Findlay City Schools PreschoolPEERS as Partners ~ Special Needs IntegrationREQUIREMENTS: v Child must be 3 years old prior to March 1, 2013. No exceptions. v Child must be toilet trained prior to the screening. v Child must participate in a play-­‐based screening on Friday, March 22, 2013, at Northview Primary, 133 Lexington Avenue. v Parents must provide transportation to and from school. v If accepted, enrollment papers and deposit must be completed and returned to the Findlay City Schools Welcome Center (TLB Corporate Center, 1219 W Main Cross, Suite 102) by May 1, 2013. v Children that currently attend Findlay City Schools Preschool may apply for the 2013-­‐2014 school year. Currently enrolled students do not need to attend the March screening. FEES: v A $20 deposit is due by May 1, 2013, to secure a spot in the preschool program. Please note that the deposit is non-­‐refundable and will be credited to the supply account at your child’s building. v $210 is due every three months for a total of $630 (in addition to the $20 deposit) **Due at the August parent meeting, December 1, and March 1** v By paying ahead in full for the entire year, you will receive a $30 discount. To qualify, $600 (in addition to the $20 deposit) is due at the Parent Meeting in August. v A $20 LATE FEE will be required for payments received after the 10 th of each month. v Payments may be made by check or on-­‐line at www.findlaycityschools.org through ezpay.
