Next steps and important informationWe have provided you with this <strong>brochure</strong> to help you understand the nextsteps in the application process.We have detailed overleaf the steps involved with this process.3


What happens to your application?<strong>Online</strong> <strong>underwriting</strong> and submissionYour Financial Adviser has submitted your application online to <strong>PruProtect</strong>. Thisapplication includes medical questions, occupation details, sport and leisure questions.Furtherinformationrequired?YesWe may require further informationfrom you to enable us to assess yourapplication. We may ask you tocomplete a declaration form that givesus authority to contact your doctorand/or obtain details on your medicalrecords. A tele-underwriter may call youduring a prearranged time slot if weneed to ask you questions about amedical condition you have told usabout. Obtaining information in this wayspeeds up the process and can avoidthe need for a doctor's report.If we do call you to discuss a medical condition we will send you a further confirmationschedule. It will detail your responses to the questions asked by the tele-underwriter.We only require you to send this one back to us if there are any errors or omissions.4


NoWe will write to you and your FinancialAdviser with these details.Based on the information you havesupplied, we may:■ Accept your application■ Accept your application withspecial terms■ Decline, or defer, your applicationYou or your Financial Adviser can acceptour offer, normally by confirming the startdate of your policy. We will then send youyour policy documents, plan schedule andconfirmation schedule.Your confirmation schedule is a record ofthe "<strong>client</strong> detail" information you providedto your Financial Adviser. You will need tocheck it carefully and let us know if thereare any errors or omissions. Failure toadvise us of incorrect or missinginformation may result in non-payment ofa claim.5


Your tele-<strong>underwriting</strong> callOne of our tele-underwriters may call you to discuss a medical conditionthat you have told us you have. Obtaining information from you in this wayspeeds up the process and may avoid the need for a medical report fromyour doctor.In order to ensure confidentiality, we will ask you to confirm your identity.Calls may be monitored or recorded for quality and security purposes andmay be referred to in the event of a claim.Failure to disclose relevant information may result in non paymentof a claim.If you wish to change the time slot when our tele-underwriter will contactyou, please call 0845 601 0072. Lines are open Monday to Friday8.30am – 6.00pm.Alternatively if you have any further queries regarding the <strong>PruProtect</strong> Planplease contact your Financial Adviser.Important informationThe following contains important contractual information whichyou must read.During the meeting with your Financial Adviser you should have beenadvised of your rights and obligations when applying for cover. This willhave included explaining, reading and agreeing to be bound by thestatements on the following pages.Providing information to usFailure to disclose any facts which would be likely to influence<strong>PruProtect</strong>'s assessment or acceptance of this application may leadto the cancellation of the agreements and any claim not being paid.If you have any doubt about whether a fact would be important tous in accepting your application, please provide full details.6


■You should not assume we will obtain a report from your doctor.■ You must notify <strong>PruProtect</strong> of changes in any information provided toyour Financial Adviser such as a change in health, or any answer givenby you to your Financial Adviser between the time you provided itto your Financial Adviser and the start of the Plan.If this application, taken together with any other insurance policies youalready have, is for Life Insurance up to a sum of £500,000, or CriticalIllness/Serious Illness Cover up to £300,000 or Income Protection Cover upto £30,000 pa you need not disclose any genetic test you may have had.You need not disclose the result of any genetic test undertaken in thecontext of research. Genetic test results need only be disclosed where thesum exceeds either £500,000 for Life Insurance £300,000 for CriticalIllness/Serious Illness Cover or Income Protection Cover up to £30,000 paand their use by insurers has been independently approved. You may, ofcourse, disclose any genetic test result which is in your favour.If you either have a family history of, are experiencing symptoms of, or arehaving treatment for, a genetic condition, you must tell us. Furtherinformation is available on request which fully explains this policy anddetails those genetic tests approved for use by insurers.Adviser authorisationYou authorise your Financial Adviser named on the application form to acton your behalf and to send, or submit online, the application to <strong>PruProtect</strong>.He/she does so as your agent and not as an agent of <strong>PruProtect</strong>.This means that you are responsible for the answers submitted on your behalf.You also authorise him/her to amend the information submitted by him/heron your behalf and to agree the commencement date of the Plan with<strong>PruProtect</strong>.You understand he does not have the authority to accept this application onbehalf of <strong>PruProtect</strong>.The Plan will not start until we have assessed and accepted your application,and have been advised of the start date. If you have a birthday whileyour application is being processed, the terms may differ from thoseoriginally quoted.7


In most instances your payments will be as originally quoted. We may offeryou revised terms, but occasionally we may not be able to offer any terms.Terms of agreement1. You understand that the agreements between you and <strong>PruProtect</strong> ("theCompany") for the <strong>PruProtect</strong> Plan will be governed by the terms of thefollowing documents:a) Policy Summaryb) This <strong>brochure</strong>c) The confirmation schedule(s)d) The plan schedulee) Plan Provisions2. You agree that you have had adequate time to review and confirm theaccuracy of the information provided by your Financial Adviser, and thisinformation together with any subsequent information provided by youto the tele-underwriter or <strong>PruProtect</strong>’s Medical Examiner, will form thebasis of an agreement between you and <strong>PruProtect</strong> for the provision ofthe <strong>PruProtect</strong> Plan. <strong>PruProtect</strong> may rely on the accuracy andcompleteness of this information for the purposes of providing the<strong>PruProtect</strong> Plan. You understand that <strong>PruProtect</strong> will prepare a recordknown as the confirmation schedule which will reflect the informationprovided by your Financial Adviser. We will send a copy of theconfirmation schedule to the address we hold for the first life assured sothat it can be checked for accuracy.If we do call you to discuss a medical condition we will send you afurther confirmation schedule. It will detail your responses to thequestions asked by the tele-underwriter. It is equally important thatyou check this document for errors or omissions. You have 14 daysfrom the date of issue of this confirmation schedule to check thisinformation and advise <strong>PruProtect</strong> in writing, on the form supplied,of any errors or omissions, otherwise it will be deemed to be correct.If this confirmation schedule is correct you do not need to do anything.In the unlikely event that you do not receive your confirmation schedule, it isimportant that you notify us by calling 0845 601 0072. Lines are openMonday to Friday 8.30am – 6.00pm. Calls may be monitored or recorded forquality and security purposes.8


