<strong>Evergreen</strong> Then and Now1971When we walk through the hospital’s corridors, we’re often reminded just how much <strong>Evergreen</strong><strong>Healthcare</strong> has grown since 1972 when we opened the doors. Back then, we had 72 beds andwere proud of offering 15 medical specialties. In that first year, we delivered 367 babies. Nowwe have 227 beds and more than 50 medical specialties, and last year our maternity centerdelivered more than 4,778 babies, the second busiest program in the state. We have more than3,000 employees, with 280 of them just recently celebrating over 10 years or more with us. Inthis mobile society, that’s a huge compliment to earn such loyalty from our staff.We don’t mean to bore you with numbers, but they do demonstrate several things. Morepeople are choosing <strong>Evergreen</strong> for their health care than ever before. We think that’s because ofresources like our world-class doctors and enthusiastic and caring employees. We offer top-notchtechnology in a pleasant, professional setting, with highly personalized care. We’ve worked hardall year to prepare our Neuroscience Institute and our enhanced cardiovascular wellness programfor their move to the DeYoung Pavilion so they can continue to grow these critical services.We understand that when <strong>Evergreen</strong> was built, someone asked why the hospital was so far fromdowntown Kirkland. Today we’re proud to say we’re in the middle of our service area, and inone of the fastest-growing population centers in the state. We’re not far from anything in ourdistrict and even continue to attract more and more patients from the rest of the state. Peoplechoose <strong>Evergreen</strong> for a variety of reasons, from our award-winning Family Maternity Center tothe Booth Gardner Parkinson’s Care Center. We have medical clinics in Kenmore, Canyon Park,Duvall, Redmond, Sammamish and Woodinville, plus a senior health clinic here in Kirkland.We’ve attracted national attention recently for earning top ratings by HealthGrades, the premierhospital tracking system in the USA.We could go on and on. Cancer Care2 3Breast HealthRadiation Oncology“There’s no institution internationally that’s everreported a higher local control rate for early-stagebreast cancer, meaning the odds that you’ll keepyour breast if you’re treated with breast-conservingtherapies at <strong>Evergreen</strong>,” says Michael Hunter, MD,medical director of the Cancer Program. “We’re at98 percent – and these results rank with the bestin the world – so we’re doing a pretty good jobhere,” he says.Staying free from cancer recurrence is associatedwith higher survival rates. “The idea that you canget MD Anderson-level care in your own backyardis foreign to many people,” Dr. Hunter says. Butthe impressive results show that it’s true.The successful outcomes aren’t all that’s new.“In 2008, we introduced stereotactic radiationtherapy, a program initially designed for treatingbrain tumors,” says Dr. Hunter. “The idea is to usemultiple beams from multiple angles that converge,so the dose is high where they overlap, ideal forcertain brain tumors. Our new technology willalso allow for better targeting of moving tumors,such as lung cancer.” A feature called respiratorygating synchronizes each dose with a patient’sbreathing cycle to hit the cancer while sparingmore healthy tissue.In 2008, <strong>Evergreen</strong> installed one of the moststate-of-the-art linear accelerators – the machinesused to deliver radiation – available anywhere inthe world. The new Varian Trilogy offers a newlevel of precision, with the ability to do stereotacticradiation therapy and image-guided radiationtherapy (IGRT). “Educated patients ‘google’these terms now,” says Sue Smiley, manager ofradiation oncology and cancer support services.“They look for cancer centers, like <strong>Evergreen</strong>’s,that offer this technology.”“There’s no institution internationallythat’s ever reported a higher localcontrol rate for early-stage breastcancer.”Comprehensive Cancer Care– Michael Hunter, MDOnly programs offering the best comprehensivecancer care are accredited by the AmericanCollege of Surgeons Commission on Cancer – arecognition that <strong>Evergreen</strong> has achieved everythree years since 1977.