Pleasereview it carefully.When this notice applies:This notice summarizes the privacy practices of BaptistHospital of Miami, Baptist Children’s Hospital, BaptistOutpatient Services, Baptist Health Home Care, DoctorsHospital, Homestead Hospital, Mariners Hospital, Medical ArtsSurgery Center at Baptist Hospital, Medical Arts SurgeryCenter at South Miami Hospital, South Miami Hospital,Baptist Cardiac & Vascular Institute, South Florida SpecialtyPhysicians, LLC, Baptist Hospital Neurosurgery, BaptistHospital Inpatient Medicine, Upper Keys Specialty Physicians,LLC, Upper Keys Orthopedics and Sports Medicine, UpperKeys Family Medicine, Upper Keys Gastroenterology, CoralGables Specialty Physicians, LLC, UHZ Sports MedicineInstitute, Orthopedic Institute of South Florida, LLC, and themedical staffs and personnel who provide you with care or servicesat these Baptist Health facilities. In addition, we may shareinformation with each other for purposes described in this notice,including for our joint healthcare operations activities.Our obligations:We have a long-standing commitment to protecting theprivacy rights of our patients. In keeping with this commitment,and as we are required by law, we:■ Obtain your consent to use and disclose records about yourhealth and healthcare;■ Maintain the privacy of protected health information;■ Give you this notice of our legal duties and privacy practicesregarding health information about you; and■ Follow the terms of our notice of privacy practices that arecurrently in effect.How we may use and disclosehealth information:When you receive services or treatment at a Baptist Healthfacility, you will be asked to sign an acknowledgment of thisnotice, which describes how we use and disclose informationabout you in ways that are permitted by federal law. The federallaw provides for significant privacy protections of healthinformation. If you receive treatment from the Baptist Healthalcohol or substance abuse program, you will receive a separatenotice describing how we may use and disclose informationfrom that program.The following categories describe ways that we may use anddisclose health information that identifies you (“HealthInformation”). Some of the categories include examples, butnot every type of use or disclosure of Health Information in acategory is listed. Except for the purposes described below, wewill use and disclose Health Information only with additionalwritten permission from you. If you give us permission to useor disclose Health Information for a purpose not discussed inthis notice, you may revoke that permission at any time bysending a written request to our Chief Privacy Officer at theaddress listed at the end of this notice.
a) For Treatment. We may use Health Information to treatyou or provide you with healthcare services. We may discloseHealth Information to doctors, nurses, technicians orother personnel, including people outside our facility whomay be involved in your medical care. For example, wemay tell your primary care physician about the care weprovided you or give Health Information to a specialist toprovide you with additional services.b) For Payment. We may use and disclose HealthInformation so that we or others may bill or receive paymentfrom you, an insurance company or a third party for thetreatment and services you receive. For example, we maygive your health plan information about your treatment so thatthey will pay for such treatment. We also may tell yourhealth plan about a treatment you are going to receive toobtain prior approval or to determine whether your plan willcover the treatment.c) For Healthcare Operations. We may use and discloseHealth Information for healthcare operations and administrativepurposes. These uses and disclosures are necessary tomake sure that all of our patients receive quality care and forour operation and management purposes. For example, wemay use Health Information to review the treatment and serviceswe provide to ensure that the care you receive is of thehighest quality, or we may post in employee areas thank-younotes or pictures that you send us.d) Appointment Reminders, Treatment Alternatives andHealth-related Benefits and Services. We may use and discloseHealth Information to contact you as a reminder thatyou have an appointment with us. We also may use and discloseHealth Information to tell you about treatment optionsor alternatives or health-related benefits and services thatmay be of interest to you.e) Fundraising Activities. We may use certain limitedHealth Information to contact you in the future to raisemoney for us. We also may provide this information to ourFoundation, for the same purpose. The money raised will beused to expand and improve the clinical services and programswe provide the community.f) Facility Directory. We may list your name, generalcondition (e.g., fair, critical) and location in our directory,unless you ask us not to. We may disclose this information toanyone who asks for you by name.g) Pastoral Care. We may disclose the information in ourfacility directory and information that you provide us regardingyour religious affiliation to members of the clergy for useand disclosure in their religious activities. In addition, whereyou elect to receive individualized spiritual services from amember of our Pastoral Care department, the clergy membermay use Health Information to provide you with such services.h) Individuals Involved in Your Care or Payment forYour Care. We may disclose Health Information to a person,such as a family member or friend, who is involved in yourmedical care or who helps pay for your care. We also maynotify your family about your location or general conditionor disclose such information to an entity assisting in a disasterrelief effort.i) Research. Under certain circumstances, we may use
