But neck, shoulder, wrist injury still<br />

a big problem among sonographers<br />

By now, it is well known that practicing<br />

sonography can be hazardous <strong>to</strong> your health<br />

<strong>with</strong>out the proper training and equipment.<br />

Since the 1980s, sonographers have been<br />

reporting work-related injuries <strong>to</strong> their necks,<br />

wrists, shoulders and arms in alarming<br />

numbers. Studies continue <strong>to</strong> show that as<br />

many as 90 percent of sonographers are<br />

<strong>scanning</strong> in pain and discomfort – and as<br />

many as 20 percent of them (one in five) have<br />

musculoskeletal (<strong>MSK</strong>) injuries severe enough<br />

<strong>to</strong> end their careers.<br />

Thanks <strong>to</strong> people like SDMS founder Joan<br />

Baker, MSR, RDMS, RDCS, FSDMS, and others,<br />

the industry is paying attention <strong>to</strong> the <strong>ergonomic</strong><br />

<strong>issues</strong> that can result in <strong>scanning</strong> injuries and<br />

addressing them.<br />

A number of companies have introduced <strong>scanning</strong><br />

<strong>beds</strong> and chairs designed <strong>to</strong> make access<br />

<strong>to</strong> patients easier. Manufacturers of ultrasound<br />

equipment have redesigned workstations so<br />

that sonographers can adjust their posture and<br />

improve their <strong>scanning</strong> health. Manufacturers<br />

also have redesigned transducers so that they<br />

are easier <strong>to</strong> grip and developed au<strong>to</strong>mated<br />

pro<strong>to</strong>cols <strong>to</strong> reduce key strokes and thus lessen<br />

the chance of repetitive motion injuries.<br />

“Injuries have become a major part of our world in<br />

sonography,” says Dave Fortney, BA, RDMS, RVT, RT<br />

(R), an ultrasound marketing manager for Siemens<br />

Healthcare. Fortunately, he says, “Over the last five<br />

years, we and other vendors have taken <strong>ergonomic</strong><br />

<strong>issues</strong> seriously and adopted our products <strong>to</strong> fit<br />

the body better and reduce work-related injuries <strong>to</strong><br />

sonographers.”<br />

Many companies that design and sell <strong>ergonomic</strong><br />

ultrasound equipment were started by sonographers<br />

who were injured on the job themselves. For example,<br />

Jason Stasney, RVT, RDCS <strong>with</strong> Heritage Medical<br />

Products, Inc, started his company in the early 1990s<br />

after he was injured on the job and saw many of his<br />

colleagues suffering from similar work-related <strong>issues</strong>.<br />

Ergonomically designed ultrasound equipment<br />

Beds are now being designed entirely from the<br />

sonographer’s point of view. Cus<strong>to</strong>mers can choose<br />

from different models specifically designed for<br />

echocardiography, general sonography, Ob/Gyn,<br />

vascular and even multispecialty.<br />

Steps are also being taken <strong>to</strong> help reduce repetitive<br />

stress <strong>issues</strong>. Traditional probes are shaped like a<br />

pencil, but some new ones are shaped like an apple<br />

allowing the sonographer <strong>to</strong> wrap their whole hand<br />

around it. One recommendation for reducing repetitive<br />

stress injuries is <strong>to</strong> change positions and flex your<br />

hand periodically, says Christine Ingle, RDMS, RDCS<br />

<strong>with</strong> Esaote North America Inc.<br />

Other transducers now available allow imaging in<br />

two planes <strong>with</strong>out moving the transducer. “You no<br />

longer need <strong>to</strong> rotate the transducer <strong>to</strong> see the second<br />

plane,” says Jim Brown, the senior direc<strong>to</strong>r of clinical<br />

and technical marketing <strong>with</strong> Philips Healthcare says.<br />

“You don’t have <strong>to</strong> twist the wrist.” As a result, he says,<br />

that the sonographer can gather twice as much clinical<br />

data in the same amount of time, which speeds<br />

workflow while reducing the potential for repetitive<br />

stress injuries.<br />

Vendors of ultrasound equipment have also made<br />

changes in software such as voice command aimed<br />

at injury prevention. The software design changes<br />

reduce scan time and au<strong>to</strong>mate measurements<br />

thereby eliminating keystrokes, says Dave Fortney, of<br />

Siemens Healthcare. “Many of the features that the<br />

sonographer would have <strong>to</strong> do while they scan such as<br />

labeling of images and turning major modes like color<br />

Doppler on and off have been au<strong>to</strong>mated. Systems<br />

are now able <strong>to</strong> do the majority of the work for the<br />

sonographers. This significantly decreases the number<br />

of keystrokes a user must do.”<br />

Global perspective needed<br />

Susan Murphey, BS, RDMS, RDCS, CECD, like so many<br />

sonographers, became involved in <strong>ergonomic</strong>s as<br />

the result of a work-related injury. Work-related injury<br />

is not just related <strong>to</strong> <strong>ergonomic</strong>s, but also workflow<br />