“Comprehensive care means we offer the fullscope of services, from state-of-the-art screening,detection, diagnostics and treatment to supportiveservices for patients and families,” says Sue Smiley,manager of radiation oncology and cancersupport services.In 2008, <strong>Evergreen</strong> achieved the highest honorgiven by the Commission on Cancer – theOutstanding Achievement Award – a recognitionthat only three other Washington hospitals earned.Cascade Cancer Center, the primary medicaloncology, or chemotherapy, provider for <strong>Evergreen</strong>,is a member of the Seattle Cancer Care Alliance,an affiliation that helps patients access clinicaltrials. <strong>Evergreen</strong>’s radiation oncology service offersleading-edge radiation therapy in an environmentthat ranks among the highest patient satisfactionscores in the nation.Cancer survivorship, a topic of growing nationalinterest, has become a unique field within cancercare. “A cancer survivor is anyone living with ahistory of cancer, from the moment of diagnosisthrough the rest of life,” says Smiley. To help meetsurvivors’ needs, <strong>Evergreen</strong> partners with nationaland community resources such as the AmericanCancer Society and Seattle’s Cancer Lifeline tooffer support groups, classes and exercise programs,and to host an annual survivorship retreatfor patients and families for celebration, educationand support.<strong>Evergreen</strong> Breast CenterDigital mammography has revolutionized breastcancer screening.Rather than peering at an X-ray film with amagnifying glass as they did a generation ago,radiologists such as Kara Carlson, MD, can nowmagnify, contrast and invert digital mammogramson high-resolution computer monitors.“Washington still ranks third in our country forthe incidence of breast cancers,” says Dr. Carlson,medical director of the <strong>Evergreen</strong> Breast Center.“Nearly 4,000 women will be diagnosed in our statethis year, so we really need to be at the forefront oftechnology for early detection.”In 2008, the Breast Center team designed betterways to identify women at high risk for developingInspired GivingDigital MammograpHy CoachThe <strong>Evergreen</strong> Breast Center uses the latest, mostadvanced digital mammography technology.breast cancer. “Some women benefit from high-riskscreening with breast MRI in addition to mammographyexams,” Dr. Carlson says. “We do a riskassessment on every woman coming in for routinescreening and calculate her five-year and lifetime risk,giving us an opportunity to educate women.”In January 2008, the Breast Center also launched amajor streamlining effort to fine-tune patient flowand provider workflow. After reversing a backlogin patient scheduling, the team now performs130 to 135 mammograms a day – and can focuson getting more women in for screenings and onexpanding services.“We’re down to next-day scheduling for ascreening mammogram and for diagnostic workups,”says Dennis Marlow, director of DiagnosticImaging.The comprehensive Breast Center offers digitalmammography, ultrasound, MRI, biopsies, andreferrals to surgery, radiation oncology, medicaloncology and genetic testing.“<strong>Evergreen</strong> has an incredible multidisciplinaryteam,” says Dr. Carlson. “We discuss new breastcancer cases in a forum that gives patients thebenefit of our pathologists, surgeons, medicaloncologists and radiation oncologists, all givinginput on the best treatment for each patient.”Signs of Growth: <strong>Evergreen</strong> Breast Center2007 / 2,200 mammograms and biopsies monthly2008 / 2,550 mammograms and biopsies monthlyA dream took shape, several years ago, among members of the Breast Center team. As radiologists, theyknew that the earlier a cancer was found, the better the prognosis.So to make it easier for more women to get breast cancer screenings, the team envisioned a way to take<strong>Evergreen</strong>’s high-tech, digital mammography out into surrounding communities. Their vision: Build a digitalmammography coach to drive out to each of the primary care clinics in the <strong>Evergreen</strong> Medical Group on arotating basis, so that women in outlying towns could see their local family doctors and get a mammogramwhile they were there.When the team presented its idea to the audience at the 2008 <strong>Evergreen</strong> Gala, several guests offereddonations ranging from $1,000 to $20,000, raising an initial $200,000 of capital support. Hearing of the need,Al and Donna DeYoung generously gave a matching gift of another $200,000.Digital mammograms on the coach offer the same state-of-the-art technology used at the <strong>Evergreen</strong> BreastCenter – a level of quality not often found in mobile units – and are performed by the same level of certifiedradiologic technologist. Set to begin seeing patients in summer 2009, the mobile coach is expected to increasethe Breast Center’s mammograms by 20 percent.


