independently <strong>of</strong> the <strong>Board</strong>, residential agencies or<br />
other service providers.<br />

The mission <strong>of</strong> the <strong>Portage</strong> <strong>County</strong> <strong>Board</strong> <strong>of</strong> Developmental Disabilities is to ensure<br />
the availability <strong>of</strong> programs, services, <strong>and</strong> supports that assist eligible individuals with<br />
developmental disabilities in choosing <strong>and</strong> achieving a life <strong>of</strong> increasing capability<br />
such that they can live, work, <strong>and</strong> socialize in the community, <strong>and</strong> to assist <strong>and</strong><br />
support the families <strong>of</strong> these individuals in achieving this objective.<br />

<strong>Service</strong> <strong>and</strong> <strong>Support</strong> <strong>Administration</strong> Provides<br />
What is <strong>Service</strong> <strong>and</strong> <strong>Support</strong> <strong>Administration</strong>?<br />
<strong>Service</strong> <strong>and</strong> <strong>Support</strong> <strong>Administration</strong> (SSA) supports individuals<br />
in determining <strong>and</strong> pursuing goals <strong>and</strong> maintains the<br />
individual as the focus while coordinating services across<br />
multiple systems. A <strong>Service</strong> <strong>and</strong> <strong>Support</strong> Administrator (SSA)<br />
will assist individuals <strong>and</strong> families to support, empower, <strong>and</strong><br />
advocate for the individual. <strong>Service</strong>s provided through the<br />
SSA are person-centered. SSAs support individuals in<br />
determining <strong>and</strong> pursuing life goals. SSAs work with<br />
families, guardians <strong>and</strong> natural supports to access, provide<br />
<strong>and</strong>/or enlist whatever support is needed, <strong>and</strong> coordinate<br />
services among different agencies serving as the single<br />
point <strong>of</strong> accountability.<br />
How is eligibility determined<br />
A SSA shall determine eligibility for individuals age 3 <strong>and</strong><br />
above according to rules adopted by the Ohio Department<br />
<strong>of</strong> Developmental Disabilities (DODD). Referrals can be made<br />
by calling 330-297-4100 or by email: intake@portagedd.org.<br />
Upon eligibility, the SSA assigned shall provide individuals<br />
with the following services:<br />
Needs Assessment<br />
The SSA assigned shall determine if there is a need for PCBDD<br />
service coordination, the level <strong>and</strong> intensity <strong>of</strong> the need,<br />
projected duration, <strong>and</strong> the types <strong>of</strong> services needed/requested.<br />
The information obtained shall include the individual’s likes,<br />
dislikes, priorities, <strong>and</strong> desired outcomes, including skill<br />
development <strong>and</strong> health <strong>and</strong> safety needs.<br />
ISP Development<br />
The SSA assigned shall incorporate the results <strong>of</strong> the needs<br />
assessment into an Individual <strong>Service</strong> Plan (ISP) which integrates<br />
all sources <strong>of</strong> supports including alternative services available to<br />
meet the needs <strong>and</strong> desired outcomes <strong>of</strong> the individual.<br />
Provider Selection<br />
The SSA assigned will ensure that individuals are given the<br />
opportunity to select the service provider(s) <strong>of</strong> their choice <strong>and</strong><br />
help them to work with their providers to resolve any concerns.<br />
Budget<br />
The SSA assigned shall establish recommendations for obtaining<br />
approval <strong>of</strong> budgets for services based on the ISP for the<br />
individual.<br />
<strong>Service</strong> Coordination & Linkage<br />
The SSA shall ensure that services are effectively coordinated<br />
<strong>and</strong> provided by providers as identified in the ISP.<br />
Quality Assurance Reviews<br />
A SSA who does not serve as the individual’s single point <strong>of</strong><br />
accountability will conduct a quality assurance review to ensure<br />
that areas <strong>of</strong> concern are identified <strong>and</strong> remedied so as to<br />
achieve desired outcomes for the individual.<br />
<strong>Service</strong> Monitoring<br />
The SSA assigned shall establish <strong>and</strong> implement service<br />
monitoring to ensure health <strong>and</strong> safety <strong>of</strong> individuals,<br />
consistent implementation <strong>of</strong> services, <strong>and</strong> achievement <strong>of</strong><br />
desired outcomes.<br />
Personal Advocate<br />
The SSA assigned shall ensure that each individual receiving<br />
service <strong>and</strong> support administration shall have a designated<br />
person to provide daily representation who is responsible<br />
on a continuing basis for providing the individual with<br />
representation, advocacy, advice <strong>and</strong> assistance related to the<br />
day-to-day coordination <strong>of</strong> services in accordance with the ISP.<br />
Crisis Intervention<br />
A SSA is on-call 24 hours per day when urgent health <strong>and</strong> safety<br />
issues arise, including the reporting <strong>of</strong> Major Unusual Incidents<br />
(MUIs), which are then forwarded to the Investigative Agent (IA)<br />
for review.<br />
Each person or his legal guardian must consent<br />
to services provided by a <strong>Service</strong> <strong>and</strong><br />
<strong>Support</strong> Administrator.<br />
