SCHEDULING AN APPOINTMENT TO TAKE THE EXAMINATIONOnce you have made payment for your examination services,you are ready to schedule your examination. It is important toremember that you are responsible for contacting PSI toschedule your examination. PSI will make every effort toschedule the examination site and time that are mostconvenient for you. Applicants with disabilities orthose who would otherwise have difficulty taking theexamination must fill out the form at the end of this CandidateInformation Bulletin and fax to PSI (702) 932-2666. This formalso includes out-of-state testing requests.FOREIGN LANGUAGESExaminations are administered in English. However, someexaminations are available in Spanish or Vietnamese for noadditional cost. To take one of these examinations you mustindicate your language preference when you schedule yourexamination.For languages other than English, Spanish or Vietnamese, youmay request to have the examination translated by contactingPSI and paying an additional cost for each translation. The costassociated with getting an examination translated isapproximately $850. This fee will be paid by the candidate.Please fill out the form at the end of this CandidateInformation Bulletin.Candidate’s may also bring a bound Word-to-Word translationdictionary. The dictionary cannot contain pictures ordefinitions. Candidates are not permitted to bring in anyadditional papers (loose or attached) with their dictionary.Highlighting, underlining, indexing or marking in the dictionaryprior to or during the examination is prohibited. Any candidatecaught writing, highlighting, underlining, and or indexing in thedictionary during the examination will be subject to forfeitingtheir dictionary, termination of their examination andreporting the incident to the Texas Department of Licensingand Regulation.EXAMINATION SITE CLOSING FOR AN EMERGENCYIn the event that severe weather or an emergency forces theclosure of an examination site on a scheduled examinationdate, your examination will be rescheduled. Every effort will be made to reschedule yourexamination at a convenient time as soon as possible.DUPLICATE SCORE REPORTSYou can write to PSI, not TDLR, to request a duplicate of yourscore report for up to 1 year after your examination. The feefor a duplicate score report is $15. Money order or cashier’scheck ONLY.EXAMINATION REVIEWPSI, in cooperation with TDLR, will be regularly evaluating theexaminations being administered to ensure that theexaminations accurately measure competency in the requiredknowledge areas. Comments may be entered on the computerkeyboard during the written examination. Your commentsregarding the questions and the examinations are welcomed.Comments will be analyzed by PSI examination developmentstaff. While PSI does not respond to individuals regardingthese comments, all substantive comments are reviewed. If,on a rare occasion, an error affecting examination scores isdiscovered, the scores of all affected candidates will beautomatically adjusted. This is the only review ofthe written examination available to candidates. NOconversing or any other form of communication is permittedonce you enter the examination area. You must wear closedtoeshoes. If you arrive late or do not appear in the appropriateattire, you will not be admitted to the examination site andyou will forfeit your examination registration fee.REQUIRED IDENTIFICATION AT EXAMINATION SITEYou must provide 1 form of identification (ID). Theidentification must be a VALID form of government-issued IDfor example, driver’s license, state ID, or passport, which bearsyour biographical information (printed name, photograph,signature, date of birth, and expiration date).As government entities, public high school, community collegeand junior college IDs are also VALID and only require yourprinted name, photograph and date of birth.Identification provided must match all information provided byTDLR to PSI upon eligibility. A digital copy of your IDdocumentation will be taken upon check in, and stored withyour personal data.An expired driver’s license will be accepted if accompanied bya VALID Texas Department of Public Safety temporary permit.TDLR policies and standards to ensure that all candidateshave a pleasant and professional examination experience. Ifyou fail to comply with these policies, you will have yourexamination results disqualified and will forfeit yourexamination fee.• NO conversing or any other form of communication amongcandidates is permitted once you enter the examinationarea.• Cell phones, pagers, PDAs, cameras, notes, electronicdevices, and recording devices, of any kind are NOTallowed to enter PSI examination sites. Additionally, guestvisitors and/or family members are not allowed at theexamination site. NO personal items are to enter theexamination site. A locker will be provided to store a fewsmall items. PSI will not be responsible for any personalitems, and suggests that you leave such items in anothersafe place, of your choosing.• Cell phones are NOT allowed in the written examinationroom. If you take your phone in the examination room andit rings or is used in any way your examination will beterminated and an incident report will be submitted toTDLR.• Only non-programmable calculators that are silent, batteryoperated,do not have paper tape printing capabilities, anddo not have a keyboard containing the alphabet will beallowed in the examination site.• No smoking, eating, or drinking will be allowed at theexamination site.