and supply chain concerns. For example, she says,<br />

if patients come <strong>to</strong> the sonography department on<br />

hospital <strong>beds</strong> versus stretchers causing sonographer<br />

injuries from transferring patients or straining <strong>to</strong><br />

scan them, it is a supply chain issue - where the<br />

process breakdown occurs outside of the ultrasound<br />

department. The problem needs <strong>to</strong> be solved <strong>to</strong>gether<br />

<strong>with</strong> the outside department (‘the supplier’) rather<br />

than independently.<br />

Also, Murphey says, if there’s one thing she’s learned<br />

in her more than ten years in health-care <strong>ergonomic</strong>s,<br />

it’s that it’s crucial <strong>to</strong> involve the frontline worker in<br />

finding solutions <strong>to</strong> problems. “You can provide better<br />

equipment <strong>to</strong> sonographers, but if you don’t engage<br />

them in the solution, you may not fully understand<br />

the root cause of their concerns. If you simply instruct<br />

them <strong>to</strong> change how they do their work, they’ll resort<br />

<strong>to</strong> their old ways as soon as you walk out the door, in<br />

spite of the potential for <strong>ergonomic</strong> benefit,” she says.<br />

“It’s amazing how much more creative, practical and<br />

sustainable the solutions will be when you allow them<br />

<strong>to</strong> problem solve and give them the <strong>to</strong>ols <strong>to</strong> be able <strong>to</strong><br />

do so.”<br />

Joan Baker says that despite the improvements <strong>to</strong> the<br />

equipment and all the attention that the high rate of<br />

injuries among sonographers has gotten over the last<br />

three decades from manufacturers and regula<strong>to</strong>rs,<br />

the numbers are no better. Baker expects things<br />

may only get worse, especially if the economy does<br />

not improve. With hospitals and other health-care<br />

facilities facing decreases in reimbursements, they see<br />

<strong>ergonomic</strong> equipment as discretionary spending and<br />

are not likely <strong>to</strong> include it in their budgets. Employers<br />

are pushing sonographers <strong>to</strong> do more scans in less<br />

time. Baker is concerned that sonographers may be<br />

concerned about their jobs and afraid <strong>to</strong> speak up for<br />

fear their employers will just find someone else who<br />

will not complain. But, she says, sonographers can’t<br />

give up their fight for their safety because if they do,<br />

sonographers who come after them will face the same<br />

or worse <strong>MSK</strong> <strong>issues</strong>.<br />

Diagnostic Medical Sonography at the Federal level. The<br />

SDMS PAC is an essential component in the Society’s efforts<br />

<strong>to</strong> represent the interests of the membership. It enables the<br />

SDMS <strong>to</strong> send a strong, collective, bipartisan message <strong>to</strong><br />

members of Congress that our sonographer members are<br />

committed <strong>to</strong> support those candidates in Congress that<br />

are engaged and supportive of our efforts <strong>to</strong> further the<br />

interests and policy positions of the SDMS.<br />

Donations from the SDMS membership-at-large <strong>to</strong> the<br />

SDMS PAC help support SDMS’ efforts such as the CARE<br />

bill. The CARE bill has been re-introduced in the House of<br />

so we created a series of articles, each featuring<br />

Around the Globe<br />

A child in Saudi Arabia was named “Nancy” after SDMS<br />

member, Nancy Duggan. After having seven boys, the<br />

parents were so excited <strong>to</strong> finally have a daughter that<br />

they named the girl after the sonographer who gave<br />

them the news!<br />

Nancy has been a sonographer since 1993, and<br />

has lived in the Middle East since 1995, where she<br />

expanded her skills from ob/gyn and abdominal<br />

sonography <strong>to</strong> include vascular sonography and<br />

echocardiography as well. Since 2009, she has also<br />

been developing a physics program for the first year<br />

residents at her hospital. Additionally, in 2011, she<br />

began teaching her vascular surgeons the finer points<br />

of sonography.<br />

Nancy first became interested in ultrasound when she<br />

was in a radiologic technology program in Brunswick,<br />

Canada. She was drawn <strong>to</strong> the direct patient<br />

interaction, and what more exciting place is there <strong>to</strong><br />

interact <strong>with</strong> patients than the Middle East? Nancy<br />

mentioned she particularly enjoys talking <strong>to</strong> the older<br />

generation who are often full of wonderful s<strong>to</strong>ries of<br />

Bedouin lifestyle.<br />

When asked about advancements seen in her career,<br />

Nancy pointed <strong>to</strong> elas<strong>to</strong>graphy and volumetry being<br />

