<strong>Aircraft</strong> Eng<strong>in</strong>eers Internati<strong>on</strong>al<str<strong>on</strong>g>Shame</str<strong>on</strong>g> <strong>on</strong> <strong>EU</strong> <strong>as</strong> <strong>Vietnam</strong> Leads <strong>the</strong> Way In Aviati<strong>on</strong> SafetyDespite <strong>the</strong> c<strong>on</strong>stant flow of statements from <strong>the</strong> <strong>EU</strong> and EASA that Europe h<strong>as</strong> <strong>the</strong> highest aviati<strong>on</strong>standards <strong>in</strong> <strong>the</strong> world; it is <strong>in</strong> fact <strong>the</strong> civil aviati<strong>on</strong> authority of <strong>Vietnam</strong> that h<strong>as</strong> shown Europe howto behave.Jetstar Pacific, an Asian low cost airl<strong>in</strong>e with Qant<strong>as</strong> <strong>as</strong> a major shareholder, h<strong>as</strong> been mak<strong>in</strong>gheadl<strong>in</strong>es for <strong>the</strong> wr<strong>on</strong>g re<strong>as</strong><strong>on</strong>s. Whistleblowers made serious claims about ma<strong>in</strong>tenance standardswhich of course have been strenuously denied by management. Now however a report 1 produced by<strong>the</strong> <strong>Vietnam</strong>ese Civil Aviati<strong>on</strong> Authority b<strong>as</strong>ed up<strong>on</strong> <strong>the</strong>ir own <strong>in</strong>vestigati<strong>on</strong> h<strong>as</strong> c<strong>on</strong>firmed multiplelapses <strong>in</strong> <strong>safety</strong> and standards due to a ''very poor and <strong>in</strong>effective'' culture of <strong>safety</strong> ma<strong>in</strong>tenance.The report menti<strong>on</strong>s <strong>the</strong> follow<strong>in</strong>g lapses:i. The implementati<strong>on</strong> of all ma<strong>in</strong>tenance activities <strong>in</strong> reality does not comply with <strong>the</strong>descripti<strong>on</strong> <strong>in</strong> MMOE (company procedures)ii.iii.iv.Jetstar Pacific did not implement <strong>the</strong> legally required plan regard<strong>in</strong>g manpower, equipment,tool<strong>in</strong>g and facilitiesJetstar Pacific w<strong>as</strong> not capable of fully provid<strong>in</strong>g facilities and workshops <strong>in</strong> accordance with<strong>the</strong> MMOE documentJetstar Pacific h<strong>as</strong> authorised Flight crew to certify A320 aircraft <strong>in</strong>to service while <strong>the</strong>y werenot tra<strong>in</strong>ed and <strong>the</strong>ir certificate for <strong>the</strong> job w<strong>as</strong> not recorded properlyv. Jetstar Pacific Technical Quality Assurance system operated very poorly and <strong>in</strong>effectivevi.Flight crew members and technical staffs deliberately did not enter defect repair <strong>in</strong>to techlogThere were also a number of ma<strong>in</strong>tenance management violati<strong>on</strong> issues such <strong>as</strong>:a) Failure to implement ma<strong>in</strong>tenance activities <strong>in</strong>correctly <strong>as</strong> required by <strong>the</strong> AMM, SRM.Technical Quality Assurance Manager <strong>on</strong>ce authorised ma<strong>in</strong>tenance schedule amendmentwhile <strong>the</strong> t<strong>as</strong>k <strong>in</strong>volved must not be postp<strong>on</strong>edb) anti-ic<strong>in</strong>g pipe system h<strong>as</strong> been brought out to repair (weld<strong>in</strong>g t<strong>as</strong>k) at an unapproved agentc) Authorised CAT A mechanics to be able to certify defects with<strong>in</strong> MEL/CDL scope without anytra<strong>in</strong><strong>in</strong>g recordd) Outsourc<strong>in</strong>g agents were not audited promptly accord<strong>in</strong>g to Ma<strong>in</strong>tenance Standard andMMOE requirement1 Civil Aviati<strong>on</strong> Authority of <strong>Vietnam</strong> Report <strong>on</strong> Jetstar Pacific January 20101


