Is your child eligible?Your child must be:♥ Under the age <strong>of</strong> 19.♥ Disabled - the disability must meetthe standards <strong>of</strong> the Social SecurityAdministration.♥ Income Eligible - your family incomemust be no more than 300% <strong>of</strong> thefederal poverty level <strong>for</strong> your family size.See the Income Amounts chart.♥ A U.S. citizen.Does your child haveother health insurance?♥ If yes, you must pay the premiumsand keep your child covered <strong>for</strong> as longas your employer pays at least half <strong>of</strong>the premium cost.♥ If no, but you can get health insurancefrom your employer and if your employerpays at least half <strong>of</strong> the premium cost,then you must enroll your child in theemployer insurance plan.♥ If your employer does not pay at leasthalf <strong>of</strong> the insurance premium cost, thenyou are not required to enroll your child.How do you apply <strong>for</strong>your child?♥ Complete the Health ServicesApplication and turn it in at your local<strong>Department</strong> <strong>of</strong> <strong>Human</strong> Services<strong>of</strong>fice (DHS).♥ To help process your application,provide in<strong>for</strong>mation requested by theworker.
