Frederick Reed at Days Park.Frederick ReedDiabetes PatientFrederick Reed was diagnosedwith diabetes in 1998, and he’sbeen managing his care withthe help of his Catholic MedicalPartners physician ever since. As a primary care patient atthe Lovejoy – St. Vincent HealthCenter, he receives personalizedcare for his chronic condition, minimizing hospital visits andmaximizing quality of life. When it comes to cost savings,we’re ahead of the curve.Healthcare reform aims to provide patients with the best care, while stemming the tide of rising costs. Long before the governmentproposed sweeping changes in how healthcare is paid for and delivered,Catholic Health was taking proactive steps toward becoming a highperforming health system. By investing in electronic medical recordstechnology; creating Patient Centered Medical Homes; launching ourFirst Choice self-funded health plan; and building a seamless networkof care, Catholic Health and Catholic Medical Partners are blazingnew trails to improve healthcare in our country.90%of Catholic MedicalPartners physiciansuse electronicmedical records.Only 69 providernetworks in the U.S.have achieved this.As Chairman of theHealthcare Associationof New York State, JoeMcDonald is bringingphysicians and hospitalsinto real partnershipsthat put the patient first.Daniel SistoPresident, Healthcare Associationof New York StateIn April 2012, the federal government chose us to be one of only27 health systems in the nation to serve as an Accountable CareOrganization (ACO) – a group of doctors, hospitals and other careproviders who give coordinated, high quality care to Medicarepatients. We are uniquely qualified in our region to provide this kindof care, which replaces the existing “pay for services” model with a“pay for outcomes” model, rewarding health providers for keepingpatients healthy and reducing the overall cost of care.By becoming a high performing health system, we are transformingthe way healthcare is delivered in our community. While othersystems try to catch up, Catholic Health is officially leading the way.An ACO recognizesthe value of everyhealthcare dollar, usingresources more wisely, and coordinating careto keep people healthy. This is about being betterstewards of the system.Joe McDonald with Nirav Shah, M.D.,New York State Commissioner of Health. The only hospital system in WNYto successfully meet CMS’s StageOne Meaningful Use criteria, resulting in more than $8 million inincentives for the systemOn the IMS top 100 list ofintegrated health networks for2012, we ranked #1 in WesternNew York, #2 in New York Stateand #3 in the NortheastCatholic Health has madea $100 million investmentin advanced imaging andinformation technologyElectronic medical records improvesafety, reduce readmissions andprevent unnecessary testsAll three Catholic Health hospitalsreceived a “Five-Star” rating inCleverley + Associates’ 2011Community Value Index ®Dr. Michael Edbauer10 Chief Medical Officer, Catholic Medical Partners11
