Looking AheadThere are a number of issues that need to beaddressed in 2012 and in the years to come. Theseinclude but are not limited to:1. Adjuvant treatment for those patients whosepathology report indicate a 50% probability ofBCR (Biochemical recurrence) within 3 years.a. Clinical trials are being established to addresspatients who are at significant risk forrecurrence of prostate cancer following radicalprostatectomy. Unfortunately, there are no trialsthat have reached maturity and offer treatmentoptions for patients that are being treated now.b. Off-protocol clinical trials for patients who, asabove, have a high probability of cancerrecurrence.2. Education of clinicians and the public regardingthe PSA controversy. The United States PreventiveServices Task Force (USPSTF) indicated that “PSAis of no value in decreasing mortality from prostatecancer.”a. Prior to PSA screening, 20% of newly diagnosedprostate cancers had bone metastases at the timeof diagnosis. Now, with PSA screening, theprobability of bone metastases at the time ofdiagnosis is 4%.b. In the decade from 1994-2004 when PSAscreening was prevalent, there was a 40%decrease in prostate cancer mortality. TheNational Cancer Institute estimates thatapproximately 60% of this observed decrease inprostate cancer mortality is due to prostatecancer screening and the use of PSA.3. Clarification of the potential for robotic-assistedradical prostatectomy to increase the incidence ofcancer control as well as establishing differences inquality of life outcomes. At this point there doesnot appear to be any decrease in cancer specificmortality, nor any evidence of increase of quality oflife outcomes with robotic-assisted radicalprostatectomy as compared to conventional openradical prostatectomy.4. We willincrease options for treating prostate cancer withgreater emphasis on older patients with Grade 3prostate cancer for which treatment may not benecessary. Utilization of Active Surveillance willcontinue to be an option for older patients withlow volume Grade 3 disease. Further, efforts toimprove survival in those patients who are at highrisk of recurrence following curative-intenttreatment will be expanded. All of these effortswill increase patient awareness of their disease,standards for follow up and options forintervention if needed.
