In the case ofinmates of a correctional institution or those who are underthe custody of a law enforcement official, we may releaseHealth Information to the appropriate correctional institutionor law enforcement official. This release would be made onlyif necessary (1) for the institution to provide you with healthcare;(2) to protect your health and safety or the health andsafety of others; or (3) for the safety and security of the correctionalinstitution.Your rights:You have the following rights regarding Health Informationwe maintain about you:a) Right to Inspect and Copy. You have the right to inspectand copy Health Information that may be used to make decisionsabout your care or payment for your care.b) Right to Amend. If you feel that Health Information wehave is incorrect or incomplete, you may ask us to amend theinformation. You have the right to request an amendment foras long as the information is kept by or for us. You must tellus the reason for your request.c) Right to an Accounting of Disclosures. You have theright to request an accounting of certain disclosures of HealthInformation we made.d) Right to Request Restrictions. You have the right torequest a restriction or limitation on the Health Informationwe use or disclose for treatment, payment or healthcare operations.Please note that we will not grant requests for restrictionsthat pertain to your treatment. In addition, you have theright to request a limit on the Health Information we discloseabout you to someone who is involved in your care or thepayment for your care, such as a family member or friend.For example, you could ask that we not share informationabout your surgery with your spouse. We are not required toagree to your request. If we agree, we will comply with yourrequest unless we need to use the information in certainemergency treatment situations.e) Right to Request Confidential Communications. Youhave the right to request that we communicate with you aboutmedical matters in a certain way or at a certain location. Forexample, you can ask that we contact you only by mail or atwork. Your request must specify how or where you wish to becontacted. We will accommodate reasonable requests.f) Right to a Paper Copy of This Notice. You have the rightto a paper copy of this notice. You may ask us to give you acopy of this notice at any time. Even if you have agreed toreceive this notice electronically, you are still entitled to apaper copy of this notice. You may obtain a copy of thisnotice at the Baptist Health website, www.baptisthealth.net,or, if you are a Baptist Health employee, on the Baptist HealthSun intranet.