• Copying or communicating examination content is aviolation of PSI security policy and Texas State Law. Eitherone may result in the disqualification of examinationresults and may lead to legal action.COMPUTER BASED EXAMSYou will be taking the PSI Written Portion by computer. You donot need any computer experience or typing skills. You willuse fewer keys than you use on a touch-tone telephone. Allresponse keys are colored and have prominent characters. Anillustration of the special keyboard is shown here. You mayalso use the mouse.If you cannot provide the required identification, you mustcall (800) 733-9267 at least 3 weeks prior to your scheduledappointment to arrange a way to meet this securityrequirement. Failure to provide the required identificationat the time of the examination without notifying PSI isconsidered a missed appointment, and you will not be ableto take the examination and will forfeit your examinationfee.SECURITY PROCEDURESThe following security procedures will apply during theexamination:• While at a PSI examination site, you are considered to be aprofessional and shall be treated as such. In turn, you mustconduct yourself in a professional manner at all times.While at the site, you shall not use words or take actionsthat are vulgar, obscene, libelous, or that would denigratethe PSI staff or other candidates. IDENTIFICATION SCREENYou will be directed to a semi-private testing station to takethe examination. When you are seated at the testing station,you will be prompted to confirm your name, identificationnumber, and the examination for which you are registered.TUTORIALBefore you start your examination, an introductory tutorial tothe computer and keyboard is provided on the computerscreen. The time you spend on this tutorial, up to 15 minutes,DOES NOT count as part of your examination time. Samplequestions are included following the tutorial so that you maypractice using the keys, answering questions, and reviewingyour answers.One question appears on the screen at a time. During theexamination, minutes remaining will be displayed at the top ofthe screen and updated as you record your answers.EXAMINATION QUESTION EXAMPLEDuring the examination, you should press 1, 2, 3, or 4 to selectyour answer or press “MARK” to mark it for later review. Oryou may simply click on the box to the left of your answer withthe mouse. You should then press “ENTER” to record youranswer (or click on Next with the mouse) and move on to thenext question. • On screen – your results will appear immediately on thecomputer screen. This will happen automatically at theend of the time allowed for the examination; if you areusing review features, you will be able to obtain yourscore immediately when you indicate that you havefinished and would like to see your results.- If you pass, you will immediately receive a successfulnotification.- If you do not pass, you will immediately receive anunsuccessful notification on the screen along with adiagnostic report indicating your strengths andweaknesses. Registration forms for submittal to PSI toretake the examination will be available at theexamination site.• On paper – an official score report will be printed at theexamination site.PRACTICAL PORTIONFor practical examination questions or concerns, please emailcosmetology@psionline.com.Read the following instructions carefully so that you will beproperly prepared for the examination.• You may select the topic for your presentation. The topicyou select for your presentation MUST be within the scopeof the license for which you are applying and must includethe demonstration of a procedure. Only mock chemicalsmay be used in the presentation, such as gel, cholesterol,water, etc. No hairspray may be used. Only mannequinsmay be used, not human models.• Your lesson plan and cover sheet must be prepared inadvance on the form or in the format prescribed by theTDLR, which will be made available on the TDLR websiteas a Word document that you can download and adjust sothat your lesson plan fits in the space provided. Thelesson plan MUST BE TYPED, not handwritten. Lesson planspresented in any other format will not be accepted andyou will NOT receive any points for the Portion of theexamination that involves the examiner’s evaluation of thelesson plan. No time will be allowed for preparation ormodification of your lesson plan at the test site; you willbe expected to hand in your lesson plan and handout uponarrival.• You will be required to furnish all equipment and suppliesneeded for your presentation, including a mannequin ifone is needed. You may NOT use a live model for yourpresentation. See below for a list of the equipment thatwill be provided at the test site. It is your responsibility todevelop a presentation that can be performed in thislimited setting.• It is important to have all needed supplies and equipmentwith you when you arrive for the exam. You will NOT bepermitted to leave the exam area once you arrive and aresigned in.• Test site personnel (a proctor) will be monitoring the timeduring the examination. Candidates must conduct theirpresentation for at least 10 minutes, not to exceed 35minutes. The examiner will stop any candidate at 35minutes and no further material presented will be rated.Candidates will FAIL the examination if their presentationdoes not last at least 10 minutes. Candidates will alsohave 10 minutes before the presentation begins to set upand arrange their materials.• All supplies must be clean, sanitary, unstained, andlabeled in ENGLISH ONLY (manufacturer labels areacceptable).• Items left behind will be discarded.