Orthopedics4 5Maternal-Fetal Medicine“We’re one of the few centers inthe region now using artificial disctechnology.”– Reginald Knight, MDComputer-Assisted Surgery<strong>Evergreen</strong>’s orthopedics team continues to expandand fine-tune its computer-assisted surgery techniquesfor total joint replacement.“Most of our surgeons now use computer-assistednavigation for their total joint surgeries,” says JeffTomlin, MD, medical director for surgical servicesat <strong>Evergreen</strong>. “More precise incisions on the boneincrease the longevity of the new joint, since a degreeor two closer to precise alignment of the weightbearingplane can make a significant difference.”Minimally invasive surgery often results in a quickerand less painful recovery. “All of our surgicalspecialties continue to develop these techniques,”Tomlin says. “Our chief of orthopedics, CraigMcAllister, MD, has developed a lab at <strong>Evergreen</strong>where minimally invasive techniques and computerassistedsurgery are taught to OR staff andother surgeons.”In 2008, total knee replacement was the numberone surgery performed in <strong>Evergreen</strong>’s OR. Thefuture will likely see computer-assisted techniquesused in total hip surgeries as the technologydevelops.Charles Chabal, MD,holds the latestspinal cord stimulator.Spine Surgery“It’s a very exciting time,” says Reginald Q. Knight,MD, who is fast becoming internationally knownfor his research on minimally invasive spine surgerytechniques and artificial disc replacement.A surgeon’s dilemma in the past, after removing apainful disc, was how to fill the gap created whenthe disc was removed. Fusing the joints meanta patient would lose mobility and transfer morestress to neighboring discs.“With artificial disc replacement, we now have away of maintaining motion after surgery and oftendecreasing future degeneration down the line,” Dr.Knight explains. “That’s why we’re all so happyabout this new technology. We brought artificialdisc technology to <strong>Evergreen</strong> and completed aprospective randomized trial in 2008.” <strong>Evergreen</strong>is now one of the few centers in the region usingartificial discs for the neck and lower back.In 2008, Dr. Knight and his team established theSeattle Minimally Invasive Spine Center, based at<strong>Evergreen</strong>. “We’re now the busiest minimally invasivespine surgery center in the region – perhaps inall of Washington,” he says.Interventional Pain Management“It’s truly gratifying to turn people’s lives around,”says Charles Chabal, MD.At the forefront of pain management for 20 years,Dr. Chabal has brought new technologies to<strong>Evergreen</strong> to help people with intractable painreturn to work and to more fully enjoying their lives.A relatively new specialty, interventional painmanagement has recently exploded at <strong>Evergreen</strong>,from about 10 procedures a month in 2003 to300 procedures a month in 2008. “Interventional”procedures include a variety of injections, surgicallyimplanted devices and other techniques, often usedto relieve back pain.“We now have a successful new treatment for lowback pain in the sacroiliac joint, called radiofrequencySigns of Growth: Orthopedics2007 / 469 total knee replacements/ 779 spine procedures2008 / 542 total knee replacements/ 861 spine procedureslesioning, which may provide long-lasting pain relieffrom heating the nerves,” Dr. Chabal says. Spinal cordstimulators, high-tech devices used for back pain, arenow smaller, easier to implant, and more programmableto relieve each patient’s unique pain pattern.“We can now put in a trial stimulator in an outpatientprocedure, so the patient can try it and decidewhether to have a permanent implant,” he says.In 2008, Dr. Chabal launched the MultidimensionalPain Profiler, an online pain assessment tool hecreated with funding from NIH, to measure painfactors such as depression, substance abuse,openness to change and dysfunctional pain beliefs.“We’re using technology to better understand thedimensions of a patient’s pain psychology, improvequality of care and track outcomes,” he says.Fetal TherapyCompassion and a human touch, Martin Walker,MD, believes, are as crucial in caring for womenwith high-risk pregnancies as medical expertise.His high-tech, high-touch approach attractspatients from as far away as Florida and Alaska.He and his team perform the most delicate of inutero procedures, using a microsurgical instrumentcalled a fetoscope to reach babies still in the womb.Internationally known for his success in treatingtwin-to-twin transfusion syndrome, a complicationof twinning, Dr. Walker’s specialty is fetoscopiclaser ablation therapy – the use of laser to close offproblematic, shared blood vessels between twins.