OVERVIEW OF THE GUIDE SECTION 11. OVERVIEW OF THE COMMUNITY CARE COMMONSTANDARDS GUIDEThis section provides background information about accountability requirements related to thecommunity care programs across jurisdictions (<strong>Common</strong>wealth, State and Territorygovernments).1.1 PURPOSEThis guide has been developed to assist service providers to prepare and participate in a quality reviewusing the <strong>Community</strong> <strong>Care</strong> <strong>Common</strong> <strong>Standards</strong> (the <strong>Standards</strong>) for ensuring quality in community care.The guide is also used by quality reviewers in conducting quality reviews <strong>of</strong> community care services.<strong>Community</strong> care includes:• the Home and <strong>Community</strong> <strong>Care</strong> (HACC) Program• packaged care programs [<strong>Community</strong> Aged <strong>Care</strong> Packages (CACP), Extended Aged <strong>Care</strong> at Home(EACH) and Extended Aged <strong>Care</strong> at Home Dementia (EACHD)]• the National Respite for <strong>Care</strong>rs Program (NRCP).1.2 GUIDE CONTENTSThis guide is designed to support service providers in responding to the requirements <strong>of</strong> the <strong>Standards</strong>during a quality review <strong>of</strong> their services, and to help them continuously improve. The guide providesinformation in the following areas:• An overview <strong>of</strong> the applicable programs and jurisdictions• Quality review processes for the <strong>Standards</strong>• Information on the <strong>Standards</strong>’ expected outcomes• Examples <strong>of</strong> results and performance measures• Other information• Appendices (including tools and forms).The Appendices to this guide contain: information for quality reviewers; acronyms and glossary; nationalprogram documents and references; quality review documentation, including the self-assessment tool, anexample <strong>of</strong> a completed self-assessment tool, the on-site visit schedule, the quality reviewer tool, thequality review report and the improvement plan. These have been included to provide service providerswith the resources they need to participate in the quality review process and an understanding <strong>of</strong> thedocuments used in the quality review process.1.3 STANDARDS AND EXPECTED OUTCOMES STRUCTUREThe <strong>Community</strong> <strong>Care</strong> <strong>Common</strong> <strong>Standards</strong> comprise three <strong>Standards</strong> and 18 expected outcomes relating tothose <strong>Standards</strong>. Each <strong>of</strong> the <strong>Standards</strong> includes a principle that summarises the intent <strong>of</strong> that Standard,as shown overleaf.COMMUNITY CARE COMMON STANDARDS GUIDE Page | 1
