This includes establishing evidencebasedmeasurement systems, and patient and GP informed consent processes.The unit also collaborates with the Ministry of Health Services, other health authorities and nongovernmentalorganizations to evaluate joint initiatives such as:• The iConnect Health Network• The Practice Support Program• Provincial Stroke CareThe introduction of a new electronic Shared Care Plan (now in the testing stage) will further improve theavailability and quality of information for program evaluation and knowledge management (e.g. outcomesorientedDiabetes, CHF, and hypertension reports).… continued on reverse →PC.04.21.09