“Anything we do in the uterus has to be minimallyinvasive,” he says, “but we’re one of the fewcenters doing fetal therapy exclusively in aminimally invasive way.”“In 2008, we started using a new technology,radiofrequency ablation, for severely abnormaltwins called TRAP sequence twins,” Dr. Walkersays. “We also started doing the ‘amnio patchprocedure,’ patching amniotic membranes if they’druptured, and in utero platelet transfusions.” In2008, he trained a second physician, Bettina Paek,MD, to perform laser ablation, to offer bettercoverage for patients.“I’m passionate about doing a good technicaljob of taking care of these very complicatedtwin pregnancies, but more than that, it’s apersonal connection.”– Martin Walker, MD, with twinsKatie and Anna Dennis, from his fetal therapy program<strong>Evergreen</strong>’s fetal therapy program offers anumber of in utero treatments, such as fetal bloodtransfusions, for babies still in the womb. “It’snot one or two people, but a whole team: ourcoordinator, Melissa; our sonographers; nurses;and all of the folks in the operating room whomake it happen,” Dr. Walker says. His team heldits first annual fetal therapy reunion in 2008. “Wehad mums and their families come for a dinner anda 5K running race, the Race for Hope,” he says.Dr. Walker continues to participate in researchtrials using laser to treat high-risk twins. “Butwhat makes us special in the world of fetaltherapy is looking after these very complicatedtwin pregnancies in a way that makes parents feelcomfortable being with us,” he says. “We’re kindto them, and we really strive to create a personalconnection with all of our patients.”


Hospice and Palliative Care6 7Spiritual Care“Hospice is really about howpeople live; people often find asense of community at times theywould expect it the least.”– Hope Wechkin, MDHospice and Palliative CareStanding in the Hospice Center’s Rose Gardenlast July, Hope Wechkin, MD, saw a familiar face.He was the young Mexican-American nephewof an elderly farmer who had spent his last daysat the Hospice Center, and he’d traveled back to<strong>Evergreen</strong> with his extended family from Yakima toremember the uncle he’d lost.“People come back year after year to the annualHospice Center gathering and say they wouldn’tmiss it,” says Dr. Wechkin, medical director of<strong>Evergreen</strong> Hospice and Palliative Care. “It’s a wayto remember the person you lost and reconnectwith the nurses and social workers who were therewith you. It’s not just about the medical care; it’sabout the community.”It was a year of tremendous growth for hospice andpalliative care. In 2008, the hospice team openeda West Seattle office to meet the growing needfor hospice care beyond Kirkland and Edmonds.“While we’re headquartered at <strong>Evergreen</strong> andcommitted to our public hospital district, wenow see patients all over King and Snohomishcounties,” says Dr. Wechkin. Her team alsoexpanded its Open Access program, making iteasier for patients to receive the full six monthsof hospice care benefits they’re entitled to, evenif they’re still receiving therapies such as palliativeradiation.Palliative care, a new medical specialty, is an approachto end-of-life care that focuses on symptom managementand quality of life, led by each patient’s goalsof care.“There’s a growing interest in palliative care,”Dr. Wechkin says. “We’re doing a lot to educatenot only referring physicians in local hospitals,but also the next generation of residents, nursesand social workers coming up. They want to learnwhat we’re learning about palliative care – and seewhat works.”Signs of Growth: Hospice and Palliative CareHospice2007 / 1,586 patients2008 / 1,806 patientsPalliative Care2007 / 103 patients2008 / 182 patientsSpiritual Care“People want to be heard during times ofuncertainty, more than they’re concerned withwhich spiritual group you belong to,” says DaveWendleton, director of Spiritual Care and ClinicalPastoral Education at <strong>Evergreen</strong>. “People want toknow: Can you listen to them and respond to theirhurt and pain? Do you judge them or not? How doyou companion them through a difficult time?”