This formalso includes out-of-state testing requests.FOREIGN LANGUAGESExaminations are administered in English. However, someexaminations are available in Spanish or Vietnamese for noadditional cost. To take one of these examinations you mustindicate your language preference when you schedule yourexamination.For languages other than English, Spanish or Vietnamese, youmay request to have the examination translated by contacting<strong>PSI</strong> and paying an additional cost for each translation. The costassociated with getting an examination translated isapproximately $850. This fee will be paid by the candidate.Please fill out the form at the end of this CandidateInformation Bulletin.Candidate’s may also bring a bound Word-to-Word translationdictionary. The dictionary cannot contain pictures ordefinitions. The feefor a duplicate score report is $15. Money order or cashier’scheck ONLY.EXAMINATION REVIEW<strong>PSI</strong>, in cooperation with TDLR, will be regularly evaluating theexaminations being administered to ensure that theexaminations accurately measure competency in the requiredknowledge areas. Your commentsregarding the questions and the examinations are welcomed.Comments will be analyzed by <strong>PSI</strong> examination developmentstaff. While <strong>PSI</strong> does not respond to individuals regardingthese comments, all substantive comments are reviewed. Theidentification must be a VALID form of government-issued IDfor example, driver’s license, state ID, or passport, which bearsyour biographical information (printed name, photograph,signature, date of birth, and expiration date).As government entities, public high school, community collegeand junior college IDs are also VALID and only require yourprinted name, photograph and date of birth.Identification provided must match all information provided byTDLR to <strong>PSI</strong> upon eligibility. A digital copy of your IDdocumentation will be taken upon check in, and stored withyour personal data.An expired driver’s license will be accepted if accompanied bya VALID Texas Department of Public Safety temporary permit.TDLR policies and standards to ensure that all candidateshave a pleasant and professional examination experience. Ifyou fail to comply with these policies, you will have yourexamination results disqualified and will forfeit yourexamination fee.• NO conversing or any other form of communication amongcandidates is permitted once you enter the examinationarea.• Cell phones, pagers, PDAs, cameras, notes, electronicdevices, and recording devices, of any kind are NOTallowed to enter <strong>PSI</strong> examination sites. Eitherone may result in the disqualification of examinationresults and may lead to legal action.COMPUTER BASED EXAMSYou will be taking the <strong>PSI</strong> Written Portion by computer. You donot need any computer experience or typing skills. You willuse fewer keys than you use on a touch-tone telephone. Allresponse keys are colored and have prominent characters. Anillustration of the special keyboard is shown here. You mayalso use the mouse.If you cannot provide the required identification, you mustcall (800) 733-9267 at least 3 weeks prior to your scheduledappointment to arrange a way to meet this securityrequirement. Failure to provide the required identificationat the time of the examination without notifying <strong>PSI</strong> isconsidered a missed appointment, and you will not be ableto take the examination and will forfeit your examinationfee.SECURITY PROCEDURESThe following security procedures will apply during theexamination:• While at a <strong>PSI</strong> examination site, you are considered to be aprofessional and shall be treated as such. In turn, you mustconduct yourself in a professional manner at all times.While at the site, you shall not use words or take actionsthat are vulgar, obscene, libelous, or that would denigratethe <strong>PSI</strong> staff or other candidates. IDENTIFICATION SCREENYou will be directed to a semi-private testing station to takethe examination. When you are seated at the testing station,you will be prompted to confirm your name, identificationnumber, and the examination for which you are registered.TUTORIALBefore you start your examination, an introductory tutorial tothe computer and keyboard is provided on the computerscreen. The time you spend on this tutorial, up to 15 minutes,DOES NOT count as part of your examination time. Samplequestions are included following the tutorial so that you maypractice using the keys, answering questions, and reviewingyour answers.One question appears on the screen at a time. During theexamination, minutes remaining will be displayed at the top ofthe screen and updated as you record your answers.EXAMINATION QUESTION EXAMPLEDuring the examination, you should press 1, 2, 3, or 4 to selectyour answer or press “MARK” to mark it for later review. The topicyou select for your presentation MUST be within the scopeof the license for which you are applying and must includethe demonstration of a procedure. Only mock chemicalsmay be used in the presentation, such as gel, cholesterol,water, etc. No hairspray may be used. Only mannequinsmay be used, not human models.• Your lesson plan and cover sheet must be prepared inadvance on the form or in the format prescribed by theTDLR, which will be made available on the TDLR websiteas a Word document that you can download and adjust sothat your lesson plan fits in the space provided.
