<strong>MEMORANDUM</strong> <strong>OF</strong> <strong>UNDERSTANDING</strong><strong>BETWEEN</strong><strong>THE</strong> DEPARTME\T <strong>OF</strong> VETERANS AFFAIRS ANDMIKE O’CALLAGHAN FEDERAL HOSPITALGeneral InformationThe Memorandum of Understanding (MOU) is an agreement between the VA Southern Nevada HealthcareSystem (VASNHS) and 99th Medical Group, Nellis Air Force Base (AF) regarding Intensive OutpatientProgram for Pathological Gambling (IOPP) services for adult (>18 years of age) active duty, dependent, andTRICARE enrollees. Dependents and TRICARE enrollees must have prior approval from TRICARE prior toreferral to IOPP.The VASNHS will provide for up to 15 consults/procedures per week to include the following:Initial Assessment and Evaluations (single episode, done by staff and MD),Additional Services as outlined below, are conducted at the VASNHS North Clinic at916 W. Owens Aye, Las Vegas NV 89106.Based on the outcome of the Initial Assessment the following services will provided:Individual therapy (staff or MD) at least 3 months,Medication management and monitoring (if appropriate) (MD),Aftercare Group therapy (staff) weekly every Tuesday for at least 6 months,Two Intensive OPT Psycho educational Modules (by staff and/or MD) (twice a week for 6 weeks),Crisis intervention (by staff and/or MD),Preparation of Reports (by staff and MD),BriefFamily or Marital counseling (by staff and/or MD.AF/Life Skills will provide a mechanism to generate consults to VASNHS Mental Health Clinic support staffwith information as outlined in this MOU. All patients referred will have completed a basic physicalexamination to include laboratory studies: CBC, UA, Thyroid profile and Chem 21. EKG is required for allpatients over age 35.As a result ofthe VASNHS completely funding, staffing, and equipping the North Clinic, the reimbursementmethodology will utilize the national VA/DoD MOO on reimbursement for all services rendered by theVASNHS North Clinic Mental Health Clinics to the AF. The reimbursement MOU specifies the use ofapplying a discount to the TRICARE fee schedule (CMAC less 10%), for professional and technical fees.Consult and Procedure RequestThe AF patient is seen in North Clinic/Mental Health clinic as a result of consult from the AF LifeSkills/Mental Health provider for procedures as described below. VASNHS Mental Health provider ordersprocedure and utilizes VistA/CPRS (Computerized Patient Record System) to document services provided toinclude any all progress notes. Results are faxed as below.I. AF provider generates a consult for Initial Assessment. Based on the initial assessment the VASNHSMental Health provider will order services in accordance with physiological needs from the servicesdelineated above utilizing the following procedural codes:a. 90801 - Initial Assessment and Evaluations (single episode, done by staff ~ MD)


. 90804/90805/90806 Individual therapy (staff or MD) at least 3 monthsc. 90805/90862 Medication management and monitoring (if appropriate)d. 90853 - Aftercare Group therapy (staff) weekly every Tuesday for at least 6 monthse. 90853 - Intensive OPT Psycho educational Modules (twice a week for 6 weeks)f. 90804/90805/90806 - Crisis intervention (by staff and/or MD)g. 90889 - Preparation of Reports (by staff and MD)h. 90804/90805/90806 - Brief Family or Marital counseling2, AF provider completes referral request and faxes referral and results of the physical examination,laboratory testing and EKG (if required) with original referral to Mental Health Clinic to the attention of“Lead Clerk” at636-3079. Referral request will contain the following demographic data and clinicalinformation needed to load patient into CPRS:a. Patient’s name, address and phone numberb. Patient’s social security number (Include sponsor’s social security number if dependent)c. Patient’s branch of service and unit assignedd. Patient’s date of birthe. Provider name and telephone numberf. Reason for consultationg. Desired time frame for consultation3. Pre-Assessment to be completed by the referral site to include the following:a. Indications for consultationb. Allergiesc. Substance abuse/Addictions Historyd. Current psychiatric issuese. Mental Status Exam1’. Suicide Risk Assessmentg. Past psychiatric historyh. Legal issues (convictions, pending)i. Medical issuesj. Medications other then listed this VA’s medical record (current community prescriptions andOTC medications)k. Current or recent pain issues1. Functional Assessment (Social (residence, income/month, support system)m. Assessmentn. Diagnostic Impressionso. Treatment Planp. Patient Educationq. Addiction Severity Index, if indicated4. VASNHS Lead Clerk will ensure the information is given and inputted by VASNHS Enrollment Section intoVistA as a non-veteran sharing agreement patient. After the data is entered into VistA, VASNHS Lead Clerkwill give the consult to the VASNHS Mental Health Addictions Provider.5. VASNHS Mental Health Addictions Provider enters the consult into CPRS based on information providedby AF.6. VASNHS Mental Health Clerks will contact the patient to schedule appointment.7. VASNHS Addictions Counselor will maintain a monthly listing of patients referred and treatment rendered.


8. VASNHS Mental Health Addictions Providers will document all treatment rendered into CPRS.9. VASNHS Mental Health Addictions Provider will prescribe medications, if necessary, appropriate to thepatient’s condition. Prescriptions will be filled at M<strong>OF</strong>H pharmacy. At the completion ofthe treatment,patients will be referred to the AF/Life Skills section for medication follow-up.10, VASNHS Addictions Counselor will send Patient’s treatment summary at completion of treatment of clinicvisits to AF/Life Skills referral office at M<strong>OF</strong>H within 5 working days after the beginning ofeach month.11. Monthly listing of patients and services rendered/workload will be faxed/hand carried to VASNHS ChiefAdministrative Officer at M<strong>OF</strong>H monthly within 5 working days after the beginning of each month.ink:i_/I I7 ~..Cynthia Jones, Lt.
