build this fine tradition. 

All proceeds from this tournament will benefit programs such as the NICU at The Family Birth & Newborn 

Center at Citrus Valley Medical Center – Queen of the Valley Campus; the new Cardiac Cath Lab for the 

Heart Center at Citrus Valley Medical Center – Inter-Community Campus; a new Outpatient Surgery Unit 

at Foothill Presbyterian Hospital; and facility improvements and repairs for Citrus Valley Hospice. 

Golf registration includes: 

• Golf green fees 

• Use of golf cart 

• Continental breakfast, lunch on the course, and 

buffet dinner 

• Tee bags with gifts 

• Opportunity for prizes 

• Unlimited balls for range practice 

• Entry in putting contest 

Schedule of events: 

9 a.m. Registration 

9-10:30 a.m. Continental breakfast 

9:30 a.m. Putting contest 

11 a.m. Shotgun start 

Noon-2 p.m. Lunch provided on the course 

5:30 p.m. Buffet dinner 

6 p.m. Awards program 

DRESS CODE STRICTLY ENFORCED – No denim material permitted. Soft spikes or tennis shoes only. 

MEN: Slacks, Bermuda shorts or shorts tailored for golfing activities. Tennis-type shorts or cargo shorts are not 

permitted. Collared shirts required. Shirts must be tucked in at all times. 

WOMEN: Golf skirts, slacks, culottes, Bermuda shorts or shorts tailored for golfing activities. Tennis-type shorts or
