Standard CDHP Plan OptionHighlightsThe Standard Consumer Driven Health Plan(CDHP) puts you, the consumer, in morecontrol of managing your health expenses.You receive an employer-funded HealthReimbursement Account (HRA) to use towardyour deductible and maximum out-of-pocket.This plan does not require the LivingWellPromise.If you currently have the Maximum Choiceplan and choose this plan, any remaining HRAbalance will roll to this plan.This plan offers the lowest premiums inexchange for higher deductibles.Any member who fails to elect or waivecoverage by Oct. 31, 2013, is automaticallyenrolled in this plan with single coverage.HRA helps reduce your costsThe HRA can be used to reduce your deductible.Your HumanaAccess Visa® debit card comespre-loaded. Just swipe the debit card at anyin-network provider’s office or pharmacy and theeligible expense amount will be reduced from yourcard balance. Nearly 80% of all debit card swipes areautomatically processed; however, you may have tosubmit expense documentation or substantiation.HRA fundsIf your annual expenses are below $250 for singlecoverage or below $500 for all other coveragelevels, you won’t have to spend any money out ofyour own pocket. Once you use all your HRAfunds, you will pay for any additional expenses upto your deductible.