Failure to disclose relevant information may result in non payment ofa claim.You understand that if you do notify the Company of any errors or omissionsthe Company will have the right to cancel the <strong>PruProtect</strong> Plan, and in suchevent the agreement will be taken never to have existed.Alternatively, the Company may write to you offering to continue the<strong>PruProtect</strong> Plan, but at amended premium rates or on special conditions. Insuch an event, you will inform <strong>PruProtect</strong> of your acceptance. If you do notaccept them, the Company will cancel the <strong>PruProtect</strong> Plan.Unemployment CoverWhere you have elected to apply for Unemployment Cover, youunderstand that by submitting this application, you are applying for twoseparate contracts. You understand that the Unemployment Cover isprovided by St Andrew’s Insurance plc and that <strong>PruProtect</strong> are acting onlyas their agents. It is noted that <strong>PruProtect</strong> reserve the right to change theprovider of this benefit in the future.You further understand that the information supplied will be forwarded to StAndrew’s Insurance plc and that insurers share information with each otherregarding Unemployment Cover, to prevent fraudulent claims. This systemutilises a register of claims, and a list of participants is available on request.You accept that information supplied on this application and the claim form,together with any other information relating to the claim, will be provided tothe register.General declarationIf you have taken out this plan in conjunction with a mortgage, we may askyou for written consent to seek details of the mortgage from the lender.You are aware that the income benefits you receive could affect the amountof any income support/income based Jobseekers Allowance, should you beeligible for state help.9


Your statutory rights under the Access to MedicalReports Act 1988Many applications are accepted without the need for a report from yourdoctor. Where we do need a medical report, we require your consent. Ifyou wish to see any medical report before it is sent to <strong>PruProtect</strong>, pleaseindicate this when you sign the Access to Medical Reports Act declaration.If we ask you to come for a medical examination, we may need to share theapplication information with another company we have authorised. Theywill make the arrangements for the examination to take place.We may need to send your application and relevant medical reports to ourreinsurers for their opinion or agreement of the terms offered. Or, we mayneed to send them at a later stage for purposes relating to managing thepolicy. You can get details of general reinsurance principles and details ofany company we use to assess your application, from our head office.On occasion the faxing of medical reports may help to ensure a speedierassessment of your Application. <strong>PruProtect</strong> only accepts faxed informationdirect to a fax machine in a secure part of its Customer Services Office.We have a confidentiality policy in place which means we hold your medicalinformation securely and access is limited to authorised individuals whoneed to see it. In the event of a claim, or at any time during the term of yourplan, you or your legal representatives may be asked to provide consent foraccess to your medical records. <strong>PruProtect</strong> reserve the right to cancel thisplan if you or your legal representatives fail to consent to this request.10


Data Protection NoticeWhy should you read this noticeWe think it’s important for all our customers to be made aware of whatinformation <strong>PruProtect</strong> as part of the PruHealth Group* holds about themand to reassure our customers that we comply with the Data Protection Act 1998.How we use your personal information<strong>PruProtect</strong> will use your personal information (including informationprovided about your dependants) to underwrite, administer, profile yourpurchase preference and service your Plan. By taking out a Plan with us,you consent to us using your personal information and sensitive personalinformation (e.g. health information). We will also use your information forstatistical data analysis, management information and fraud prevention purposes.Who we may give personal information toWe may disclose your personal information to other companies in thePruHealth Group, our business associates, agents or service providers forthe purposes above. Your information may be used by service providers in acountry outside the European Economic Area, which may not have thesame standard of data protection as in the UK. We will ensure appropriatesafeguards are in place to protect your information.We will pass your personal information and information about your Plan toany legal or regulatory body if required to do so. We may also use yourinformation or give it to others, for research, statistical purposes or toimprove our services, but we will remove your name and address from thisfirst. We may send copies of correspondence relating to your Plan to yourFinancial Adviser, if you’ve appointed one. We may provide informationabout a claim to them, although no medical information will be providedwithout your consent.When giving us information about another person, you confirm that theyhave appointed you to act on their behalf. This includes providing consentto process the personal information, receive this Data Protection Notice ontheir behalf and unless you decide otherwise, receive marketing information.Your information, and that of others also covered by the Plan, may be givento other parties (for example, other insurance companies) with a view topreventing fraudulent or improper claims.11


Our marketing policy<strong>PruProtect</strong>, PruHealth’s group of companies and our business associates,service providers and agents may use your personal information to informyou of other services and products that may be of interest to you bytelephone, post, email or text.
