Contact in<strong>for</strong>mation including: contact name, company name, mailing address, phone <strong>and</strong> faxnumbers <strong>and</strong> a copy of the NJ Business Registration Certificate (issued by Treasury Dept). Ifapplicable, please include documentation if you have been certified by the State of New Jerseyas a Minority/Woman-owned or Disadvantaged Business Enterprise.2. Proposed scope of work3. Brief resumes <strong>for</strong> key personnel4. List of similar project experience5. Schedule <strong>for</strong> project completion6. Cost plus fixed fee <strong>proposal</strong>, broken down by employee titles <strong>and</strong> associated hourly costs. Anitemized cost schedule <strong>for</strong> out-of-pocket expenses must also be included.