<strong>Aircraft</strong> Eng<strong>in</strong>eers Internati<strong>on</strong>alwhilst EC 2042/2003 PART 145 states that:145.A.30 Pers<strong>on</strong>nel requirements(a) The organisati<strong>on</strong> shall appo<strong>in</strong>t an accountable manager who h<strong>as</strong> corporate authority for ensur<strong>in</strong>gthat all ma<strong>in</strong>tenance required by <strong>the</strong> customer can be f<strong>in</strong>anced and carried out to <strong>the</strong> standardrequired by this Part. The accountable manager shall:1. ensure that all necessary resources are available to accomplish ma<strong>in</strong>tenance and shall <strong>in</strong> <strong>the</strong> c<strong>as</strong>eof aircraft l<strong>in</strong>e ma<strong>in</strong>tenance, have appropriate aircraft type rated certify<strong>in</strong>g staff qualified <strong>as</strong> categoryB1 and B2 <strong>in</strong> accordance with Part-66 and 145.A.35.Yet <strong>the</strong> follow<strong>in</strong>g items are openly <strong>on</strong> <strong>the</strong> agenda 5 :a) Work<strong>in</strong>g around <strong>the</strong> EASA restricti<strong>on</strong>s <strong>on</strong> c<strong>on</strong>tract-to-permanent staff ratiosb) <strong>the</strong> ris<strong>in</strong>g trend to delay ma<strong>in</strong>tenance until <strong>the</strong> aircraft h<strong>as</strong> returned to <strong>the</strong> home hub areall add<strong>in</strong>g up to a decre<strong>as</strong>e <strong>in</strong> <strong>the</strong> l<strong>in</strong>e ma<strong>in</strong>tenance requirement. L<strong>in</strong>e stati<strong>on</strong>s are earlyc<strong>as</strong>ualties <strong>in</strong> <strong>the</strong> fight to cut costsSo just like <strong>the</strong> situati<strong>on</strong> <strong>in</strong> <strong>Vietnam</strong> senior aviati<strong>on</strong> management are c<strong>on</strong>stantly plac<strong>in</strong>g profit before<strong>safety</strong> but unlike <strong>Vietnam</strong>, here <strong>in</strong> Europe <strong>the</strong> 3 m<strong>on</strong>key’s pr<strong>in</strong>cipal applies with regulators see<strong>in</strong>g noevil, hear<strong>in</strong>g no evil and speak<strong>in</strong>g no evil. This approach can <strong>on</strong>ly lead to more preventable accidents.Yet even here <strong>the</strong> regulators are just unable to learn from previous mistakes. The Helios accidentreport w<strong>as</strong> very specific <strong>in</strong> its <strong>as</strong>sessment of c<strong>on</strong>tributory factors relat<strong>in</strong>g to <strong>the</strong> tragedy criticis<strong>in</strong>gregulators for “<strong>in</strong>effectiveness of <strong>in</strong>ternati<strong>on</strong>al aviati<strong>on</strong> authorities to enforce implementati<strong>on</strong> ofcorrective acti<strong>on</strong> plans after relevant audits”. This resulted <strong>in</strong> <strong>the</strong> follow<strong>in</strong>g <strong>safety</strong> recommendati<strong>on</strong>:“EASA/JAA and ICAO implement a means to record <strong>in</strong>ternati<strong>on</strong>al <strong>safety</strong> audits of <strong>the</strong> States’ CivilAviati<strong>on</strong> Authorities, which ensures that <strong>the</strong> f<strong>in</strong>d<strong>in</strong>gs can be tracked <strong>in</strong> depth, acti<strong>on</strong> plans aredeveloped and implemented <strong>in</strong> shortest possible time; and impose <strong>the</strong> necessary pressurewhen <strong>the</strong>y become aware that <strong>in</strong>ternati<strong>on</strong>al obligati<strong>on</strong>s and standards are not be<strong>in</strong>g met by <strong>the</strong>Authorities”. 6The <strong>safety</strong> recommendati<strong>on</strong> h<strong>as</strong> all but been ignored with no significant progress be<strong>in</strong>g made. The<strong>safety</strong> culture of airl<strong>in</strong>es and <strong>the</strong>ir adherence to <strong>the</strong> regulati<strong>on</strong>s rema<strong>in</strong>s completely hidden.
