Is your child eligible?Your child must be:♥ Under the age of 19.♥ Disabled - the disability must meetthe standards of the Social SecurityAdministration.♥ Income Eligible - your family incomemust be no more than 300% of thefederal poverty level for your family size.See the Income Amounts chart.♥ A U.S. citizen.Does your child haveother health insurance?♥ If yes, you must pay the premiumsand keep your child covered for as longas your employer pays at least half ofthe premium cost.♥ If no, but you can get health insurancefrom your employer and if your employerpays at least half of the premium cost,then you must enroll your child in theemployer insurance plan.♥ If your employer does not pay at leasthalf of the insurance premium cost, thenyou are not required to enroll your child.How do you apply foryour child?♥ Complete the Health ServicesApplication and turn it in at your localDepartment of Human Servicesoffice (DHS).♥ To help process your application,provide information requested by theworker.
