The supporting documents are:1) Appeal form for company (original available at HAAD CS.).2) Violation details and calculation as provided by the insurance company (violated members details, violation period & violation fineamount)3) Trade license4) The list of the company’s employees as provided by the Ministry of Labor5) The amount of AED 2000 should be paid and it’s non-refundable even if the violation amount was waived.6) If any listed employee escaped or his visa was cancelled a copy of the related circular or labor card cancelation proof should beattached.Notes:• The above applies only in case of appealing for member’s violation or for the full list of employees.• If the Names provided in both forms (from the Insurance Company & the Ministry of Labor/ point 1 &3) are not matching a copyof the insurance card for non-violator members (insured under HAAD scheme) should be providedFor further details please refer to:• HealthCare Insurer Policy Manual / CHAPTER VII - HAAD REGULATION – INSPECTIONS, COMPLAINTS, APPEALS AND SANCTIONS;and• HealthCare Regulators Policy Manual / CHAPTER X – APPEALS – available on www.HAAD.ae.Certificate of Continuity:Certificates of Continuityshould include the following• Name of Insurer• Health Insurance policy number• Effective Date of Health Insurance policy• Expiry Date of Health Insurance policy• Policy Type (Thiqa, enhanced, basic, emergency…)• Name of insured• Nationality of insured• Sex of insured: male / female• Date of birth of insured• Authorized signatory (insurance company)• Insurance company seal• Certificate issuance date• The following text: “This is to certify that the person/persons insured underthe above mentioned health policy, is/are covered as per Law No. 23 of 2005regarding health insurance in the Emirate of Abu Dhabi and its Regulations.