powerful diagnostic <strong>to</strong>ols when used in conjunction<br />

<strong>with</strong> B-mode ultrasound. She is also excited <strong>to</strong> see<br />

developments in the areas of wireless transducers and<br />

lighter-weight ultrasound machines and their potential<br />

<strong>to</strong> improve sonography <strong>ergonomic</strong>s, thus protecting<br />

sonographers from painful injuries.<br />

Nancy describes her SDMS membership as a “lifeline”<br />

while overseas. “The [JDMS] articles keep me up <strong>to</strong><br />

date on a lot of the advancements going on back<br />

home,” Nancy says. She also takes advantages of the<br />

SDMS CME opportunities.<br />

What inspired you <strong>to</strong> make a career of<br />

sonography?<br />

My academic background was in veterinary<br />

virology. In the mid- <strong>to</strong> late-1970s, I directed a<br />

labora<strong>to</strong>ry that was involved <strong>with</strong> isolation of a<br />

canine gastrointestinal virus that had reached<br />

epidemic levels in the United States. Our veterinary<br />

labora<strong>to</strong>ry did not have adequate facilities <strong>to</strong><br />

complete the full range of isolation procedures and<br />

I turned <strong>to</strong> the animal research facilities at Penn<br />

State University College of Medicine in Hershey,<br />

Pa., for help. During my visit there, I met Dr. Gary<br />

Nicholas, who was then chief of vascular surgery<br />

at Penn State’s Mil<strong>to</strong>n S. Hershey Medical Center.<br />

Dr. Nicholas had established a vascular research<br />

labora<strong>to</strong>ry and many of their investigations used<br />

animal models. Dr. Nicholas introduced me <strong>to</strong><br />

noninvasive vascular physiologic test procedures<br />

for evaluation of the extremity arterial and venous<br />

systems and the extracranial carotid arteries. I was<br />

hooked! In 1980, I joined the Hershey vascular<br />

lab team as a senior research technologist. In<br />

1981, we convinced the College of Medicine that<br />

its vascular labora<strong>to</strong>ry should expand in<strong>to</strong> the<br />

clinical arena and we began offering vascular<br />

testing <strong>to</strong> patients. Dr. Nicholas entered private<br />

practice in 1983 and Dr. Brian Thiele, an Australian<br />

vascular surgeon working on the development of<br />

duplex <strong>scanning</strong> at the University of Washing<strong>to</strong>n<br />

<strong>with</strong> Dr. Gene Strandness, joined our Hershey<br />

team. Under Dr. Thiele’s direction, we continued<br />

research in vascular surgery and technology and<br />

expanded the vascular labora<strong>to</strong>ry in<strong>to</strong> a fullservice<br />

facility offering testing in cerebrovascular,<br />

peripheral arterial and venous, organ transplant,<br />

and abdominal vascular procedures. In the late<br />

1980s, we opened the vascular labora<strong>to</strong>ry as an<br />

international center for physician and sonographer<br />

training in noninvasive diagnostic techniques.<br />

Did you have a memorable role model/ teacher?<br />

Who was it?<br />

This question is very difficult for me <strong>to</strong> answer<br />

because over the course of my career, I have had<br />

many sonographer and physician men<strong>to</strong>rs. My very<br />

first men<strong>to</strong>r was Dr. Nicholas who introduced me <strong>to</strong><br />

vascular technology and the important role it could<br />

play in identification and localization of vascular<br />

disease. By looking at vascular disorders through<br />

Dr. Thiele’s eyes, I experienced the excitement and<br />

challenge of developing technology and diagnostic<br />

pro<strong>to</strong>cols and was humbled and honored <strong>to</strong> work<br />

as his team mate during his years at Hershey. He<br />

was dedicated <strong>to</strong> the advancement of vascular<br />

technology and the role of the sonographer as a<br />

key member in the healthcare team. Dr. Ann Marie<br />

Kupinski, who served as technical direc<strong>to</strong>r of the<br />

vascular labora<strong>to</strong>ry at Albany Medical College for<br />

many years; Dr. John Pelleri<strong>to</strong>, who is direc<strong>to</strong>r of<br />

