Please read the following instructions in order for you appointment to be as
helpful as possible for you.
Prior to your appointment with Dr. Gladstone:
1.
Please complete the two New Patient Headache Questionnaires.
Questionnaire #1 is a General Health and Headache-Specific
Questionnaire which is 6 pages and Questionnaire #2 is a Headache-
Related Disability Questionnaire which is 4 pages. Please take the time
to fill out the questionnaires as accurately as possible. The information on
these forms is very important for your first consultation with Dr. Gladstone.
If you have difficulty with English, please get someone to help you fill out
the questionnaires.

After you have completed the questionnaires, if you have access to a fax
machine, please fax it back to my Sunnybrook Headache Clinic at 416-
480-4992. Please fax it back at least 24 hours prior to your appointment
so I will have the opportunity to review it prior to your appointment. If you
do not have access to a fax machine, please remember to bring the
questionnaires with you to your first appointment.

All patients should bring their completed questionnaires with them to their
first appointment with Dr. Gladstone.
4.
If you have seen other specialists about your headaches (neurologists,
pain clinics, ear-nose-throat doctor, eye doctor), please ensure that your
records are faxed to Dr. Gladstone before your appointment (contact
the specialist’s office or your family physician’s office to request that your
notes be faxed).
5.
If you have had a CT or MRI scan to evaluate your headaches, please
ensure that the results are faxed to Dr. Gladstone before your
appointment (contact the Film Library at the Hospital where you had the
scan done or contact your family physician’s office to request that your
results be faxed).

Please ensure that you bring with you your current OHIP card.
Please ensure that you bring with you your completed New Patient
Questionnaires (all 10 pages).
Please be aware that new patient consultations take approximately 45
minutes.
Please note that there are 2 different Dr. Gladstones in Neurology at
Sunnybrook Health Sciences Centre.
• My brother, Dr. David Gladstone, is a Stroke Neurologist at
Sunnybrook and his office is located on the 4 th floor (A454).
• My office (Dr. Jonathan Gladstone) is located on the 2 nd floor in A233.
To avoid confusion, please ensure that you find your way to the office of
Dr. Jonathan Gladstone (A233) rather than Dr. David Gladstone.
