In addition, we may shareinformation with each other for purposes described in this notice,including for our joint healthcare operations activities.Our obligations:We have a long-standing commitment to protecting theprivacy rights <strong>of</strong> our patients. In keeping with this commitment,and as we are required by law, we:■ Obtain your consent to use and disclose records about yourhealth and healthcare;■ Maintain the privacy <strong>of</strong> protected health information;■ Give you this notice <strong>of</strong> our legal duties and privacy practicesregarding health information about you; and■ Follow the terms <strong>of</strong> our notice <strong>of</strong> privacy practices that arecurrently in effect.How we may use and disclosehealth information:When you receive services or treatment at a <strong>Baptist</strong> <strong>Health</strong>facility, you will be asked to sign an acknowledgment <strong>of</strong> thisnotice, which describes how we use and disclose informationabout you in ways that are permitted by federal law. The federallaw provides for significant privacy protections <strong>of</strong> healthinformation. If you receive treatment from the <strong>Baptist</strong> <strong>Health</strong>alcohol or substance abuse program, you will receive a separatenotice describing how we may use and disclose informationfrom that program.The following categories describe ways that we may use anddisclose health information that identifies you (“<strong>Health</strong>Information”). Some <strong>of</strong> the categories include examples, butnot every type <strong>of</strong> use or disclosure <strong>of</strong> <strong>Health</strong> Information in acategory is listed. Except for the purposes described below, wewill use and disclose <strong>Health</strong> Information only with additionalwritten permission from you. If you give us permission to useor disclose <strong>Health</strong> Information for a purpose not discussed inthis notice, you may revoke that permission at any time bysending a written request to our Chief <strong>Privacy</strong> Officer at theaddress listed at the end <strong>of</strong> this notice.