the vascular labora<strong>to</strong>ry and chief of the Division of<br />

Ultrasound, CT and MRI at North Shore University<br />

Hospital in New York; Dr. Joe Polak, direc<strong>to</strong>r of the<br />

Ultrasound Reading Center at Tufts-New England<br />

Medical Center; and Dr. Ed Grant, chair of the<br />

Radiology Department at University of Southern<br />

California School of Medicine, have inspired me<br />

over the years and have always been willing <strong>to</strong><br />

share their knowledge and experience. Individually,<br />

they have made major contributions <strong>to</strong> the field of<br />

vascular diagnostics through research, lectures<br />

and publications. I am proud <strong>to</strong> call them men<strong>to</strong>rs,<br />

colleagues and friends.<br />

What was the most significant advancement in<br />

the technology that you directly experienced?<br />

I began my career in vascular technology a<br />

few years before the development of duplex


ultrasound. In those years, we used indirect<br />

physiologic test modalities and continuous wave<br />

Doppler ultrasound <strong>to</strong> detect obstruction in the<br />

extremity arteries and veins and in the extracranial<br />

carotid arteries. I remember my excitement at<br />

being able <strong>to</strong> actually see atherosclerotic plaque in<br />

a carotid artery and thrombus <strong>with</strong>in the deep and<br />

superficial veins and <strong>to</strong> explore the variety of flow<br />

patterns associated <strong>with</strong> disease using spectral<br />

Doppler. With improved image resolution and<br />

the ability <strong>to</strong> penetrate <strong>to</strong> the depth of the renal<br />

arteries, the range of possible vascular diagnostic<br />

procedures expanded dramatically. The Hershey<br />

Vascular Labora<strong>to</strong>ry was among the first <strong>to</strong> explore<br />

the clinical utility of color flow imaging for vascular<br />

evaluations and participated in investigations<br />

of power Doppler, real-time compound imaging<br />

and the use of contrast agents for detection of<br />

abdominal vascular lesions.<br />

Although there has been a wealth of fabulous<br />

technical advances that have benefited the<br />

assessment of vascular disease over the years,<br />

I believe that the single most beneficial has<br />

been the introduction of broad bandwidth, lowfrequency<br />

pulsed Doppler technology. With that<br />

development, investiga<strong>to</strong>rs were able <strong>to</strong> explore<br />

the abdominal and intracranial circulations, areas<br />

of the vasculature that were inaccessible <strong>with</strong> high<br />

frequency ultrasound probes.<br />

Were there any memorable developments,<br />

cases or events that changed the way you<br />

The most rewarding and memorable event<br />

that changed the way I practiced sonography<br />

was having the opportunity <strong>to</strong> participate in the<br />

development of the Intersocietal Commission<br />

for the Accreditation of Vascular Labora<strong>to</strong>ries<br />

(ICAVL). In 1989, in response <strong>to</strong> pressure from<br />

CMS and third-party payers <strong>to</strong> curtail vascular<br />

labora<strong>to</strong>ry entrepreneurial activities and <strong>to</strong><br />

enforce mechanisms <strong>to</strong> ensure high quality<br />

vascular testing, I joined Dr. Chris Merritt, Dr.<br />

Eugene Strandness, Dr. Thiele, and Ann Jones<br />

<strong>to</strong> discuss the formation of an organization that<br />

would provide peer review and accreditation of<br />

vascular labora<strong>to</strong>ries. As a result of this initial<br />

meeting, sonographers and physicians from<br />

organizations representing medical disciplines<br />

involved in noninvasive vascular testing formed<br />

the first ICAVL board of direc<strong>to</strong>rs and wrote the<br />

Essentials and Standards for Vascular Labora<strong>to</strong>ry<br />

Accreditation. The organization was incorporated in<br />

1990 and, at present, many states have mandated<br />

reimbursement directives, accreditation of the<br />

vascular labora<strong>to</strong>ry, or the presence of a certified<br />

vascular sonographer during performance of<br />

vascular test procedures.<br />

The development of duplex ultrasound techniques<br />

for evaluation of the abdominal vessels also<br />

impacted the way I practice sonography. We<br />

initiated our research in<strong>to</strong> this procedure in the<br />

Vascular Studies section at Hershey in the mid-<br />

1980s, prior <strong>to</strong> the introduction of color flow<br />

imaging. Bob Martin, a vascular sonographer who<br />

was at that time working <strong>with</strong> Dr. Strandness at the<br />

University of Washing<strong>to</strong>n vascular lab, had been<br />

exploring this procedure for several months. I spent<br />

some time <strong>with</strong> Bob learning his techniques and<br />

returned <strong>to</strong> Hershey full of enthusiasm and ready<br />

<strong>to</strong> meet the challenge. We used mechanical sec<strong>to</strong>r<br />