<strong>Evergreen</strong> is the only health care center onthe Eastside – and one of about 400 centersnationwide – to offer Clinical Pastoral Education,a nationally accredited pastoral training program, aspart of its spiritual care. <strong>Evergreen</strong>’s nondenominationalprogram brings in four pastoral residentseach year from various faiths to provide on-callspiritual care for patients and their families,24 hours a day.“We’re not a huge hospital, but we have a strongcommitment to spiritual care,” says Wendleton.“<strong>Evergreen</strong>’s commitment originally came fromIntimate by design, the new chapel is always open toanyone seeking solace and a few moments to reconnectquietly with heart and soul.our community advisors, who said that spiritualcare should be looked at closely, and if we do it,we should do it right.“We’re here to provide a safe place for people todo the work they need to do when facing a healthcrisis,” Wendleton says. “Each of our residentsworks in both the hospital and hospice, offeringa holistic approach that considers the impact oneveryone involved with caring for and loving aperson facing a crisis.”Through its Faith in Action program, the teamhelps local congregations provide spiritual carefor their members who are facing end-of-life ormedical crises, teaching them how to talk withpeople who are seriously ill and use humor andstorytelling to bring comfort. Faith in Action,through its outreach coordinator, Donna Oiland,created a book, Legacy: Reflections Along the Way, tohelp patients and families share their life storieswith each other.Inspired GivingTHE ROSE GARDENA poem, song or picture often graces the deeply personal memorial plaques in the Hospice Center’s RoseGarden, giving it a paradoxically “alive” feeling. It’s a place to remember not how people died – but how they lived.At <strong>Evergreen</strong>, families may choose to remember their loved ones with an intimate, personally engraved plaque, ora granite or adobe brick. In 2008, 75 donors gave more than $45,000 to the Hospice Center through memorialplaques and bricks. The welcome addition of memorial benches now gives families another opportunity for a lasting,personal tribute.“Since hospice care provides a full year of bereavement support for family members, we’re in people’s lives for longerthan a few weeks,” says Hope Wechkin, MD. “The hospice becomes woven into their lives, so many families leavebequests to <strong>Evergreen</strong> Hospice, wanting to give so that others can receive the compassionate care they’ve had.”Donations to <strong>Evergreen</strong> Hospice in 2008 totaled close to $280,000, including contributions for memorial plaques,bricks and other gifts. These generous donations help fund renovations to the 15-bed Hospice Center and itsgrounds, and support ongoing expenses such as training for staff and volunteers.Inspired GivingThe Elling and Barbara Halvorson ChapelCome to <strong>Evergreen</strong> early in the morning, and you’re likely to see a handful of people beginning their day in<strong>Evergreen</strong>’s new chapel, a beautiful, meditative and prayerful space that opened in October 2008.“It’s a place for people from all faith traditions to come pray, meditate and seek comfort, and is used bypatients, families and staff,” says Dave Wendleton, director of Spiritual Care. “We have a prayer request bookin the chapel where anyone can write prayers, and we read those prayers every day and remember the peoplewho request specific prayers.”Funded primarily by a generous gift from Elling and Barbara Halvorson, longtime friends of <strong>Evergreen</strong>, thenondenominational chapel also received a donation from the <strong>Evergreen</strong> <strong>Hospital</strong> Auxiliary. Local artists werecommissioned to provide artwork, with the focal piece created by Cassandria Blackmore, a reverse glasspainter from Duvall.“It’s a blessingto the wholeexperience, forpatients andfamily membersgoing throughchallenging times.”– Dave Wendleton

