Request <strong>For</strong> Quotations (RFQ) NO. 1011-910-99-3388 <strong>Clean</strong>, <strong>Paint</strong>, & <strong>Minor</strong> <strong>Repair</strong> Services=====================================================================13.18 Green Initiatives: The Contractor is encouraged to investigate <strong>and</strong> offeralternatives for cleaning supplies <strong>and</strong> materials that would increase the“green” or environmental efficiency of the project.13.19 If not on file already with SAHA, Contractor shall provide completed W-9form <strong>and</strong> insurance certificate within twenty four (24) hours of beingnotified by SAHA of awarded bid. The insurance certificate shall meet all ofour insurance requirements as stated in section 14.0 below.13.20 EPA Requirements. SAHA requires that all make ready Contractors mustbe EPA certified. Contractors performing renovation, repair <strong>and</strong> paintingprojects that disturb lead- based paint in homes, child care facilities, <strong>and</strong>schools built before 1978 must be certified by EPA <strong>and</strong> must follow specificwork practices to prevent lead contamination.This includes, but is not limited to:Contain the work area.Minimize dust.<strong>Clean</strong> up thoroughly.EPA requires that firms performing renovation, repair, <strong>and</strong> paintingprojects that disturb lead-based paint in pre-1978 homes, child carefacilities <strong>and</strong> schools be certified by EPA <strong>and</strong> that they use certifiedrenovators who are trained by EPA-approved training providers to followlead-safe work practices. Contractors must provide to SAHA <strong>and</strong> tenants acopy of the EPA pamphlet “Renovate Right: Important Lead HazardInformation for Families, Childcare Providers <strong>and</strong> Schools,” beforethe renovations start. Federal law requires this in housing, child-carefacilities <strong>and</strong> schools built before 1978 <strong>and</strong> when renovating six square feetor more of painted surfaces in a room for interior projects or more thantwenty square feet of painted surfaces for exterior projects. <strong>For</strong> a copy ofthis pamphlet go towww.epa.gov/lead/pubs/renovaterightbrochure.pdf13.21 Contractor shall remove <strong>and</strong> dispose of any items or debris ab<strong>and</strong>oned inthe apartment. Dispose of these materials to an approved offsite location.Hazardous materials shall be restricted to the use of appropriate containers<strong>and</strong> disposed of in a proper <strong>and</strong> legal manner to an approved offsitelocation. This is the sole responsibility of the contractor.14.0 INSURANCE: The following table details the st<strong>and</strong>ard liability policies with therequired limits <strong>and</strong> waivers of subrogation required by SAHA of all contractorsperforming work on SAHA property.7