transducers and began our investigations initially<br />

<strong>with</strong> studies on renal transplants. This research<br />

led <strong>to</strong> perfection of Doppler spectral pattern<br />

recognition for differentiation of the various causes<br />

of transplant dysfunction. Over the next several<br />

years, we gained expertise in native renal artery<br />

duplex evaluations, and I continue <strong>to</strong> promote this<br />

evaluation as the initial screening procedure <strong>to</strong> rule<br />

out renovascular disease.<br />

What changes do you predict in the immediate<br />

future?<br />

Now that 3 and 4-dimensional ultrasound are a<br />

reality, I expect <strong>to</strong> see much of the research and<br />

technology concentrated on miniaturization of<br />

ultrasound systems and transducers. Looking<br />

in<strong>to</strong> the future, I predict that physicians and<br />

sonographers will use small, hand-held imagers<br />

in the lab and at remote sites for not only routine<br />

examinations but also for emergency evaluations.<br />

These units most likely will be found on trauma<br />

helicopters and in use by police and fire rescue<br />

teams. Imaging and Doppler spectral data may be<br />

sent <strong>to</strong> central reading stations for interpretation.<br />

In the future, I see Doppler crystals housed <strong>with</strong>in<br />

body wraps that could be placed around a limb so<br />

that the crystals lie over an arterial bypass graft or<br />

native artery. Doppler spectral waveforms and/or<br />

images would be transmitted <strong>to</strong> the reading center<br />

in much the same way as pacemaker moni<strong>to</strong>rs<br />

work <strong>to</strong>day. Patients could be instructed on home


use of these wraps for moni<strong>to</strong>ring function of grafts<br />

or medical personnel could use them for collection<br />

of blood flow data in an emergency situation in the<br />

field.<br />

What advice would you give <strong>to</strong> students/ future<br />

sonographers?<br />

Sonographers who are entering the field of<br />

medical ultrasound have potential for exciting,<br />

challenging careers. With exceptional imaging<br />

<strong>to</strong>ols at your command you can offer patients<br />

the best of noninvasive diagnostic procedures.<br />

Above all, a sonographer must be certain that<br />

they are providing the patient the highest quality<br />

examination possible. Continuing education<br />

is a window <strong>to</strong> the latest in technology and<br />

credentialing is confirmation that the sonographer<br />

has met the minimum level of competence required<br />

<strong>to</strong> perform a high quality examination. Study, work<br />

hard, be the very best that you can be. Listen and<br />

learn from others. Share your knowledge because<br />

you are stronger as a member of a team than as<br />

an individual. One of the greatest gifts that you<br />

have been given is your ability <strong>to</strong> learn and one<br />

of the greatest joys you will experience is sharing<br />

what you have learned <strong>with</strong> others.<br />

What is your favorite part of your job?<br />

What I enjoy most about my current job is having<br />

the opportunity <strong>to</strong> share what I have learned <strong>with</strong><br />

others practicing medical sonography. I have<br />

been blessed in so many ways over the course of<br />

my career. I have served as vice president of the<br />

SDMS, as an At-Large Direc<strong>to</strong>r of the SDMS, as<br />

president of the Society for Vascular Ultrasound,<br />

on the Board of Direc<strong>to</strong>rs of the ICAVL, and the<br />

Board of Governors of the American Institute of<br />

Ultrasound in Medicine (AIUM). Presently, I enjoy<br />

providing international education and consulting<br />

in vascular diagnostics. Through past and present<br />

experiences, I have had the opportunity <strong>to</strong> interact<br />

and make friends <strong>with</strong> so many dedicated, talented<br />

sonographers and physicians. I love seeing<br />

sonographers and physicians working <strong>to</strong>gether<br />

as a team <strong>to</strong> accomplish a common goal; sharing<br />

experiences and knowledge; and continually<br />

promoting professionalism, standardization and<br />

excellence in medical sonography. These are the<br />

fundamentals upon which our profession was<br />

established and they are embodied in the mission<br />

statement of Vascular Diagnostic Educational<br />

Services, the company that I now own and direct.