We wantthe family to feel intimately involved in every partof care.”“We’ve probably tripled the length of stay since2006, when the program first started, so ourpatients are now staying a month to two months,”says Brady. “We’re now getting transfers infrom other hospitals, as providers recognize thatthis is a place where they want to send their highriskpatients.”In 2008, her team created the Healing Wall, madeof hand-painted tiles created by families duringhigh-risk pregnancy. “As a way to share the difficultjourney they’ve been on, it has touched a lot offamilies,” Brady smiles. “They come back andshare it with their children.”Keeping Families ConnectedGeorgia Harlowe knows well the worry of howto stay connected during a medical crisis. As abreast cancer patient who underwent surgery andchemotherapy at <strong>Evergreen</strong>, Harlowe was also oneof the first patients to use CarePages, a patientcommunication and networking Web site launchedin 2008, to stay in touch with family and friends.She posted messages and photos, later returningonline to see who had read her notes and writtenback with encouraging words.“It’s an amazing service,” says Harlowe. “I havea new friend in California who’s been where I’vebeen, and she knows it’s a fight. She sent me pinkboxing gloves to hang in my car.”For families separated by geography, CarePagesbrings emotional support right to the bedside.In its first three months, from October toDecember 2008, <strong>Evergreen</strong>’s CarePages had more“It is only now beginning to sink in, all thathas happened this month and all that is tocome. By year’s end,CarePages had 721 members with 1,143 messagesreceived. How do I helpthis patient, since asurgeon’s fees areonly part of thetotal cost of surgery?Project Access makesit easy for us to getinvolved.”– Kelly Clinch, MDCommunity Health LeadershipThe spirit of generosity is alive and well – andgrowing – at <strong>Evergreen</strong>.Through Project Access, doctors can volunteer togive specialty care to low-income and uninsuredpatients. With its commitment to communityhealth, <strong>Evergreen</strong> was the first Eastside hospital tojoin King County Project Access, the program thatfacilitates the donation of physician servicesto families in need.“The thing that really excites us is when we canhelp people get back to work,” says Cherie Green,RN, project coordinator for the program. “Withoutinsurance, they’re not able to access the specialtycare they need and often end up in more acutesituations. Through the generosity of our specialistsproviding free care to them, they’re able to get backon their feet and take care of themselves and theirfamilies again.”Nearly eight percent of <strong>Evergreen</strong>’s service areapopulation is uninsured. Since its inception in lateCherie Green, RN, helps coordinate volunteer surgery forpatients in need.2006, Project Access has helped a total of 307patients, with a 50 percent increase in enrollmentsbetween 2007 and 2008.“We have 74 physicians who pledge to see patientsat no charge in their offices,” says Green. “Ourmost frequent referrals are to orthopedics, generalsurgery, gastrointestinal surgery and hand surgery.”Project Access, which provides volunteer specialtycare, is part of the larger <strong>Healthcare</strong> Access Program,which provides volunteer primary care through the<strong>Healthcare</strong> Access Clinic. All told, participatingproviders from <strong>Evergreen</strong> have donated more than660 hours.5-Star ServiceWhen Kathy Schoenrock pulled a team togetherto design an organization-wide service excellenceinitiative, one thing became instantly clear.“We realized that to give ‘5-star service’ to ourpatients and families, we had to start with giving‘5-star service’ to each other as colleagues,”says Schoenrock, <strong>Evergreen</strong>’s director ofquality management.To begin, the Service Excellence SteeringCommittee created an initiative to promote fourkey service standards: Collaborate, Acknowledge,Respond and Exceed Expectations. Servicechampions were identified in each department,and in January, a kickoff event introduced the newinitiative to more than 1,000 staff, physicians andvolunteers. “The ‘5-star’ recognition program hasbeen one of the most exciting things we’ve done toget everyone involved,” Schoenrock says.In organization-wide trainings from February toJune 2008, the famous Pike Place Fish Markettraining video helped promote high-energy, high-quality teamwork – and fun. “Everyone really‘got it’ that a positive attitude and connectingpersonally with your customers is the foundationfor promoting ‘5-star service,’” Schoenrock says.Another success? Recruiting 30 patient and familyadvisors to work on clinical teams in 2008. “Whata difference it makes when you have patients andfamilies sitting at the table with you as you developprograms and services,” she says.The first-ever “Service ExcellenceQuarterly Team Award” in 2008was won by <strong>Evergreen</strong>’s MedSurgNursing.
